[Herpes zoster duplex associated with Ramsay Hunt syndrome and cervical zoster paresis. A case report].

A 63-year-old Japanese female in an immunocompetent state developed right Ramsay Hunt syndrome and left shoulder pain, and left upper limb motor paresis with herpes zoster (HZ) duplex in the right auricle and left shoulder regions. With her Ramsay Hunt syndrome, neural deafness, tinnitus and vestibular symptoms were observed, and she lacked facial nerve palsy. Cerebrospinal fluid (CSF) findings revealed an increase in lymphocytes (21 cells/μl) and protein content (29 mg/dl), and polymerase chain reaction for varicella-zoster virus DNA in CSF was negative. Electrical stimulation enhances peripheral nerve regeneration after crush injury in rats.

Injured peripheral nerves have the ability to regenerate; however, there is conflicting evidence with regard to whether electrical stimulation (ES) accelerates or hinders neural regeneration. To study the effect of ES on peripheral nerve regeneration following nerve crush injury, 54 Wistar rats were randomly divided into three groups (n=18/group); the control, crush and crush+ES group. Four weeks after surgery, the sciatic functional index (SFI), compound muscle action potential (CMAP) conduction velocity and amplitude in the regenerated nerve, nerve histomorphometry, and levels of myelin prot

Cerebral and spinal somatosensory evoked potentials in children with CNS degenerative disease.

Spinal and cerebral somatosensory evoked potentials to peroneal nerve and median nerve stimulation were recorded in 17 children with CNS degenerative disease and compared with similar potentials obtained in a group of age-matched normal control subjects. Spinal potentials were increased in duration over caudal cord segments and were poorly defined or absent over the rostral cord in some patients. In 12 patients the conduction velocity of the spinal response was slow over spinal cord segments. However, conduction velocity over peripheral nerve and cauda equina was normal in all patients. The sc

Coexisting Inflammatory and Hereditary Neuropathies: A Review.

The concurrent presentation of acquired neuropathy and hereditary neuropathy is both diagnostically challenging and important to recognize. This review focuses on cases of concurrent chronic inflammatory demyelinating polyneuropathy (CIDP) and Charcot-Marie-Tooth (CMT) disease. Two cases of patients diagnosed with both CIDP and CMT1A are presented, and reported cases of CMT1A, CMT1B, and CMTX and CIDP overlap are reviewed. Although clinical history and examination have primacy, electrodiagnostic (EDx) testing and other supportive tests can help identify cases with dual pathology. Features that

Defining the link between peripheral neuronal activity and neuropathic pain: observational study protocol to investigate in vivo neurophysiological properties of nociceptors in patients with chronic neuropathic pain.

INTRODUCTION: Neuropathic pain affects up to 10% of the population and remains difficult to diagnose and treat effectively. Peripheral nociceptor hyperexcitability is thought to drive pain in a substantial subset of patients, yet human studies have not fully defined which nociceptor populations are abnormal or how these abnormalities relate to clinical phenotype, sensory profiling, or genetic mechanisms. Microneurography uniquely enables direct in vivo recording from human C-fibres, offering a mechanistic approach to patient stratification. This observational study investigates the relationshi

Neuromuscular ultrasound in combination with nerve conduction studies helps identify inflammatory motor neuropathies from lower motor neuron syndromes.

BACKGROUND AND PURPOSE: Identifying patients with inflammatory motor neuropathies (IMNs) is warranted since effective treatments are available and the prognosis of these patients differs from that of amyotrophic lateral sclerosis patients. METHODS: Between January 2019 and May 2022, 102 consecutive treatment-naïve lower motor neuron syndrome (LMNS) patients were recruited; these patients were suspected of having multifocal motor neuropathy, pure motor chronic inflammatory demyelinating polyneuropathy or amyotrophic lateral sclerosis with initial lower motor neuron presentation. Neuromuscular

Origin, domain, and dynamics of rate-induced variations of functional refractory period in rabbit atrioventricular node.

Different aspects of the intrinsic regulation of rate-induced variations of functional refractory period of atrioventricular node (FRPN) were studied in isolated rabbit heart preparations. First, the hypothesis that these variations originate from the net interaction between facilitation and fatigue was tested. For a constant fast rate, selective effects of faciliation and of steady-stage fatigue were independently shown to shorten and prolong, respectively, FRPN while their combined effects were shown to result in intermediate changes corresponding to the sum of their individual effects. Participation of fast and slow A-V nodal pathways in tachycardias complicating the Wolff-Parkinson-White syndrome. Report of a case.

Electrophysiological studies in one patient with type B pre-excitation and dual A-V nodal pathway revealed several types of paroxysmal narrow QRS tachycardia (PSVT). One type of PSVT reflected antegrade fast A-V nodal pathway and retrograde anomalous pathway conduction. This PSVT was characterized by early retorgrade activation of right atrial appendage, P following QRS and cycle length of 290 to 350 msec. A second PSVT reflected antegrade slow A-V nodal pathway and retrograde anomalous pathway conduction. This PSVT was characterized by early retrograde activation of right atrial appendage, P

Ultrasonographic, Electrophysiological and Clinical Profiles of Motor and Motor Predominant Chronic Inflammatory Demyelinating Polyneuropathy.

INTRODUCTION/AIMS: The distinct ultrasonographic and electrophysiological features separating typical chronic inflammatory demyelinating polyneuropathy (CIDP) from its motor-predominant and pure motor variants remain poorly defined. We aimed to characterize the ultrasonographic, electrophysiological, and clinical features of these CIDP subtypes within a Chinese cohort. METHODS: Between March 2015 and January 2025, we prospectively enrolled consecutive patients meeting the 2021 European Academy of Neurology/Peripheral Nerve Society (EAN/PNS) diagnostic criteria for CIDP. Paroxysmal nonreentrant supraventricular tachycardia due to simultaneous fast and slow pathway conduction in dual atrioventricular node pathways.

Electrophysiologic studies were performed on a 49 year old woman who had paroxysmal nonreentrant supraventricular tachycardia due to simultaneous anterograde conduction through dual atrioventricular (AV) node pathways. Slow pathway conduction was inversely related to the preceding sinus cycle length and fast pathway conduction was determined by the Hs-A interval (measured from the His potential due to slow pathway conduction to the onset of the subsequent atrial electrogram). Major determinants of sustained simultaneous anterograde fast and slow pathway conduction during sinus rhythm were 1) a

Effects of 4-aminopyridine on normal and demyelinated mammalian nerve fibres.

We have previously demonstrated that a drug which prolongs action potentials can, like a reduction in temperature, overcome conduction failure in demyelinated nerve fibres. Although the particular substance then used, a scorpion venom, was not a suitable therapeutic agent, we suggested that other drugs, with similar but milder effects on the action potential, might be effective in the symptomatic treatment of multiple sclerosis. We now report some encouraging results obtained with 4-amino-pyridine (4AP), a substance which blocks the voltage-dependent potassium current in squid giant axons. The

Effect of treatment of high fat fed/low dose streptozotocin-diabetic rats with Ilepatril on vascular and neural complications.

We have previously shown that treating streptozotocin-induced diabetic rats, an animal model of type 1 diabetes, with Ilepatril (an inhibitor of neutral endopeptidase and angiotensin converting enzyme (ACE)) improves vascular and neural functions. In this study we sought to determine the effect of Ilepatril treatment of high fat fed/low dose streptozotocin-diabetic rats, a model for type 2 diabetes, on vascular and neural complications. Following 8 weeks on a high fat diet rats were treated with 30 mg/kg streptozotocin (i.p.) and after 4 additional weeks a group of these rats was treated for 1

Blocking CTGF/CCN2 reverses neural fibrosis and sensorimotor declines in a rat model of overuse-induced median mononeuropathy.

Encapsulation of median nerves is a hallmark of overuse-induced median mononeuropathy and contributes to functional declines. We tested if an antibody against CTGF/CCN2 (termed FG-3019 or Pamrevlumab) reduces established neural fibrosis and sensorimotor declines in a clinically relevant rodent model of overuse in which median mononeuropathy develops. Young adult female rats performed a high repetition high force (HRHF) lever-pulling task for 18 weeks. Rats were then euthanised at 18 weeks (HRHF untreated), or rested and systemically treated for 6 weeks with either an anti-CCN2 monoclonal antib

Suppression of axonal conduction by sinusoidal stimulation in rat hippocampus in vitro.

Deep brain stimulation (DBS), also known as high frequency stimulation (HFS), is a well-established therapy for Parkinson's disease and essential tremor, and shows promise for the therapeutic control of epilepsy. However, the direct effect of DBS on neural elements close to the stimulating electrode remains an important unanswered question. Computational studies have suggested that HFS has a dual effect on neural elements inhibiting cell bodies, while exciting axons. Prior experiments have shown that sinusoidal HFS (50 Hz) can suppress synaptic and non-synaptic cellular activity in several in

Conduction block of mammalian myelinated nerve by local cooling to 15-30°C after a brief heating.

This study aimed at understanding thermal effects on nerve conduction and developing new methods to produce a reversible thermal block of axonal conduction in mammalian myelinated nerves. In 13 cats under α-chloralose anesthesia, conduction block of pudendal nerves (n = 20) by cooling (5-30°C) or heating (42-54°C) a small segment (9 mm) of the nerve was monitored by the urethral striated muscle contractions and increases in intraurethral pressure induced by intermittent (5 s on and 20 s off) electrical stimulation (50 Hz, 0.2 ms) of the nerve. Cold block was observed at 5-15°C while heat block

Patterns of bulbar respiratory neurons during and after artifical hyperventilation.

Single bulbar respiratory unit activity was recorded in urethane-anesthetized rabbits. During artificial ventilation, reflex contractions of the diaphragm were elicited by the forced deflations of the respirator. Out of 44 neurons, four I, one E and two EI units exhibited a shift of their discharge relative to the contractile phase of the diaphragm, compared to normal respiration. During hypocapnic apnea, some I, IE and E neurons became silent. Other cells belonging to all phase types, however, continued to fire tonically at a rate less than the peak rate in normal bursts. When rhythmic respir

Classification of naturally evoked compound action potentials in peripheral nerve spatiotemporal recordings.

Peripheral neural signals have the potential to provide the necessary motor, sensory or autonomic information for robust control in many neuroprosthetic and neuromodulation applications. However, developing methods to recover information encoded in these signals is a significant challenge. We introduce the idea of using spatiotemporal signatures extracted from multi-contact nerve cuff electrode recordings to classify naturally evoked compound action potentials (CAP). 9 Long-Evan rats were implanted with a 56-channel nerve cuff on the sciatic nerve. Afferent activity was selectively evoked in t

[Vagal thermoreceptors in the gastro-intestinal area. Their role in the regulation of the digestive motility (author's transl)].

In anaesthetized cats, sensory vagal units were recorded in the nodose ganglion by means of extracellular glass microelectrodes. In the antrum and the duodenum we have found receptors tonically activated by warm (38-51 degrees C with an optimum at 46-49 degrees C) or cold (36-10 degrees C with an optimum at 12-10 degrees C) solutions. These receptors did not respond to mechanical stimuli (compression and distension of the viscera) and to chemical ones (perfusion with glucose and acid solutions). Thus they did not belong to polymodal type, but they must be considered as true thermoreceptors, sp

Some electrophysiological properties of neurones of rat locus coeruleus.

1. Electrical activity of neurones of the locus coeruleus (LC) was studied in rats anaesthetized with urethane. By stimulating the dorsal pathway (DP) of axons of LC neurones in the mid=brain and observing field responses in the dorsolateral tegmentum of the pons, micro-electrodes were oriented to record unit discharges of LC neurones. They were evoked by DP stimulation mainly during the negative wave of the field response. 2. In the extraceullar records of spike discharges of LC meurones A and B spikes were distinguished. Very often the third component (C spike) was observed to ride on the de
