Current opinions in nephrology and hypertension: kidney transplantation in patients with plasma cell dyscrasias.

PURPOSE OF REVIEW: Plasma cell dyscrasias encompass a group of hematological disorders characterized by increased production of immunoglobulins by clonal B cells. Kidney involvement is common. Significant advances in the treatment of plasma cell dyscrasias have resulted in improved survival and may permit kidney transplantation in candidates previously denied transplantation. Treatments may also have effects on kidney transplant recipients who develop plasma cell dyscrasias post transplantation. RECENT FINDING: The available evidence suggests that transplantation of candidates with nonmultipl

[The treatment of AKI in nephrology hospitalization: the SLE-HDF 15 litres in 10 hours].

The AKI in intensive care has been widely treated by international and national guidelines. The treatment of AKI in patients not requiring admission in Intensive Care Unit, but often hospitalized in Nephrology Unit, it is showed of less relevance. For over 5 years we have used for the treatment of AKI of patients admitted in Nephrology Unit an intermittent slow technique, implemented in approximately 600 patients with AKI for a total of about 3000 treatments. In this study we report the clinical results obtained in 100 consecutive patients referred to our Nephrology Unit from 1st January 2014.…

[Management of side effects of targeted therapies in renal cancer: nephrological side effects].

Several types of nephrological side-effects can occur during treatment with targeted therapy: high blood pressure, proteinuria, thrombotic m … e, which interfere with cytochrome P450 (verapamil and diltiazem). Specialist advice (cardiology or nephrology) is recommended in the event of uncontroled hypertension. It is essential to monitor proteinuria with a urine strip test: if proteinuria is less than 2+ (grade 1), maintain treatment with molecular targeted therapy; if proteinuria is greater or equa …

Electron-beam CT as a diagnostic modality in pediatric nephrology and renal transplant surgery.

Electron-beam computed tomography is an imaging technology with a variety of medical applications, primarily in cardiology due to its sub-second acquisition time enabling visualization of a beating heart. Recently, this technique has also been introduced into other fields because of lower radiation exposure compared to traditional computed tomography, as well as the strengths of post-procedural three-dimensional visualization. This report evaluates electron-beam computed tomography as a diagnostic modality in pediatric nephrology patients. Seven patients reflecting typical clinical scenarios i…

Measurement of erythropoietin in anephric children. A report of the Southwest Pediatric Nephrology Study Group.

Serum erythropoietin (Ep) levels were measured using a highly sensitive radioimmunoassay in 69 children undergoing chronic dialysis; 31 were anephric, whereas 38 were non-nephrectomized (nephric). Twenty-nine normal children were studied as controls. Serum Ep levels in the anephric group were much higher than anticipated (mean 19.7 +/- 1.8 mU/ml), albeit significantly lower than those measured in normal children (mean 26.2 +/- 2.4 mU/ml, P less than 0.05), or in nephric children on dialysis (33.0 +/- 2.9 mU/ml, P less than 0.001). Anephric children on peritoneal dialysis (PD) had significantly

[Urological and nephrological complications after general surgery. Urological complications. Kidney and ureter].

Urologic complications can result from abdominal, gynecologic, and retroperitoneal operations. As a result, surgical treatment of colonic tumors, genital tumors, the vertebral column, the lymphatic system, and the aortoiliacal vessels assumes a greater importance. Immediate therapy is possible as a result of intraoperative diagnosis. The postoperative diagnosis may become manifest as early or late symptoms. Certain criteria for avoiding such complications are mentioned. Following diagnosis of a urologic complication, immediate therapy seems to produce the best results and the shortest period o

Renal survival in hereditary urolithiasis: a monocentric cohort study from a Tunisian nephrology department.

INTRODUCTION: hereditary urolithiasis (HUL) is rare and often underdiagnosed. The genetic type contributes, with other factors, to the onset of renal failure and its progression to end-stage renal disease (ESRD). The aim of our study was to determine the incidence of progression to ESRD and to identify factors influencing renal survival. METHODS: we conducted an analytical retrospective study in our department of nephrology for 31 years [1990-2021]. Records of patients with HUL who were not in ESRD at diagnosis were retained. We performed a Cox proportional hazards model. RESULTS: we include…

Association Between Pre-Dialysis Nephrology Care Duration and Short-Term Outcomes in Hemodialysis Patients.

INTRODUCTION: Despite recommendations for early nephrology referral in CKD stage G4+, emergency dialysis remains common. METHODS: This retrospective study evaluated whether pre-dialysis nephrology care duration influences outcomes after initiation. We analyzed 388 hemodialysis patients (2014-2022), divided into Short (< 3 months) and Long (> 6 months) care groups. Clinical and socioeconomic factors were compared using Fisher's exact test, Mann-Whitney U test, and Kaplan-Meier analysis. RESULTS: The proportion of Short Group patients did not decline over time. Ninety-day survival was signific…

Transition From Pediatric to Adult Nephrology Care: Program Report of a Single-Center Experience.

PURPOSE OF PROGRAM: Adolescents and young adults with chronic disease face many personal and systemic barriers that may impede their successful transition from pediatric to adult care, putting them at risk for treatment nonadherence, loss to follow-up, and poor health outcomes. Such barriers include impaired socioemotional functioning, overreliance on adult caregivers, lack of disease-specific knowledge, and poor coordination between pediatric and adult health care services. In 2007, we established a specialized youth to adult nephrology transition clinic at a tertiary care center to address t…

Contrast-enhanced ultrasound (CEUS) in nephrology: Has the time come for its widespread use?

Grey-scale ultrasound has an important diagnostic role in nephrology. The absence of ionizing radiations and nephrotoxicity, rapidity of execution, excellent repeatability, the possibility to perform the test at the patient's bed and the low cost represent important advantages of this technique. Paired with real-time sonography and colour-power-Doppler contrast-enhanced ultrasound (CEUS) reduces the diagnostic gap with computed tomography (CT) and magnetic resonance (MR) and represents a major step in the evolution of clinical ultrasound. Although there are several situations in which contrast…

Radiation dose associated with dialysis vascular access interventional procedures in the interventional nephrology facility.

The number of dialysis access procedures performed by interventional nephrologists using a mobile C-arm fluoroscopy machine in freestanding centers continues to rise. With this activity comes the risk of radiation exposure to patients being treated and staff. This study was conducted to assess the levels of radiation dosage involved with these procedures. Dosimetry information including kerma area product (KAP), reference point air kerma (RPAK) and fluoroscopy time (FT) was collected prospectively. Radiation dosage data were collected from 24 centers in various parts of the United States and r

Interaction between phosphorus and parathyroid hormone in non-dialysis CKD patients under nephrology care.

BACKGROUND: Whether the traditional treatment of chronic kidney disease (CKD)-mineral and bone disorder is effective in the setting of tertiary nephrology care is an unexplored question. We evaluated phosphate, calcium and PTH levels during the first year of nephrology care and the prognostic role of month-12 levels in non-dialysis patients referred prior to availability of the novel P binders. METHODS: We studied a historical cohort of consecutive patients with CKD stage 3-5 at referral (baseline), and after 6 and 12 months of nephrology care; thereafter, patients were followed for renal sur…

Do aluminium-based phosphate binders continue to have a role in contemporary nephrology practice?

BACKGROUND: Aluminium-containing phosphate binders have long been used for treatment of hyperphosphatemia in dialysis patients. Their safety became controversial in the early 1980's after reports of aluminium related neurological and bone disease began to appear. Available historical evidence however, suggests that neurological toxicity may have primarily been caused by excessive exposure to aluminium in dialysis fluid, rather than aluminium-containing oral phosphate binders. Limited evidence suggests that aluminium bone disease may also be on the decline in the era of aluminium removal from d
