When to consider thyroid dysfunction in the neurology clinic.

BACKGROUND: To investigate the distribution of CCR5-Delta32, CCR2-64I, and SDF1-3'A alleles in Guangxi province Zhuang population. METHOD: A total of 180 healthy individuals and 150 HIV-positive patients were recruited. All individuals were genotyped for CCR5 polymorphism using polymerase chain reaction (PCR), and CCR2 and SDF1 polymorphism using PCR restriction fragment length polymorphisms (PCR-RFLPs). RESULTS: No CCR5-Delta32 mutation was found. CCR2-64I frequency was 24.7% in the healthy group and 20.3% in the HIV-positive group, and SDF1-3'A was 30.6% and 29.3% in the 2 groups, respecti

Cerebral white matter: neuroanatomy, clinical neurology, and neurobehavioral correlates.

Lesions of the cerebral white matter (WM) result in focal neurobehavioral syndromes, neuropsychiatric phenomena, and dementia. The cerebral WM contains fiber pathways that convey axons linking cerebral cortical areas with each other and with subcortical structures, facilitating the distributed neural circuits that subserve sensorimotor function, intellect, and emotion. Recent neuroanatomical investigations reveal that these neural circuits are topographically linked by five groupings of fiber tracts emanating from every neocortical area: (1) cortico-cortical association fibers; (2) corticostri

Cerebral pathophysiology and clinical neurology of hyperthermia in humans.

Deliberate hyperthermia has been used clinically as experimental therapy for neoplastic and infectious diseases. Several case fatalities have occurred with this form of treatment, but most were attributable to systemic complications rather than central nervous system toxicity. Nonetheless, demyelating peripheral neuropathy and neurological symptoms of nausea, delirium, apathy, stupor, and coma have been reported. Temperatures exceeding 40 degrees C cause transient vasoparalysis in humans, resulting in cerebral metabolic uncoupling and loss of pressure-flow autoregulation. These findings may be

[An analysis of avoidable admissions to a neurology service].

INTRODUCTION: Inappropriate admissions to a hospital service generate unnecessary costs for our health care service. Most admissions to a hospital service come from the emergency department. The presence of a neurologist to attend hospital emergencies would be an important factor allowing admission criteria to be streamlined. AIMS: To determine the number of avoidable admissions (AA) in a neurology service, and to define their characteristics. PATIENTS AND METHODS: We conducted a prospective, descriptive study of the admissions that took place in the Neurology Service of the Hospital General…

[Analysis of the headaches treated in emergency neurology departments].

INTRODUCTION AND AIMS: Headache is the main cause of visits to the emergency neurology department at our hospital. We perform a descriptive analysis of the patients who visited the Emergency department because of this pathology and who required neurological assessment according to the International Headache Society's new classification of headaches. PATIENTS AND METHODS: A retrospective study based on 6 months' observation was conducted in order to analyse patients' diagnosis, age, sex, recurring visits to the Emergency department, complementary tests, day and time of visit to the Emergency s…

[Use of i.v. immunoglobulins in neurology. Evidence-based consensus].

Intravenous immunoglobulins (IVIg) have become an alternative immunomodulatory treatment for neurological autoimmune disorders. On the basis of several well conducted placebo-controlled trials evidence is available supporting the efficacy of IVIg. However, of all neurological diseases, IVIg is currently only licensed for Guillain-Barré syndrome. Thus, the off-label use of IVIg has to be focussed although an effective treatment must be made available for the patients, in particular when there is no other treatment alternative. The consensus paper presented here critically discusses the modern s

[The neurology training in the United States and Japan].

The practice of neurology, like many other fields of medicine, currently faces a number of critical problems in the United States mostly by cumbersome restrictions designed to contain rapidly escalating medical costs. Fortunately, however, the residency training has maintained the spirit of the time-tested tradition, which has served the medical community well ever since its inception in 1910. The system has continued with little modifications after a switch of internship to Basic Residency Program (PG1) in the late 1960s. In comparison, the neurology training in Japan suffers from following d…

A review of clinical trials of lithium in neurology.

Lithium has been put to clinical trials in no less than fifteen neurological disorders. They are Huntington's chorea, tardive dyskinesia, spasmodic torticollis, Tourette's syndrome, L-dopa induced hyperkinesia and the "on-off" phenomenon in parkinsonism, organic brain disorders secondary to brain-injury, drug induced delusional disorders, migraine and cluster headache, periodic hypersomnolence, epilepsy, meniere's disease and periodic hypokalemic paralysis. This paper gives a brief summary of the clinical trials with lithium salts reported in the literature. There are encouraging results on th

[PET in Neurology: Imaging Pathology, Metabolism, and Disease Progression].

Anti-amyloid-β therapies for Alzheimer's disease-lecanemab (Leqembi approved in September 2023) and donanemab (Kisunla®; approved in September 2024)-have been authorized in Japan and are now in clinical use. In parallel, amyloid positron emission tomography (PET) is now reimbursed by the national health insurance for determining treatment eligibility and three amyloid PET radiopharmaceuticals are currently approved. Amyloid PET images interpretation relies primarily on visual reads, which require dedicated training, and clinicians with limited PET imaging experience may find some cases challen

A new era of integration of genetics and neurology.

PURPOSE OF REVIEW: Pediatric neurology has undergone a profound transformation over the past two decades. Alongside traditional diagnostic tools such as neuroimaging, electroencephalography, and cerebrospinal fluid analysis, next-generation sequencing has emerged as a central component of clinical evaluation. Simultaneously, the field has shifted from predominantly symptomatic management toward disease-specific and precision-based therapies. This review synthesizes recent advances in neurogenomics and discusses their implications for the future of pediatric neurology and pediatric medicine mor…
