The history of obstetric forceps - evolution of the instrument that transformed obstetrics.

Obstetric forceps have played a pivotal role in perinatal medicine for centuries, representing one of the most significant innovations in the history of obstetrics. As a life-saving instrument, forceps revolutionized the management of complicated labours by offering an alternative to destructive procedures and maternal mortality, thereby transforming the outlook for both mothers and newborns. Throughout their history, however, forceps have provoked both admiration and controversy - praised for their ability to facilitate safe delivery, yet criticized for their potential risks and the technical…

Anaesthesia for the obstetric patient with (non-obstetric) systemic disease.

The number of women with serious (non-obstetric) systemic diseases achieving pregnancy and requiring obstetric anaesthetic management is increasing. The conditions that are most likely to cause maternal morbidity and mortality are cardiac disease, respiratory disease, neuromuscular disease, haematological disease, connective and metabolic diseases and psychiatric conditions including substance abuse. This article discusses the anaesthetic management of the pregnant mother with such serious systemic diseases.

Obstetric characteristics profiles as quality assessment of obstetric care.

In The Netherlands only about 50% of all pregnancies are defined as high risk pregnancies and consequently come into the domain of care of an obstetrician. In order to qualify as a high risk pregnancy, the pregnancy must satisfy certain criteria contained in an officially approved list of indications. Due to varying perceptions of these selection criteria by the selectors, larger differences in the treated population, obstetric interventions and results, respectively, were expected than in other countries. Within the framework of the project 'Obstetric Peer Review' (Verloskundige Onderlinge Kw

Obstetric Pharmacomicrobiomics.

Pregnancy is characterized by physiologic, metabolic, immunologic, and microbial adaptation. Medication use during pregnancy is common. Yet, therapeutic response often remains unpredictable because obstetric pharmacology has historically focused on pharmacokinetic changes while paying little attention to host-microbiome-drug interactions. Pharmacomicrobiomics, the study of bidirectional interactions between the microbiome and therapeutic agents, has emerged as an important frontier in precision medicine. Increasing evidence demonstrates that the maternal microbiome may influence drug bioavaila

Obstetric pharmacogenomics.

Pregnancy profoundly alters drug disposition through dynamic physiologic adaptations affecting absorption, distribution, metabolism, and exc … netic modeling and multi-omics technologies offers an opportunity to redefine precision-medicine in obstetrics and advance safer, more effective, and individualized pharmacotherapy for pregnant-women and their offspring.

Obstetric emergencies.

Uterine rupture, uterine inversion, and pelvic lacerations/hematomas are all associated with increased risk for hemorrhage at the time of delivery. Certain clinical factors will increase the likelihood that these complications will occur, and clinical management can be adapted to reduce risk. Prior cesarean section is a significant risk factor for uterine rupture, with the degree of risk directly related to the location and quantity of prior uterine incisions. Obstetric trauma is another independent risk factor for uterine rupture. In turn, uterine inversion has traditionally been associated w

Obstetric MRI.

Ultrasound is the imaging modality of choice for pregnant patients. However, MRI is increasingly utilized in patients in whom the sonographic diagnosis is unclear. These include maternal conditions unique to pregnancy such as ectopic pregnancy, placenta accreta, and uterine dehiscence. MRI is also being increasingly utilized in the assessment of abdominopelvic pain in pregnancy, in particular in assessment for appendicitis. Fetal MRI is performed to assess central nervous system (CNS) abnormalities and patients who are considering fetal surgery for conditions such as neural tube defects, conge
