Fifty-eight needle biopsies of the kidneys with the light microscopical diagnosis of so-called minimal glomerular lesions were subjected to ultrastructural evaluation. The series contained children and adults under 25 years of age with the mean age of 16.9 years. The most frequent findings were those of the glomerular epithelia: vacuolation and swelling of their cytoplasm in 72.8%, microvilloustransformation in 13.6%. Endothelial lesions were less frequent: swelling and vacuolation of their cytoplasm occurred in 46.6%. The most important ultrastructural lesions appeared to be the mesangial one

Certain types of glomerulonephritis develop typically as the result of focal infection, and may recover following effective treatment of the infective focus at cause. Apart from the classical immunity mechanism consisting of the formation of immune complexes with the bacterial antigen, a major role for environmental allergens must also be evoked, as well as the probable existence of different immunity mechanisms at the origin of certain types of GN (mainly mesangial GN with IgA deposits and lipoid nephrosis). Invasion of the pulmonary allergen through a rhinosinusal route is responsible for a

Clinical and experimental observations suggest that lipoid nephrosis (Minimal change nephrotic syndrome) results from T cell dysfunction due to still unknown mechanisms. By substractive screening library, we identified 84 transcripts, of which 42 correspond to known genes, 12 match with proteins of yet unknown function and 30 are unknown clones. Among the 42 known transcripts, at least 18 are closely involved in the T-Cell Receptor mediated signaling cascades. This includes genes encoding components of the T-Cell Receptor and proteins associated with the cytoskeleton scaffold, as well as trans

Renal involvement in parasitic infections are polymorphic. Plasmodium malariae often leads to membranoproliferative glomerulonephritis whereas acute tubular necrosis or post-infectious acute glomerulonephritis are observed with Plasmodium falciparum. Urogenital taxis of Schistosoma haematobium is responsible for frequency of chronic tubular and interstitial nephritis. Without specific treatment, the renal function progressively deteriorates and urological complications appear. Schistosoma mansoni mainly leads to mesangial and membranoproliferative glomerulonephritis. Membranoproliferative and

Clinico-histological features in 32 patients with nephrotic syndrome (NS) due to focal segmental glomerulosclerosis (FSGS) were examined. Thirteen (group A1) were diagnosed as cases of FSGS within 2 years of the onset of NS, and 8 (61%) showed progressive renal dysfunction. Ten (group A2) developed FSGS more than 2 years after the onset of NS and had a favorable prognosis. Nine (group B) differed from groups A1 and A2 in that the remaining nonsclerosed glomeruli showed slight mesangial proliferation. All but 1 patient of group B developed FSGS within 2 years of the onset of NS, and the prognos

BACKGROUND: In the very large majority of cases, nephrotic syndrome with minimal glomerular lesions is an idiopathic condition. Drugs can favor the glomerulopathy. The effect of non-steroidal antiinflammatory drugs is well known, but other drugs, particularly tiopronin may be incriminated.
CASE REPORT: A 73-year-old patient developed severe nephrotic syndrome with minimal glomerular lesions 6 weeks after tiopronin therapy was initiated. Complete and spontaneous remission of the nephrotic syndrome was achieved 5 weeks after drug withdrawal. No recurrent lipoidic nephrosis has been observed at

Immunofluorescence is a useful technique in the study of human renal diseases, both from the point of view of elucidating pathogenic mechanisms and as a diagnostic tool. The finding of characteristic staining patterns for immunoglobulins and complement indicates that many forms of glomerulonephritis are immune complex diseases and that a few are due to anti-GBM antibodies. On the other hand, in lipoid nephrosis and toxemia of pregnancy, deposits of immunoglobulins and complement are generally absent, indicating that immunologic mechanisms are probably not responsible for these glomerular disea

Levamisole (2.1-3.1 mg/kg twice a week) was administered to 6 children aged between 20 months and 6.5 years for 1 to 4 months. All children suffered form the renal diseases exacerbation (nephrotic syndrome and pyelonephritis--2 children, lipoid nephrosis and pyelonephritis--2 children, pyelonephritis with glomerular reactions--1 child, recurrent pyelonephritis--1 child) due to recurrent respiratory infections levamisole improved both clinical and biochemical parameters of renal disease. Subsequent respiratory infections were milder and less frequent. Transient decrease in thrombocyte count was

OBJECTIVES: To establish a simple-to-use nomogram based on quantification of color Doppler sonography data from a region of interest (ROI) to diagnose minimal change disease (MCD) promptly and non-invasively, and to evaluate the prediction capability of the nomogram.
METHODS: We recruited 564 patients with pathology-proven renal disease who were admitted to our hospital from July 2020 to July 2021 (388 patients in the training dataset and 176 patients in the validation dataset), and their color Doppler sonography data were acquired from a ROI and underwent ipsilateral renal biopsy. The collec

Minimal change nephrotic syndrome (MCNS) is the most common cause of the nephrotic syndrome in children, accounting for 90% of cases under the age of 10 years and more than 50% in older children. It has been proposed that MCNS reflects a disorder of T-lymphocytes. These T cells are thought to release a cytokine - so-called permeability factor - that injures the glomerular epithelial cells. The identity of this permeability factor is still uncertain. Epithelial cell damage may lead to albuminuria in MCNS by altering the metabolism of polyanions, such as heparan sulphates, that constitute most o

BACKGROUND: The pathophysiology of edema in the nephrotic syndrome is controversial. Some investigators believe that sodium retention may result from a primary renal defect that causes an "overfilled" blood volume. In contrast, other authors believe that fluid escapes the vascular compartment due a low oncotic pressure, and sodium retention is a compensatory physiological response to an "underfilled" blood volume. The patients that best fit the "underfilled" hypothesis are children with minimal-change nephrotic syndrome (MCNS).
METHODS: We analyzed critically the available evidence for and ag
