Expert Consensus on Obesity Management Protocols - A Joint Position Statement by Obesity and Metabolic Surgery Society of India (OSSI) and Endocrine Society of India (ESI).

BACKGROUND: Obesity in India is rising rapidly, with higher body fat at lower BMI and younger age compared to Western populations, leading to earlier onset of type 2 diabetes and cardiovascular disease in a resource-constrained health system. Protocols for obesity care therefore need to address region-specific challenges and ensure culturally acceptable, feasible treatment options. METHODS: The Obesity and Metabolic Surgery Society of India (OSSI) and the Endocrine Society of India (ESI) jointly developed India-specific obesity management protocols using a modified Delphi consensus. A protoco…

Orlistat: a comprehensive review of a lipase inhibitor from obesity management to emerging oncological applications.

Orlistat, a potent inhibitor of gastric and pancreatic lipases, has been a cornerstone of pharmacological obesity management for over 2 decades. By reducing dietary fat absorption, it facilitates modest but clinically significant weight loss and helps prevent weight regain when combined with lifestyle interventions. Its use is associated with improvements in a range of obesity-related comorbidities, including enhanced glycemic control, reduced risk of type 2 diabetes progression, and favorable modifications of cardiovascular risk factors such as dyslipidemia. However, its clinical utility is o…

Gestational weight management in women with class III obesity : associations with obstetric and perinatal outcomes.

BACKGROUND: Class III obesity is associated with increased maternal and neonatal complications. Gestational weight gain (GWG) is a modifiable factor that may influence these outcomes, but evidence in women with severe obesity remains limited. PURPOSE: To evaluate the impact of GWG adherence to the Institute of Medicine (IOM) and Journal of the American Medical Association (JAMA) recommendations on obstetric and neonatal outcomes in women with class III obesity, and to explore the role of nutritional follow-up. METHODS: Retrospective, single-center cohort study including women with class III

Nutritional management of individuals with obesity and COVID-19: ESPEN expert statements and practical guidance.

The COVID-19 pandemics has created unprecedented challenges and threats to patients and healthcare systems worldwide. Acute respiratory complications that require intensive care unit (ICU) management are a major cause of morbidity and mortality in COVID-19 patients. Among other important risk factors for severe COVID-19 outcomes, obesity has emerged along with undernutrition-malnutrition as a strong predictor of disease risk and severity. Obesity-related excessive body fat may lead to respiratory, metabolic and immune derangements potentially favoring the onset of COVID-19 complications. In ad

Psychiatric and metabolic characteristics of childhood versus adult-onset obesity in patients seeking weight management.

The purpose of the study was to examine whether childhood-onset obesity differed from adult-onset obesity in lifetime prevalence of mood and eating disorders, and metabolic abnormalities, in currently obese adults seeking weight loss. A subgroup of childhood-onset obesity participants (N=44) was compared with a subgroup with adult-onset obesity (N=69) on a number of clinical and metabolic features. The results showed high lifetime prevalence rates of mood (78%) and eating (81%) disorders, and metabolic syndrome (45%), in the group as a whole. However, patients with childhood-onset obesity had

Childhood Obesity: Global Burden, Multifactorial Etiologies, Biological Mechanisms, and Emerging Interventions for Prevention and Management.

Childhood obesity, characterized by excessive accumulation of body fat that adversely affects health, is a global public health challenge. It affects approximately 8.2% of children and adolescents globally and contributes significantly to the burden of chronic diseases in later life. Obese children are at increased risk of developing metabolic disorder cluster, type 2 diabetes (T2D), coronary artery disease (CAD), and psychosocial complications. Increasing prevalence of childhood obesity imposes substantial economic costs and highlights the urgent need for effective prevention and management s

Obesityand Gynaecological Cancers, with a Focus on Morbid Obesity: Risk Stratification, Early Diagnosis and Management.

Obesity is a chronic, relapsing disease and a significant oncological risk factor. The correlation is most pronounced and consistent for endometrial cancer. Conversely, evidence linking obesity to ovarian cancer is less robust and varies by histotype, while the association with cervical cancer is influenced by factors related to screening, diagnosis, treatment, and survival. This review examines obesity, particularly class III (morbid) obesity, in relation to the risk of gynaecological cancer, diagnostic approaches, and management strategies. A structured narrative review of PubMed/MEDLINE, Co

Obesity in Living Kidney Donors from Risk to Ethics: Bridging Gaps in Evaluation and Management.

Obesity is increasingly prevalent among living kidney donor candidates, presenting complex clinical, ethical, and logistical challenges. Although obesity is linked to increased post-donation hypertension, diabetes, proteinuria, and chronic kidney disease, the overall absolute risks are modest and may differ depending on metabolic health and patterns of fat distribution. Current reliance on body mass index alone fails to capture the full spectrum of obesity-related risks, leading to inconsistent donor selection practices and potential inequities. This review examines the pathophysiological mech

BODY-Q-7D Measures Consensus-Based Patient-Reported Outcomes for the Management of Obesity Treatment.

OBJECTIVE: A global multidisciplinary consensus initiative identified eight patient-reported outcome domains important in obesity care: self-esteem; physical, mental, and social health; eating; stigma; body image; and excess skin. Several of these domains are not represented in generic preference-based measures typically used in weight loss research. We developed the BODY-Q-7 Dimensions (BODY-Q-7D) to provide a valuation-ready framework for a preference-based instrument. METHODS: Concepts from the BODY-Q were used to inform the development of items that covered the eight consensus initiative

Lipedema and obesity: biological interactions, diagnostic challenges, and principles of interdisciplinary management - a narrative review.

Lipedema and obesity are chronic, frequently progressive disorders that can present with increased adipose-tissue volume, functional impairment and reduced quality of life. Although often approached as differential diagnoses, they frequently coexist and may aggravate one another. This narrative review addresses how lipedema and obesity interact biologically and clinically and what evidence supports a staged interdisciplinary approach to diagnosis and management. Obesity is increasingly conceptualized as an adiposity-based chronic disease characterized by excess adipose, abnormal adipose-tissue
