OBJECTIVES: To evaluate the incidence rate of nasopharyngeal carcinoma with dermatomyositis, the influence of dermatomyositis on clinical course, and complications and survival of patients with nasopharyngeal carcinoma after radiotherapy.
DESIGN: A retrospective study of 12 patients with nasopharyngeal carcinoma associated with dermatomyositis, with a maximum follow-up of 228 months.

A 73-year-old man was admitted to our hospital with nasal hemorrhage and high grade fever on Aug, 1992. Xylitol is known to reduce caries by inhibiting the growth of Streptococcus mutans. We hypothesized that xylitol could also affect the growth of other nasopharyngeal bacterial flora, which could be important when considering respiratory infections caused by these bacteria. We studied this in vitro by adding xylitol to the medium and observed that 1 and 5% xylitol reduced markedly the growth of alpha-hemolytic streptococci, including S. pneumoniae. Combined radiotherapy and chemotherapy have attained good response rates in the treatment of nasopharyngeal cancer (NPC). Despite excellent response rates to initial treatment, however, five year survival rates are reported to be poor. The reason for this poor prognosis is the high rates of local recurrence and distant metastasis. It is therefore thought to be important to detect local recurrence as early as possible. Nasopharyngeal carcinoma is a tumor that is uncommon in children and adolescents. Presenting symptoms may be subtle and may include epistaxis, neck pain, hearing impairment, otorrhea, rhinorrhea, and, most likely, painless cervical adenopathy. The tumor is curable, but late diagnosis makes this possibility much less likely. Diagnosis of this tumor requires a high index of suspicion on the part of clinicians treating the pediatric population.

A case of extensive osteoradionecrosis of the posterior maxillae, pterygoid plates and floor of the sphenoid sinuses is reported. The patient received more than one full course of radical radiotherapy for nasopharyngeal carcinoma. Although there was complete remission of the tumour, she suffered from repeated episodes of gum bleeding which was found to be due to oroantral fistulas and severe dental decay. The large fraction dose and total dose, severe dental caries and xerostomia are the most likely contributory causes to this unusual complication in our patient.

Nasopharyngeal-like carcinoma is an extremely rare tumor when localised in the laryngopharyngeal mucosa. Only 10 of such tumors could be found by the AA. in the medical literature. They report this additional case in which the radiotherapy plus the chemotherapy have controlled, till now, the primary disease and its spreading to the cervical lymph nodes.

UPPP has emerged over the past decade as a surgical procedure to improve or possibly eliminate Obstructive Sleep Apnea. The procedure itself is not uniformly successful and the technique continues to undergo evolution. Most complications occur in the acute post-operative period and are not unusual. Fifty-three patients with locally recurrent or persistent nasopharyngeal carcinoma were re-treated with megavoltage radiation therapy at The University of Texas M. D. Anderson Cancer Center from 1954 through 1989. The time from initial treatment to re-treatment ranged from 2 to 189 months (median 33 months). Magnetic resonance (MR) images of 9 patients with a small nasopharyngeal tumor were examined, and among them three cases were evaluated with Gd-DTPA enhancement technique. A nasopharyngeal mass with cranial neuropathies usually indicates an advanced neoplastic process. We present three patients with these findings and concurrent invasive Pseudomonas otitis in whom repeated nasopharyngeal biopsies were negative for tumor. All of the nasopharyngeal masses resolved following treatment of the otitis. Mechanisms of disease spread from the temporal bone to the nasopharynx are discussed. Clinicians may choose to modify diagnostic and therapeutic approaches to the nasopharyngeal mass in patients with concurrent invasive otologic disease.

Experimental evidence suggests that altered muscular function can influence craniofacial morphology. The switch from a nasal to an oronasal breathing pattern induces functional adaptations that include an increase in total anterior face height and vertical development of the lower anterior face. While some animals studies have suggested predictable growth patterns may occur, studies in human subjects have been much more controversial. Therefore, individual variations in response should be expected from the alteration of a long face syndrome patient's breathing mode.

Craniopharyngioma is an intracranial tumor that occurs rarely in the infrasellar region. Eight patients with craniopharyngioma located within the nasopharynx have been reported previously. Human nasopharyngeal mucosa from 22-cases of chronic nasopharyngitis was transplanted into 26 nude mice. The xenografts were examined on 15, 30, 45 and 60 days after transplantation, and found to have survived in 19 mice. The survival rate was 73.1 per cent. Evidence is presented of the effectiveness and relative lack of serious toxicity of external beam megavoltage radiation therapy (RT) as primary treatment for juvenile nasopharyngeal angiofibroma. Eight patients with distant metastases from nasopharyngeal carcinoma are presented. In four the first evidence of distant metastases and generalised disease was a deposit in the soft tissue of the forehead. In seven of the eight cases the primary tumour at presentation had been a locally advanced tumour involving bone or cervical lymph nodes, either bilateral or fixed. Seven of the eight patients were under 25 years of age at the time of presentation. The distribution of metastatic carcinoma of the nasopharynx in some other recorded series is discussed.
