An autopsy case of myotonic dystrophy with mental disorders and various neuropathologic features.

An autopsy case of myotonic dystrophy (MD) is reported. The patient was a 58-year-old male. He presented with muscular weakness and muscular atrophy at the age of 33 and was diagnosed as having MD from myotonic symptoms (i.e. percussion and grip myotonia) at 49 years old. Mental disorders including a delusional hallucinatory state, mental slowness, indifference, and lack of spontaneity as well as visual cognitive impairments were noted at the age of 55. He showed Parkinsonism and died of septic shock. T2-weighted magnetic resonance imaging demonstrated diffuse cortical atrophy with a marked fr…

[Primary suprarenal insufficiency and multiglandular atrophy in a patient affected by myotonic dystrophy].

We present the case of a 27 year old man diagnosed as having myotonic dystrophy (MD) who showed two novelties with respect to those endocrinopathies hitherto described as being associated with MD: primary suprarenal failure and pluriglandular atrophy (thyroid and suprarenal, in addition to the already known testicular type). We describe here the results of a clonic, hormonal and genetic study of the proband and his family (a carrier father and an affected brother). We discuss the possible etiopathogenesis of the picture which, in our opinion, could consist of an abnormality of the AMPc depende…

Deterioration of motor function in myotonic dystrophy and hereditary motor and sensory neuropathy.

In order to obtain more information about the deterioration of motor function in patients with myotonic dystrophy and in patients with hereditary motor and sensory neuropathy, changes in strength and functionality were determined at one year intervals, over a follow-up period of 1 to 5 years. Twenty-five myotonic dystrophy patients and 16 hereditary motor and sensory neuropathy patients participated. Strength measurements were restricted to knee extension and flexion torques measured isokinetically on a dynamometer at two velocities (120 and 60 degrees/s). Functionality was measured as scaled …

[A case of myotonic dystrophy associated with intracardiac conduction abnormalities and aortic regurgitation].

A rare case of myotonic dystrophy with mechanical derangement in a 48-year-old male is reported. The patient was admitted to Fuzisawa City Hospital because of chest discomfort. On physical examination, he had the typical facial appearance of myotonic dystrophy, and displayed grip myotonia. The blood pressure was 120/60 mmHg and the pulse 51, regular. A systolic ejection murmur at the cardiac base, an early diastolic blowing murmur along the left sternal border, and a pansystolic murmur at the apex were heard. The deep tendon reflexes were all normal. Elevation of serum creatine kinase and aldo…

Proof of genetic heterogeneity in the proximal myotonic myopathy syndrome (PROMM) and its relationship to myotonic dystrophy type 2 (DM2).

Recently, myotonic dystrophy type 2 has been described as a separate disease entity that is distinctive from classical Steinert's disease since it lacks a CTG repeat expansion on chromosome 19q. A gene locus for myotonic dystrophy type 2 has been mapped to chromosome 3q. Independently, proximal myotonic myopathy has been recognized as yet another form of a multisystem myotonic disorder. Its relationship to myotonic dystrophy type 2 remains to be clarified. In our linkage study of 17 German proximal myotonic myopathy families nine of them mapped to the myotonic dystrophy type 2 locus (LOD score…

Antisense Oligonucleotide-Mediated Degradation of Toxic RNA in Myotonic Dystrophy Type 1.

Myotonic dystrophy (DM) exists in two forms, type 1 (DM1) and type 2 (DM2), both inherited in an autosomal dominant manner and caused by toxic gain-of-function RNAs produced from unstable repeat expansions. In DM1, the mutation lies in the DMPK gene, whereas in DM2 it involves the CNBP gene. These repeats are inherently unstable, with a strong tendency to further expand in somatic tissues and across generations, driving disease progression and anticipation. Despite arising from distinct genetic defects, DM1 and DM2 share a common pathogenic pathway-RNA-mediated toxicity-which accounts for thei…

Quantitative susceptibility mapping in myotonic dystrophy: clinical relevance of subcortical iron accumulation.

Myotonic dystrophy type 1 is a dominantly inherited disorder, affecting musculoskeletal and central nervous systems and mainly characterized by progressive muscular atrophy and multisystemic damages including cardiac, respiratory and sleep dysfunctions. Neuroimaging studies conducted in myotonic dystrophy type 1 patients have documented widespread cerebral alterations encompassing structural, microstructural, functional and metabolic aspects of the brain, while comparatively few studies have investigated the role of iron concentration in the pathophysiology of central nervous system impairment…

Prevalence and prognostic impact of cardiac resonance abnormalities in myotonic dystrophy patients.

Myotonic dystrophy (DM) is an autosomal dominant inherited neuromuscular disorder characterized by progressive muscular weakness and multisystem involvement. Cardiac involvement is recorded in about 80 % of cases and often precedes the involvement of skeletal muscle. Cardiovascular magnetic resonance (CMR) has been recently included as a recommended test in the cardiac assessment of DM patients at the time of diagnosis, and subsequently with periodic testing. Our systematic review aims to describe the prevalence and prognosis of disease-related CMR abnormalities in DM patients.

Multisystem Symptoms in Myotonic Dystrophy Type 1: A Management and Therapeutic Perspective.

Myotonic dystrophy type 1 (DM1) is a complex, multisystemic neuromuscular disorder with several pathological phenotypes, disease severities and ages of onset. DM1 presents significant challenges in clinical management due to its multisystemic nature, affecting multiple organs and systems beyond skeletal muscle. Tackling this condition requires a comprehensive approach that goes beyond symptom management, particularly considering the complexity of its manifestations and in the delayed diagnosis. In this review we will discuss the multisystem symptoms of DM1 and how this understanding is guiding…

Quantitative Magnetic Resonance Imaging of the Forearm in Myotonic Dystrophy Type 1.

INTRODUCTION: Myotonic dystrophy type 1 is the most prevalent muscular dystrophy in adults, characterized by weakness, impaired functional abilities, and myotonia. However, little is known about the relationship between quantitative MRI measures (fat fraction and T2 relaxation time) and clinical findings of the upper extremity. This study assessed forearm muscle structure in patients with myotonic dystrophy using quantitative MRI and correlated these measures with strength, function, and handgrip myotonia. MATERIALS AND METHODS: Eighteen adults with myotonic dystrophy type 1 underwent MRI usi…

Sudden Death Caused by Bilateral Diaphragmatic Eventration in Myotonic Dystrophy Type 1.

Myotonic dystrophy type 1, or dystrophia myotonica type 1 (DM1), is a multisystem disorder inherited in an autosomal dominant manner. It is caused by a CTG tri-nucleotide expansion in the 3'-untranslated region (3'-UTR) of the dystrophia myotonia protein kinase ( DMPK ) gene. Core clinical features include progressive skeletal muscle weakness, myotonia, and systemic complications, with premature mortality most often due to respiratory or cardiac dysfunction. We report the case of sudden death in a 48-year-old woman with a prior clinical diagnosis of DM1, who had not undergone adequate neuromus…

Analysis of Corneal Phenotypes in Japanese Patients With Myotonic Dystrophy Type 1.

PURPOSE: To analyze the corneal phenotypes of Japanese patients with myotonic dystrophy type 1 (DM1). METHODS: We included patients with DM1 who were diagnosed with clinical neuromuscular symptoms by neurologists and CTG trinucleotide repeat (TNR) expansion of the (myotonic dystrophy protein kinase) DMPK gene. We analyzed the corneal phenotype using slit-lamp examination, specular microscopy, and anterior segment optical coherence tomography. We evaluated TNR expansion in the TCF4 gene of leukocyte-derived genomic DNA by fragment analysis using polymerase chain reaction and triplet-repeat pri…

In Myotonic Dystrophy Type 1 Head Repositioning Errors Suggest Impaired Cervical Proprioception.

Background: Myotonic dystrophy type 1 (DM1) is a rare multisystemic genetic disorder with motor hallmarks of myotonia, muscle weakness and wasting. DM1 patients have an increased risk of falling of multifactorial origin, and proprioceptive and vestibular deficits can contribute to this risk. Abnormalities of muscle spindles in DM1 have been known for years. This observational cross-sectional study was based on the hypothesis of impaired cervical proprioception caused by alterations in the neck spindles. Methods: Head position sense was measured in 16 DM1 patients and 16 age- and gender-matched…

Cognitive phenotype of childhood myotonic dystrophy type 1: A multicenter pooled analysis.

INTRODUCTION/AIMS: There is clear evidence for brain involvement in childhood myotonic dystrophy type 1 (DM1) from imaging studies and the prevalence of intellectual impairment and neurodevelopmental disorders. The cognitive profile of children with DM1 however is poorly understood. The aim of this study was to assess the cognitive profile of children with DM1. METHODS: Neuropsychological examination reports of 45 children aged 2-17 y were analyzed. All cognitive subtests used in this cohort were pooled in 10 cognitive domains. For every patient a composite z-score was calculated for every as…

Acute cricopharyngeal achalasia after general anesthesia in myotonic dystrophy: A case report.

RATIONALE: Myotonic dystrophy type 1 (DM-1) is a progressive multisystem genetic disorder that causes myotonia and both distal limb and facial/neck muscle weakness by expanding the CTG repeats of the DMPK gene in chromosome 19q13.3. General anesthesia is indicated in DM-1 patients owing to their sensitivity to anesthetic drugs such as opioids, hypnotics, and neuromuscular blocking agents. PATIENT CONCERNS: A 48-year-old male patient underwent a laparoscopic cholecystectomy for gallstones under general anesthesia. He experienced sudden cardiac arrest and respiratory failure the day after surge…

Global Prevalence of Myotonic Dystrophy: An Updated Systematic Review and Meta-Analysis.

INTRODUCTION: Myotonic dystrophy (DM), the most common muscular dystrophy in adults, is a group of autosomal inherited neuromuscular disorders characterized by progressive muscle weakness, myotonia, and cardiac conduction abnormalities. Due to the different gene mutations, DM has been subclassified into DM type 1 (DM1) and type 2 (DM2). However, the prevalence studies on DM and its subtypes are insufficient. METHODS: The PubMed (1966-2022), MEDLINE (1950-2022), Web of Science (1864-2022), and Cochrane Library (2022) databases were searched for original research articles published in English. …

Unexpected diagnosis of myotonic dystrophy type 2 repeat expansion by genome sequencing.

Several neurological disorders, such as myotonic dystrophy are caused by expansions of short tandem repeats (STRs) which can be difficult to detect by molecular tools. Methodological advances have made repeat expansion (RE) detection with whole genome sequencing (WGS) feasible. We recruited a multi-generational family (family A) ascertained for genetic studies of autism spectrum disorder. WGS was performed on seven children from four nuclear families from family A and analyzed for REs of STRs known to cause neurological disorders. We detected an expansion of a heterozygous intronic CCTG STR in…

Child Neurology: Maternal Transmission of Congenital Myotonic Dystrophy Type 2: Case Report.

Congenital manifestations in Myotonic Dystrophy type 2 (DM2) point to anticipation and have only rarely been described. We report a three-generation family with genetically confirmed DM2. The youngest family member presented with unilateral congenital pes planovalgus and equinus. Genetic analysis in four family members showed a CCTG repeat expansion in the CNBP gene. We highlight the association between foot deformities and congenital DM2. Remarkably, the transmission to the congenital form of DM2 has been exclusively maternal so far. If this association is confirmed in other families, clinica…
