An overview of the neurosurgical implications, pathophysiology, diagnosis and recent treatment strategies for Grade IV idiopathic osteolysis, also known as Gorham-Stout or phantom bone disease.

OBJECTIVE: Gorham-Stout disease (GSD), commonly referred as vanishing bone or phantom bone disease, is a rare disorder characterized by spontaneous bone osteolysis due to proliferation of lymphangiomatous tissue. This disease can involve multiple bones and cause pathologic fractures. The exact cause of GSD is unknown and its severity is unpredictable; the disorder can potentially cause disfigurement or functional disability. According to CARE guidelines, we studied a 46 years old lady with a progressive defect of the skull. Differential diagnosis included: benign and malignant diploic lesions

The combined transpetrosal approach: Anatomic study and literature review.

The combined petrosal approach is an essential technique for the skull base neurosurgeon. In this manuscript, the authors provide a brief history of the development of this approach, technical instruction with consideration of important landmarks, and a literature review of the broad range of clinical applications for this approach. The combined petrosal approach was performed bilaterally in 6 cadaveric injected specimens. The relationship of middle and posterior fossa dura, venous sinuses, cranial nerves (CNs), and the vascular anatomy were studied. Additionally, the authors performed a syste

A systematic review of outcomes of contralateral C-7 transfer for the treatment of traumatic brachial plexus injury: an international comparison.

OBJECTIVE The effectiveness of contralateral C-7 (CC7) transfer is controversial, yet this procedure has been performed around the world to treat brachial plexus injuries. The authors performed a systematic review to study whether Asian countries reported better outcomes after CC7 transfer compared with "other" countries. METHODS A systematic literature search using PubMed, EMBASE, and 3 Chinese databases was completed. Patient outcomes of CC7 transfer to the median and musculocutaneous (MC) nerves were collected and categorized into 2 groups: Asia and "other" countries. China was included as

Current status and future perspectives on the medical treatment of neurocysticercosis.

Neurological disease resulting from neurocysticercosis (NCC) is common in most of the world. The variability in the biology of the infection and in its clinical manifestations has led to much confusion regarding appropriate management. Therapeutic options have evolved from surgery, symptomatic measures, and steroids, to include the use of anti-parasitic drugs and minimally invasive neurosurgery. This manuscript reviews the principles of medical therapy for NCC, from discussion of the need for individualized management approaches for each type of NCC to exploration of the most likely potential

Hydrocephalus Resolution with Third Ventriculostomy with Floor Stenting: Technical Note and Literature Review.

INTRODUCTION: The management of refractory obstructive hydrocephalus is a paramount neurosurgical challenge. The endoscopic third ventriculo … 20-year-old man with obstructive hydrocephalus due to primary aqueductal stenosis performed several neurosurgical procedures, including two previous ETV, without long term resolution. We performed another ETV, with stent placement at the stoma to prevent occlusion. After 25 months of asymptomatic follow-up, the patient presented with an enlarged fourth ventricle, and a ne …

Traumatic intracranial aneurysms complicating anterior skull base surgery.

Traumatic cerebral aneurysm formation following closed head injury is uncommon, although well documented in the literature. Aneurysmal development following surgical procedures on the anterior skull base is extremely rare. This article reports successful neurosurgical management of 3 cases of anterior circulation aneurysms that developed following relatively straightforward rhinological procedures. These cases illustrate the vulnerability of the vessels of the anterior circle of Willis; they also document the sites of penetration of the anterior skull base. As reported in the literature, most

Technical nuance of ventriculoperitoneal shunt placement in a patient with cutis verticis gyrata: illustrative case.

BACKGROUND: Cutis verticis gyrata (CVG) is a rare benign scalp condition that causes the formation of skin ridges and furrows. Because of the altered scalp anatomy, this condition can pose unique challenges in neurosurgical procedures. OBSERVATIONS: The authors report the case of a 47-year-old man with CVG and neurosarcoidosis who developed hydrocephalus requiring a ventriculoperitoneal shunt. Initial shunt placement at Kocher's point proceeded without complications. However, the patient presented several months later with shunt failure caused by migration of the proximal catheter. The cause …

Case report: proximal tubule impairment following volatile anesthetic exposure.

The safety of contemporary volatile anesthetic agents with respect to kidney function is well established, and growing evidence suggests that volatile anesthetics even protect against ischemic nephropathy. However, studies examining effects of volatile anesthetics on kidney function frequently demonstrate transient proteinuria and glycosuria following exposure to these agents, although the cause of these findings has not been thoroughly examined. We describe the case of a patient who underwent a neurosurgical procedure, then experienced glycosuria without hyperglycemia that resolved within day

A Model for Intracranial Bacterial Infection After Neurosurgery Integrating Bacterial Culture and Genetic Testing.

BACKGROUND: Early diagnosis of intracranial bacterial infections following neurosurgical procedures remains challenging because of delayed or inconclusive pathogen identification. This study aimed to develop and validate a predictive model for early detection of post-neurosurgical procedure bacterial infections by integrating clinical features and cerebrospinal fluid (CSF) biomarkers. METHODS: We conducted a retrospective study including 429 patients who underwent neurosurgery at Tianjin Huanhu Hospital between November 2018 and December 2024. Patients were randomly divided into training (n =…

Visual hallucinations in neurosurgery: A systematic review and two case insights into Charles Bonnet Syndrome.

INTRODUCTION: Visual disturbances in brain tumor patients occur as a direct consequence of tumor location or as a complication after surgery. These can range from minor vision changes to severe impairments, including visual hallucinations. RESEARCH QUESTION: Charles Bonnet Syndrome (CBS) is an underrecognized cause of complex visual hallucinations in patients with visual system disorders, particularly following neurosurgical interventions. We present two cases of CBS following neurosurgical procedures, recruited from our department. MATERIALS AND METHODS: Detailed patient histories, neurolog…

The current status of endoscopy in transsphenoidal surgery: an international survey.

OBJECTIVE: A survey-based study was designed to assess opinions related to transsphenoidal endoscopy, to evaluate the current global attitudes of neurosurgeons regarding the use of this procedure, and to solicit basic data from key institutions worldwide, with the advantage that large series of cases become available for analysis. METHODS: A web-based multi-item questionnaire was distributed to the surgical departments of 393 neurosurgical centers by means of an invitation e-mail. The questionnaire was composed of 2 sections: section 1 focused on the surgical technique for pituitary adenomas

Can Non-Neurosurgeons Operate on Traumatic Brain Injuries in Non-Metropolitan Areas? A Scoping Review.

Traumatic brain injuries (TBIs) with increased intracranial pressure (ICP) require time-sensitive surgical intervention. In non-metropolitan areas, neurosurgeons are often unavailable to provide definitive treatment. Therapeutic surgical intervention by a non-neurosurgeon, for example, general surgeons, is a potential alternative; however, the feasibility and utility of non-specialist intervention are poorly defined within the literature. A scoping review was conducted within Scopus, Emcare, MEDLINE and CINAHL for original literature about emergency neurosurgical interventions performed by a n
