[Surgical treatment for trigeminal neuralgia].

We treated 50 patients at the Hospital de Especialidades, Centro Médico Nacional La Raza, IMSS, in Mexico City, with trigeminal neuralgia by two procedures: a) microvascular decompression of the trigeminal nerve with asterional craniectomy, or b) compression of Gasser's nodule by percutaneous puncture. Each patient was allowed to choose one of the procedures after informed consent. Twenty two patients underwent percutaneous puncture, while 28 patients underwent microvascular decompression. Our study group comprised 35 females and 15 males between the ages of 38 and 80 years. After 3 months, we…

Trigeminal neuralgia: pathology and pathogenesis.

There is now persuasive evidence that trigeminal neuralgia is usually caused by demyelination of trigeminal sensory fibres within either the nerve root or, less commonly, the brainstem. In most cases, the trigeminal nerve root demyelination involves the proximal, CNS part of the root and results from compression by an overlying artery or vein. Other causes of trigeminal neuralgia in which demyelination is involved or implicated include multiple sclerosis and, probably, compressive space-occupying masses in the posterior fossa. Examination of trigeminal nerve roots from patients with compressio…

[Operative treatment of trigeminal neuralgia].

THERAPY: Nowadays the theory of microvascular compression of the root entry zone of trigeminal nerve in case of trigeminal neuralgia is widely accepted. Pathological contacts between a-beta fibers and c-fibers are believed to be caused by an arterial compression in the root entry zone. It is these ephaptic contacts that have led to a logical therapeutic sequence of treatment by means of membrane stabilising agents like anticonvulsant drugs as carbamazepine, nonablative - and, in view of the pathophysiological concept, causal - surgery in form of microvascular decompression described by Jannett…

Trigeminal neuralgia associated with sinusitis.

When a patient presents with trigeminal neuralgia, one usually thinks of a vascular loop at the root entry zone of the nerve and consequently of vascular decompression. An image of sinusitis on the MRI may be considered an incidental finding. We present a case of an elderly woman who experienced severe neuralgic pain in the distribution of the trigeminal nerve on the left side following a mild upper respiratory tract infection. Routine MRI revealed severe sinusitis with no pathology in the brain. Following antibiotic treatment for the sinusitis, the symptoms of the neuralgia resolved completel…

Surgical treatment of trigeminal neuralgia.

Patients who had uncontrolled pain with carbamazepine therapy and those who suffered adversely from its side effects were selected for this safe, simple and repeatable surgery. Pain-free periods were achieved for at least 2 years. Functional difficulty was not experienced in the affected numb areas. When it recurred, pain was less intense and was better controlled with a reduced dosage of carbamazepine. In four cases, after avulsion of the involved nerves, a suitable-sized titanium screw was inserted into the mental and infra-orbital foramina. Obturation of the foramen prevented nerve regenera

Treatment options in postherpetic neuralgia.

Postherpetic neuralgia (PHN) is a separate disease entity that represents a complication of acute herpes zoster. PHN, involving aberrant somatosensory processing in the peripheral and/or central nervous system, is considered to be a chronic neuropathic pain, frequently unresponsive to all treatment modalities. Despite the clinical trial data demonstrating successful pain relief with several drug regimens, the pharmacologic management of neuropathic pain is difficult, particularly in PHN. Response to therapy is generally inhomogeneous. Some patients experience long-term pain control with either…

Trigeminal neuralgia: mechanisms of treatment.

BACKGROUND: Mechanisms of the surgical treatment of trigeminal neuralgia are considered. RESULTS: Trigeminal neuralgia is effectively treated by microvascular decompression (MVD) and other surgical procedures. These procedures often cause reactivation of herpes simplex virus (HSV), which is latent in trigeminal ganglion neurons. CONCLUSION: MVD and other surgical procedures alter ganglion neuron transcription, as indicated by HSV reactivation. Controlled injury of the trigeminal root-ganglion probably occurs with the disparate surgical procedures, and this is likely the means of their effect…

Surgical management of geniculate neuralgia.

BACKGROUND: Geniculate ganglion or nervus intermedius neuraigia is an unusual condition resulting in deep ear pain with or without signs of atypical trigeminal neuralgia, deep face, or throat pain. This article describes an experience with 14 patients who came to the neurosurgical service at the University of Pittsburgh Medical Center with a diagnosis of geniculate neuralgia. METHODS: After failing conservative treatment and after undergoing neurologic, otologic, and dental evaluations, these 14 patients underwent 20 intracranial procedures consisting of retromastoid craniectomies with microv…

[Microvascular decompression in trigeminal neuralgia].

30 cases of trigeminal neuralgia (TN) were operated between 1988-1995. 26 underwent microvascular decompression (MVD) and 4 others had partial rhizotomies. 87% of the patients became and remained pain-free, while 13% had only partial relief of pain. Permanent side-effects included unsteadiness of gait in 1 and hearing loss on the operated side, in another. There was no serious morbidity or mortality. In the same period, 119 patients underwent retrogasserian glycerol injections for TN. MVD is recommended as an effective procedure in those under 65 years of age, in whom there is no medical contr…

Epidural morphine for postherpetic neuralgia.

Eleven patients with established postherpetic neuralgia, unresponsive to antidepressant therapy, entered this single-blind, placebo-controlled study to assess the effectiveness of epidural morphine in the control of the pain of postherpetic neuralgia. Two patients obtained a reduction in pain of greater than 50% following morphine 0.5 mg. The duration of this pain relief was 36 h in one patient and 72 h in the other. Repeated doses, however, were ineffective in one patient and resulted in intolerable side effects in the other. The other six patients who received morphine developed opioid-relat…

Glossopharyngeal neuralgia with cardiac syncope.

Glossopharyngeal Neuralgia is an uncommon craniofacial pain syndrome that is occasionally associated with cardiac syncope. Involvement of the glossopharyngeal nerve may be painless or may be marked by true episodic neuralgia, and this justifies the term neuralgia reported here. We present 5 cases of this uncommon syndrome, of a total of 15 observed cases of glossopharyngeal neuralgia, successfully treated by section of the rootlets of Cranial Nerves IX and X or by microvascular decompression in the posterior cranial fossa. We also analyze the relevant literature and discuss the pathogenesis an…
