Anatomical study of the transfer of flexor digitorum superficialis nerve branch of median nerve to restore wrist extension and forearm pronation.

OBJECTIVE: To analyze the anatomical variations of the innervation of the flexor digitorum superficialis muscle and to determine if the branch of the median nerve that supply this muscle is connected to the branches to the extensor carpi radialis brevis and the pronator teres muscles, without tension, and how close to the target-muscles the transfer can be performed. METHODS: Fifty limbs of 25 cadavers were dissected to collect data on the anatomical variations of the branches to the flexor digitorum superficialis muscle. RESULTS: This muscle received innervation from the median nerve in the

Restoration of elbow flexion by performing contralateral lateral thoracic and thoracodorsal nerve transfers after experimental musculocutaneous nerve transection.

OBJECT: The immediate transfer of the right lateral thoracic nerve (LTN) and the thoracodorsal nerve (TDN) to the transected left musculocutaneous nerve (MCN), leading to nerve cross-neurotization, was performed in cats to evaluate reinnervation of the biceps brachii muscle (BBM). METHODS: Surgery to produce cross-neurotization of the MCN was performed in 12 cats (treatment group). Transection of the MCN was performed without attempts at neurotization in three cats (control group). Reinnervation of the BBM was assessed by performing electromyography (EMG) 6 months (14 cats) and 26 months (one

Comparison of hand responses to recipient nerve stimulation in sequential variations of supercharged anterior interosseous nerve transfer to the ulnar nerve - Identifying the golden recipient fascicles.

BACKGROUND: In supercharged end-to-side (SETS) anterior interosseous nerve (AIN) transfer for compressive ulnar neuropathy, selecting the optimal recipient fascicle remains challenging owing to weak or absent motor responses. We hypothesized that modifying the procedural sequence would improve intraoperative nerve responsiveness, thereby enhancing the identification of recipient fascicles in the thumb, 4 digits, and hypothenar muscles (3 target muscle groups). METHODS: We retrospectively reviewed 52 patients who underwent SETS AIN transfer from 2019 to 2024. Group 1 (n = 25) followed this seq

Pronator teres nerve branch transfer to the extensor carpi radialis brevis nerve branch for wrist extension reconstruction in proximal radial nerve injury following humeral shaft fractures.

BACKGROUND: Tendon and nerve transfers are used for functional reconstruction in cases of proximal radial nerve injury complicated by humeral fractures in patients who do not show functional recovery after primary nerve repair. The effectiveness of pronator teres (PT) nerve branch transfer to the extensor carpi radialis brevis (ERCB) nerve branch for wrist extension reconstruction was investigated and compared to the results of tendon transfer. METHODS: This study included 10 patients with proximal radial nerve injury, who did not show functional recovery after primary nerve repair at our hos…

In-silico biomechanical simulation of tendon transfers for finger extension in radial nerve palsy.

BACKGROUND: Tendon transfers are commonly used to restore finger extension in patients with radial nerve palsy, but quantitative comparisons of different techniques remain limited. Achieving the correct tension continues to be a surgical art, guided more by experience than by objective standards. In-silico biomechanical simulation may serve as a valuable tool to evaluate and optimize tendon transfer strategies in a more reproducible and quantitative manner. METHODS: We used OpenSim software to simulate four tendon transfers targeting the extensor digitorum communis (EDC): flexor carpi radiali

Axillary to Radial Nerve Transfer for Elbow Extension in the Tetraplegic Patient: A Case Series.

BACKGROUND AND OBJECTIVES: Recovery of upper extremity function is considered the top priority in the tetraplegic population. Midcervical injury often results in paralysis of the hand and loss of elbow extension. Elbow extension is essential for extending one's workspace and facilitating mobility (ie, self-transfer, wheelchair propulsion). Tendon transfers have traditionally been used to restore elbow extension, but the axillary to radial nerve transfer (ARNT) has emerged as a promising alternative. Our objective was to review a case series of single-surgeon outcomes for recovery of elbow exte…

Combined nerve and tendon transfer strategy for the restoration of grasp in tetraplegia; a case report.

INTRODUCTION: By combining nerve and tendon transfer procedures, a more versatile hand function can be expected. Here we report the long-term outcomes of novel, individualized reconstruction strategies using combined nerve and tendon transfer procedures (CNaTT) to restore prehension and grasp in two patients with tetraplegia. CASE PRESENTATION: Two women, 45 years of age, underwent bilateral nerve transfer according to the Bertelli S-PIN (supinator to posterior interosseous nerve transfer) procedure. The grip reconstruction included tendon transfers using brachioradialis to flexor pollicis lo…

The Clinical Outcomes of Spinal Accessory to Suprascapular Nerve Transfer Through a Posterior Approach.

BACKGROUND: Spinal accessory nerve (SAN) to suprascapular nerve (SSN) transfer can restore function to the rotator cuff following brachial plexus injuries. The traditional anterior approach using the lateral branch of the SAN causes denervation of the lateral trapezius limiting shoulder elevation. Suprascapular nerve pathology at the suprascapular notch may be missed resulting in poor reinnervation of the rotator cuff. The posterior approach uses the medial SAN and allows decompression and visualization of the SSN at the notch and nerve transfer coaptation closer to the target muscles with a s…

Management of Birth Brachial Plexus Injury Including Use of Distal Nerve Transfers.

Birth Brachial plexus injury continues to remain a problem despite significant care from obstetricians to prevent it. Many children show spontaneous recovery but a significant proportion do not have adequate recovery. This review article discusses, etiology, assessment, investigations and overall strategy to treat the condition. Surgical strategy consists of primary intraplexal repair as the standard of care but of late the distal nerve transfers used in adult plexus injuries are increasingly being used in infants too. We discuss the history, current usage and pros and cons of distal nerve tra…

Masseter-to-Facial Nerve Transfer for Reanimation of a Patient With Long-Term Facial Paralysis.

Facial paralysis is a condition caused by a wide variety of etiologies, including neurologic, congenital, infectious, neoplastic, systemic, and iatrogenic causes. A patient suffering from long-term facial paralysis, with minimal innervation detected through electroneurography, who was successfully reanimated by performing a masseter-to-facial nerve transfer, was presented in this study. Facial paralysis had been caused after resection of an acquired middle ear cholesteatoma more than 5 years before.

Testing the effectiveness and the contribution of experimental supercharge (reversed) end-to-side nerve transfer.

OBJECTIVE: Supercharge end-to-side (SETS) transfer, also referred to as reverse end-to-side transfer, distal to severe nerve compression neuropathy or in-continuity nerve injury is gaining clinical popularity despite questions about its effectiveness. Here, the authors examined SETS distal to experimental neuroma in-continuity (NIC) injuries for efficacy in enhancing neuronal regeneration and functional outcome, and, for the first time, they definitively evaluated the degree of contribution of the native and donor motor neuron pools. METHODS: This study was conducted in 2 phases. In phase I,

Analyzing cost-effectiveness of ulnar and median nerve transfers to regain forearm flexion.

OBJECTIVE Peripheral nerve transfers to regain elbow flexion via the ulnar nerve (Oberlin nerve transfer) and median nerves are surgical options that benefit patients. Prior studies have assessed the comparative effectiveness of ulnar and median nerve transfers for upper trunk brachial plexus injury, yet no study has examined the cost-effectiveness of this surgery to improve quality-adjusted life years (QALYs). The authors present a cost-effectiveness model of the Oberlin nerve transfer and median nerve transfer to restore elbow flexion in the adult population with upper brachial plexus injury…
