[Immunotherapy of intravenous immunoglobulin preparations in neurologic diseases].

Intravenous immunoglobulin (IVIG) has been used in a wide range of neurological conditions. Clinical effect of IVIG has been shown in controlled trials in patients with Guillain-Barré syndrome, chronic inflammatory demyelinating polyneuropathy, multifocal motor neuropathy, dermatomyositis and recently in multiple sclerosis. This article aims to provide clinicians with an overview of the potential benefits of intravenous immunoglobulins in neurological practice.

Persistent hypersensitivity after repeat concussion is associated with chronic cognitive dysfunction.

Persistent sensory hypersensitivity is a common but under-recognized feature of post-concussion syndrome (PCS) following mild traumatic brain injury (mTBI), and may contribute to chronic cognitive complaints. However, the mechanistic relationship between long-term sensory dysregulation and cognitive dysfunction after repeat concussion remains poorly understood. Here, we used an experimental repeated closed-head injury (rCHI-5x), in young adult mouse model targeting the frontal cortex at 8-weeks of age to test whether chronic sensory processing abnormalities contribute to cognitive impairment u

Neurological manifestations of chronic graft versus host disease.

INTRODUCTION: Allogeneic haemopoietic stem cell transplant (allo-HSCT) is an established therapy for many malignant and non-malignant conditions, with an increasing number of uses and indications in recent decades. Complications including graft versus host disease (GvHD) carry significant morbidity and mortality. Chronic GvHD can affect multiple organs, including skin, gastrointestinal, respiratory and eyes. Neurological involvement is less common and can be a diagnostic and therapeutic challenge for patients and clinicians. METHODS: A retrospective analysis was conducted of patients who unde

Pulmonary Hypertension in Chronic Lung Diseases: What Role Do Radiologists Play?

Pulmonary hypertension (PH) is a pathophysiological disorder, defined by a mean pulmonary arterial pressure (mPAP) > 20 mmHg at rest, as assessed by right heart catheterization (RHC). PH is not a specific disease, as it may be observed in multiple clinical conditions and may complicate a variety of thoracic diseases. Conditions associated with the risk of developing PH are categorized into five different groups, according to similar clinical presentations, pathological findings, hemodynamic characteristics, and treatment strategy. Most chronic lung diseases that may be complicated by PH belong

[Signaling pathways controlling skeletal muscle mass].

The skeletal muscle mass accounts for more than 40% of the body weight of healthy adults. The skeletal muscle not only plays an important role in physical activities but also affects the function of other organs as a secretory organ secreting multiple muscle factors. Therefore, it is important to maintain the normal quantity and function of skeletal muscle. Skeletal muscle mass is the basis of skeletal muscle function and is often affected by many factors such as exercise and disease. Resistance exercise training induces increased protein synthesis in skeletal muscle cells, while limb disuse,

Physiological and functional failure in chronic obstructive pulmonary disease, congestive heart failure and cancer: a debilitating intersection of sarcopenia, cachexia and breathlessness.

PURPOSE OF REVIEW: Loss of skeletal muscle mass and cachexia are important manifestations of chronic obstructive pulmonary disease and have been associated with breathlessness, functional limitation and poor prognosis. A number of other life-limiting illnesses, including cancer and chronic heart failure as well as acute conditions seen in ICU such as sepsis, are characteristically associated with cachexia and sarcopenia. These conditions may have respiratory muscle atrophy of sufficient magnitude to contribute to the development of breathlessness and associated functional limitation. The purpo

Epigenetic Regulation Alters Biofilm Architecture and Composition in Multiple Clinical Isolates of Nontypeable Haemophilus influenzae.

Biofilms play a critical role in the colonization, persistence, and pathogenesis of many human pathogens. Multiple mucosa-associated pathogens have evolved a mechanism of rapid adaptation, termed the phasevarion, which facilitates a coordinated regulation of numerous genes throughout the bacterial genome. This epigenetic regulation occurs via phase variation of a DNA methyltransferase, Mod. The phasevarion of nontypeable Haemophilus influenzae (NTHI) significantly affects the severity of experimental otitis media and regulates several disease-related processes. However, the role of the NTHI ph

New definitions and subtypes of chronic prurigo.

During the past few years, chronic prurigo, including prurigo nodularis, has become a focal point of considerable research attention, resulting in enhanced insights into this condition. For over two centuries, the term "prurigo" has been used in dermatology, often applied loosely to describe a range of pruritic, papulous conditions lacking clear diagnostic criteria. In 2017, the term "chronic prurigo" was introduced, delineating chronic prurigo, including prurigo nodularis, as a distinct disease entity characterized by chronic pruritus, a characteristic clinical history, and physical signs suc

Tinnitus update: what can be done for the ringing?

Physicians will frequently encounter patients who report tinnitus. Tinnitus is a symptom whereby there is the perception of sound or sounds in the ear or head in the absence of an external source of sound. Most individuals experiencing tinnitus will have a neutral reaction to the percept, but in a small proportion of patients, tinnitus can be a debilitating symptom. When it causes burden, patients can be affected in multiple different facets of life, including impairment in sleep, hearing cognition and psychological and psychiatric well-being, often resulting in high healthcare utilisation and

Iatrogenic Chronic Abdominal Pain in a Geriatric Patient: A Case Report.

Chronic abdominal pain is a challenging problem in clinical practice, with several pathophysiological mechanisms underlying its aetiologies. This case report presents a geriatric patient with multiple comorbidities who had experienced intermittent abdominal pain for over 10 years. Alarming symptoms were ruled out, and a functional gastrointestinal disorder was determined as the most likely cause. The patient's medical history and previous treatments were thoroughly reviewed, revealing that long-term use of metformin and an oral iron supplement was the iatrogenic symptom triggers. The abdominal

Beyond the Pain: Patterns of Somatic Comorbidity in Patients With High-Impact Chronic Pain Referred to Specialised Interdisciplinary Treatment.

BACKGROUND: Chronic pain frequently co-occurs with multiple somatic comorbidities. However, little is known about how these conditions naturally cluster in patients with high-impact chronic pain. This study aimed to describe somatic comorbidities and identify clusters of co-occurring diseases in patients with high-impact chronic pain referred to specialised interdisciplinary treatment and to examine associations between these clusters and patient-reported physical health, mental health and pain distribution. METHODS: A cross-sectional study was conducted with 10,256 chronic pain patients refe

Co-existence of hyperparathyroidism, hypergastrinaemia and multiple gastric carcinoids is not always due to incomplete expression of the MEN-1 syndrome.

Until recently, the association of primary hyperparathyroidism and gastric carcinoid, with or without hypergastrinaemia, had been considered an incomplete form of multiple endocrine neoplasia type 1. This is because it seemed unlikely that the rare joint appearance of these diseases could occur only by chance. It is now possible to evaluate the pathogenetic involvement of the multiple endocrine neoplasia type 1 gene in many, apparently sporadic, clinical conditions. This is a case report of a female mimicking multiple endocrine neoplasia type 1 due to the presence of hyperparathyroidism, gastr

Experimental and Clinical Biomarkers for Progressive Evaluation of Neuropathology and Therapeutic Interventions for Acute and Chronic Neurological Disorders.

This article describes commonly used experimental and clinical biomarkers of neuronal injury and neurodegeneration for the evaluation of neuropathology and monitoring of therapeutic interventions. Biomarkers are vital for diagnostics of brain disease and therapeutic monitoring. A biomarker can be objectively measured and evaluated as a proxy indicator for the pathophysiological process or response to therapeutic interventions. There are complex hurdles in understanding the molecular pathophysiology of neurological disorders and the ability to diagnose them at initial stages. Novel biomarkers f
