Anaesthesia for Friedreich's ataxia. Case report and review of the literature.

A patient with Friedreich's ataxia was anaesthetised on two occasions. The neuromuscular blocking agent was atracurium 0.5 mg/kg on the first occasion and tubocurarine 0.5 mg/kg on the second. The effect of each was monitored using the train-of-four twitch technique. Friedreich's ataxia has been reported to cause a marked sensitivity to non-depolarising muscle relaxants and hyperkalaemia, with resulting cardiac dysrhythmias after suxamethonium. This patient did not demonstrate an abnormal response to either relaxant; the operating conditions were satisfactory and recovery was not delayed. Thes

[Mivacurium].

Mivacurium is a short-acting nondepolarizing muscle relaxant (NDPMR) with a benzyl-isoquinoline structure and rapid, spontaneous reversal. It is hydrolyzed by cholinesterase in plasma and its chemical structure favors histamine release, leading to cutaneous or cardiovascular symptoms, particularly when the dose is increased or when the drug is injected rapidly. Both duration of effect and reversal of mivacurium are less dose dependent than they are with intermediate-duration NDPMRs. In adults the recommended dose for intubation (2 to 3 times the ED95) induces clinically effective blockade last

Feasibility of a Modified Wake-Up Test Without Intraoperative Neurophysiological Monitoring in Scoliosis Surgery: A Case Series.

The wake-up test (WKT) is an important intraoperative method for assessing spinal cord function during scoliosis surgery, particularly in settings where intraoperative neurophysiological monitoring (IONM) is unavailable. This short case series describes seven patients undergoing scoliosis surgery with a modified wake-up test (mWKT). Sugammadex was used to reverse neuromuscular blockade to facilitate motor assessment. All patients were able to follow commands and demonstrate lower-limb movement during the test. The duration of mWKT ranged from 15 to 35 min and decreased with increasing procedur

Seropositive Myasthenia Gravis Triggered by Lyme Disease: A Case Study of Molecular Mimicry.

Lyme disease, caused by Borrelia burgdorferi, is a multisystem infection that rarely produces neuromuscular complications beyond classic neuroborreliosis. Myasthenia gravis (MG) is an autoimmune disorder characterized by fatigable weakness due to acetylcholine receptor (AChR) antibodies. We describe a 74-year-old man with coronary artery disease, chronic kidney disease, and prostate cancer who developed progressive dysphagia, dysphonia, ptosis, and neck weakness. Initial attribution to medication-related angioedema delayed recognition. Neurological examination revealed bulbar weakness and fati

Motor unit number estimation in neuromuscular disease.

Motor unit number estimation (MUNE) is an electrophysiological method designed to quantify motor unit loss in target muscles of interest. Most of the techniques are noninvasive and are therefore well suited for longitudinal monitoring. In this brief review, we describe the more commonly used techniques and their applications in amyotrophic lateral sclerosis, poliomyelitis, spinal muscular atrophy and hereditary sensorimotor neuropathies. Findings in some of these studies offer important pathophysiological insights. Since conventional electrophysiologic methods are not sensible measures of moto

Monitoring the Return to Sport Transition After ACL Injury: An Alpine Ski Racing Case Study.

Alpine ski racing is an extreme sport and ski racers are at high risk for ACL injury. ACL injury impairs neuromuscular function and psychological readiness putting alpine skiers with ACL injury at high risk for ACL reinjury. Consequently, return to sport training and testing protocols are recommended to safeguard ACL injured athletes against reinjury. The aim of this paper was to present a real-world example of a return to sport training plan for a female elite alpine ski racer who sustained an ACL injury that was supported by an interdisciplinary performance team (IPT) alongside neuromuscular

Single fiber EMG and cardiovascular reflexes in botulism: a follow-up study.

A patient with botulism was studied at different times after intoxication using various autonomic tests of the cardiovascular reflexes, and by single fiber EMG (SFEMG). The control of heart rate and blood pressure appeared markedly impaired in the early stage of the disease as well as SFEMG. Autonomic function recovered more slowly as neuromuscular transmission. Monitoring autonomic derangement in botulism may give the opportunity to select patients at risk for cardiac or respiratory arrest.

Neuromuscular Alterations of Knee Extensors During Padel Match Play.

Despite padel's growing popularity, its neuromuscular fatigue profile during prolonged match play remains poorly understood. This study examined the magnitude and etiology of neuromuscular alterations in the knee extensors using voluntary and evoked contractions during extended padel play. Sixteen trained padel players (14 males, 2 females) completed a 135-min match. Neuromuscular assessments (maximal and explosive force, twitch and doublet responses) and vertical jump tests were conducted pre-match (Pre) and after 45, 90, and 135 min (Mid-1, Mid-2 and Post). Heart rate and match characteristi

Effects of sevoflurane and adenosine receptor antagonist on the sugammadex-induced recovery from rocuronium-induced neuromuscular blockade in rodent phrenic nerve-hemidiaphragm tissue specimens.

Sevoflurane affects on the A1 receptor in the central nervous system and potentiates the action of neuromuscular blocking agents. In the present study, we investigated whether sevoflurane (SEVO) has the ability to potentiate the neuromuscular blocking effect of rocuronium and if the specific antagonist of adenosine receptor (SLV320) can reverse this effect. In this study, phrenic nerve-hemidiaphragm tissue specimens were obtained from 40 Sprague-Dawley (SD) rats. The specimens were immersed in an organ bath filled with Krebs buffer and stimulated by a train-of-four (TOF) pattern using indirect

Receptor analysis of differential sensitivity change to succinylcholine induced by nerve injury in rat gastrocnemius.

BACKGROUND: Urgent tracheal intubation is common in intensive care units and the emergency room, and succinylcholine is a first-line neuromuscular blocker used in these situations. Paraplegic or critically ill patients may be at a high risk of receiving succinylcholine because the denervation stage changes nicotinic receptors, which affect the efficacy and safety of succinylcholine. The objective of this study was to determine the receptor subtypes associated with changes in the pharmacodynamics of succinylcholine and its time-line trend. METHODS: Denervated gastrocnemius was collected from t

International survey of neurosurgical anesthesia (iSonata) : An international survey of current practices in neurosurgical anesthesia.

BACKGROUND: No standardized recommendations have been currently defined for anesthesia management of patients undergoing elective intracrani … es. Nevertheless, anesthesia management (e.g. central venous catheterization, moderate hypothermia, neuromuscular monitoring) is still handled differently across many European institutions. A lack of standardized guidelines defining anesthetic management in patients undergoing intracranial procedures could explain this variability. Further studies could help establish opt …

Tetanus-induced changes in apparent recovery after bolus doses of atracurium or vecuronium.

The current study evaluated the duration and magnitude of post-tetanic effects in 56 patients recovering from a bolus dose of nondepolarizing relaxant to assess the impact of tetanus on monitoring in a common clinical setting. After induction of general anesthesia (thiopental, fentanyl, oxygen, nitrous oxide, and isoflurane), a baseline response to train-of-four (TOF) stimulation was recorded using an adductor pollicis force transducer, and the ratio of the fourth response (T4) to the first (T1) was calculated. Patients then received a bolus dose of either atracurium 0.50 mg.kg-1 (n = 28) or v
