Coexistence of Fungal Infections in Psoriatic Nails and their Correlation with Severity of Nail Psoriasis.

BACKGROUND: Nail involvement in psoriasis is often complicated by concomitant fungal infections. The aim of this study was to investigate th … dex (NAPSI) score. The nail clippings from the the least and most severely involved psoriatic fingernails and toenails were cultured to determine the presence of coexisting fungal infections and isolate the fungal species. RESULTS: Sixty-two patients (33 males, 29 females) fulfilling the inclusion criteria were included in the study. The mean age at th …

Use of a Nancy nail to remove a broken intramedullary nail: A technical note.

Intramedullary (IM) nailing is a standard surgical technique for treating long bone diaphyseal fractures. However, one complication is breakage of the IM nail. Many methods have been reported for removing broken nails. We devised another technique, using a Nancy nail, for removing a broken IM nail and report on the surgical technique and a case involving the use of our method.

Nail dystrophy in psoriatic arthritis presenting as nail onychomycosis: a case report.

Dystrophic nail changes without a skin lesion is a rare form of psoriasis. We describe a patient that presented with a chronic nail onychomycosis and a destructive thumb interphalangeal joint which was initially considered as onychomycotic joint infection, and was later diagnosed as having dystrophic nail changes in psoriatic arthritis and coexistent onychomycosis. The nail changes were completely normalized after methotrexate treatment.

[Intramedullary nailing of the distal tibia illustrated with the Expert(TM) tibia nail].

OBJECTIVE: Restoration of axis, length, and rotation of the lower leg. Sufficient primary stability of the osteosynthesis for functional aftercare and to maintain joint mobility. Good bony healing in closed and open fractures. INDICATIONS: Closed and open fractures of the tibia and complete lower leg fractures distal to the isthmus (AO 42), extraarticular fractures of the distal tibia (AO 43 A1/A2/A3), segmental fractures of the tibia with a fracture in the distal tibia, and certain intraarticular fractures of the distal tibia without impression of the joint line with the use of additional im

Proximal humeral fractures and intramedullary nailing: Experience with a new nail system.

OBJECTIVES: The goal of this study was to evaluate fracture healing and alignment as well as functional outcome and complication risks after internal fixation with the intramedullary proximal humeral nail (PHN). This device shows promise for applications involving the reconstruction of the humeral shaft and head with minimal soft tissue stripping and for providing a locked, fixed-angle construct for secure fixation to permit controlled, early, and active rehabilitation. DESIGN: Prospective case-series. SETTING: Multicenter study in 11 trauma units. PATIENTS: One-hundred and fifty-one patien

Nail Psoriasis Severity Index: a useful tool for evaluation of nail psoriasis.

The Nail Psoriasis Severity Index (NAPSI) is a numeric, reproducible, objective, simple tool for evaluation of nail psoriasis. This scale is used to evaluate the severity of nail bed psoriasis and nail matrix psoriasis by area of involvement in the nail unit. The NAPSI will be useful during clinical trials for evaluating response to treatment of psoriatic nails. The scale is reproducible, and because there are few data points, statistical analysis is simplified.

Twenty-nail dystrophy. A clinical manifestation of spongiotic inflammation of the nail matrix.

Twenty-nail dystrophy is a distinctive pattern of nail changes, in which all 20 nails are uniformly affected with excessive longitudinal ridging and loss of luster. Because of the rarity of this disorder, and controversy surrounding its cause and pathogenesis, we share our experience with five patients whose clinical diagnoses were consistent with 20-nail dystrophy. Histologically, all patients showed distinctive changes of spongiotic inflammation of the nail matrix. The cause and pathogenesis of these changes are still unknown. We suggest the concept that they represent a subgroup of endogeno…

[Sandy nails and twenty-nail dystrophy of childhood. Apropos of 2 cases].

2 apparently idiopathic cases of twenty-nail dystrophy of childhood are reported with follow-ups of 4 and 16 years, respectively. The associated koilonychia appears to be only fortuitous and related to the fragility of the nail. Trachyonychia should be considered as a symptom often related to external traumatism or dermatoses, particularly alopecia areata or lichen planus, of which it could be a specific isolated location. Within the spectrum of apparently idiopathic trachyonychies , the twenty-nail dystrophy of childhood appears to be a real entity on the basis of its clinical features and co

Comparison of hindfoot arthrodesis nail and humeral intramedullary nail for tibiotalocalcaneal arthrodesis.

BACKGROUND: Tibiotalocalcaneal (TTC) arthrodesis is a standard procedure for severe hindfoot deformities. While both hindfoot arthrodesis nails (HAN) and repurposed humeral intramedullary nails (HIN) are used for fixation, direct comparisons of their outcomes are scarce. PURPOSE: This study aimed to directly compare the surgical, clinical, and functional outcomes of HAN and HIN fixation in TTC arthrodesis. STUDY DESIGN: Retrospective comparative cohort study. METHODS: This retrospective cohort study included 55 patients who received TTC arthrodesis from 2019 to 2023. Based on the implant us…

"Preventing nail migration by stack nailing in forearm fractures" - A technical note.

Diaphyseal forearm fractures are typically treated with open reduction and plate fixation, which has been proven to be the most effective method in numerous trials. However, the risk of periosteal stripping, haemorrhage evacuation, nonunion, and infection is always present and may cause a delayed union and other surgical consequences. Although intramedullary nails have been mentioned in the literature for forearm bone fractures, issues regarding their common complication of nail migration, especially of the ulnar fracture, before the fracture has been consolidated is an issue, and has not been…

Nail bed trauma reconstruction and artificial nail replacement - a case report.

Nail bed reconstruction is crucial after fingertip trauma, impacting both function and aesthetics. In this article, the authors describe a case of partial distal phalanx amputation of the index finger with laceration of the nail bed's remaining part. A traumatically elevated skin-fat flap covered the exposed bone on the fingertip, preserving finger length and sensitivity on the radial side. A full-thickness skin graft from the forearm closed a secondary defect on the finger pulp. Nail bed suturing prevented scarring and nail deformity, and a temporary artificial plastic nail replacement mainta

Nail Salon Safety: From Nail Dystrophy to Acrylate Contact Allergies.

Nail cosmetics are an integral practice in many patients' lives. However, the manicuring process can result in nail damage via instrumentation, allergens in nail polish, and infections. Many of these nail disorders are preventable through proper education. Therefore, it is critical for physicians to understand the steps involved in regular, gel, and acrylic manicures and educate patients on how to protect their natural nails. Simple preventative measures can be discussed with patients and make a substantial difference in their long-term nail health.

Painful nails: A practical approach to the diagnosis and management of painful nail conditions.

Because several nail disorders share similar clinical features, their diagnosis and management can be challenging to clinicians. The physical examination may disclose localized abnormalities or point to an underlying systemic disease, requiring additional workup. Furthermore, cosmetic distress and nail-related symptoms (e.g., tingling, stinging, numbness, and pain) are common factors that influence the patient's search for medical assistance. Nail pain (i.e., onychalgia) can accompany both localized and systemic pathology. Onychalgia can be acute or chronic according to the time of evolution;

Therapeutics for Adult Nail Psoriasis and Nail Lichen Planus: A Guide for Clinicians.

Nail psoriasis (NP) and nail lichen planus (NLP) can be limiting, stigmatizing and difficult to treat. Dermatologists commonly treat psoriasis and lichen planus but when associated onychodystrophy is present or is an isolated finding, some develop apprehension. The goal of this review is to develop therapeutic ladders to be used as a guide for the management of NP and NLP in everyday clinical practice. Evidence-based therapies for NP are robust and range from topical treatments to conventional systemic therapies (i.e., methotrexate, cyclosporine), new oral agents (i.e., apremilast and tofaciti
