Pycnodysostosis is a very rare autosomal recessive disease. This disorder presents with osteosclerosis, leading to fragile bones, short stature, craniofacial abnormalities, laryngomalacia, sleep apnea syndrome, and many other findings. Difficulty intubation is very common. In our case, we have a patient scheduled for a plastic surgery on her skull under general anesthesia (GA), but she could not lie in a flat position as this could lead to cyanosis and cardiac arrest. So, we have to intubate her in 45° back-up position. Intubation in positions different from the usual supine one probably becau…

Osteopetrosis is a rare form of sclerosing bone dysplasias which in some cases requires orthopedic surgical management of its complications. It is characterized by osteosclerosis, brittle bones and obliteration of the medullary cavity. Therefore, surgery in these cases is difficult and results can be compromised by postoperative complications. We present a case of a 73-year-old patient treated at our institution in a period of 24 years. In this period, the patient had bilateral primary hip arthroplasty (THA), total knee arthroplasty, revision surgeries of both THA's and lumbar spine surgeries.…

Results of investigation of mineral density condition of skeletal osseous tissue in patients with inflammatory and dystrophic-inflammatory diseases of periodontal tissues with ultrasound densitometry method have been presented. Various changes of osseous tissue of skeletal bones have been detected: osteopenia, osteoporosis and osteosclerosis, which correlated with the severity of pathological process in periodontium. Analysis of the obtained results has been carried out depending on patients' sex as well as form and severity degree of the course of periodontal diseases. It has been established…

Hyperostosis cranialis interna is a hereditary bone disorder that is characterized by endosteal hyperostosis and osteosclerosis of the calvaria and the skull base (OMIM 144755). The progressive bone overgrowth causes entrapment and dysfunction of cranial nerves I, II, V, VII, and VIII, its first symptoms often presenting during the second decade. This study analyzes the clinical course of 13 affected individuals of three related families (32 individuals). The disorder appears to have an autosomal-dominant transmission pattern. Facial and vestibulocochlear nerve dysfunction are most frequently …

Primary hyperoxaluria is a rare autosomal recessive disorder resulting in diffuse deposition of insoluble oxalate crystals in multiple organs. Two-thirds of patients have nephrolithiasis by 5 years of age and 80% die of renal failure by 20 years of age. Rarely, the disease will present in adulthood, with the onset of symptoms occurring as late as the sixth decade. Oxalosis is a condition in which the highly insoluble calcium oxalate crystals are deposited in extrarenal tissue. Pathologic involvement of bone is recognized as osteosclerosis on radiograph. Oxalosis should be considered in patient…

Pycnodysostosis, an autosomal recessive osteochondrodysplasia characterized by osteosclerosis and short stature, maps to chromosome 1q21. Cathepsin K, a cysteine protease gene that is highly expressed in osteoclasts, localized to the pycnodysostosis region. Nonsense, missense, and stop codon mutations in the gene encoding cathepsin K were identified in patients. Transient expression of complementary DNA containing the stop codon mutation resulted in messenger RNA but no immunologically detectable protein. Thus, pycnodysostosis results from gene defects in a lysosomal protease with highest expr…

A new sclerosing bone disease in two Japanese siblings born to first-degree cousin parents is reported. Clinically the disease is characterized by early developmental delay, hypotonia and later spastic paraplegia. The unique radiographic changes consist of peripheral osteosclerosis affecting predominantly metaphyses of the long bones and to a lesser degree ends of the ribs and clavicles, iliac crests, acetabulae, ischio-pubic synchondroses and vertebrae. The epiphyses are sclerotic in early life. The round bones, short tubular bones and the skull are little affected. The shafts of the tubular …

A patient received 2.5 mg vitamin D2 daily for 10 years and presented with increasing skeletal pain and hypercalcaemia. The limbs were painful to touch especially at the insertions of ligaments and tendons, and radiographs showed osteosclerosis with calcification in the periosteum, blood vessels, tendoachilles and plantar fascia. Bone histomorphometry showed increased amounts of osteoid and defective mineralisation despite hypercalcaemia, hyperphosphataemia and raised serum concentrations of vitamin D metabolites. A negative external calcium balance was documented in the presence of enhanced i…

CASE: A 13-year-old boy presented with a painful limp for 2 weeks without trauma and guarded (L) hip movements with spasm. Radiographs revealed a valgus slipped capital femoral epiphysis (SCFE) of the left hip with generalized osteosclerosis. Elevated serum calcium, phosphate, and parathyroid hormone levels were noted. Whole-exome sequencing identified a homozygous FGF23 variant, confirming hyperphosphatemic tumoral calcinosis. He was managed with phosphate binders, dietary restriction, and in situ fixation of both hips. At the 2-year follow-up, alignment and function were normal.
CONCLUSION:…

Erdheim-Chester disease (ECD) is a rare form of systemic histiocytosis, typically presenting with striking osseous involvement characterized by bilateral osteosclerosis and involvement of organs such as the lung, pituitary gland, heart, and brain. Liver involvement with ECD is extremely uncommon. We report a 56-year-old woman presenting with newly diagnosed cirrhosis and signs concerning for intra-abdominal malignancy, including omental caking and peritoneal thickening. Liver biopsy demonstrated xanthogranulomatous infiltration from ECD. The patient showed initial improvement with interferon t…

INTRODUCTION: Erdheim-Chester disease and langerhans cell histiocytosis are two rare diseases separate on clinical, radiological and histological aspects. However, several cases involving both entities have been described.
OBSERVATION: A 70-year-old man had a central diabetes insipidus, xanthelasmas, retroperitoneal fibrosis and osteosclerosis of the legs suggestive of Erdheim-Chester disease. Bone biopsy showed langerhans cell histiocytosis CD1a positive with the presence of BRAF V600E mutation. The patient was treated with vemurafenib with a good clinical course.
CONCLUSION: The literature…

Osteopetrosis is a genetic disorder characterized by impaired osteoclast function, leading to diffuse osteosclerosis, brittle bones, and hematologic abnormalities. Herein, we report the case of a 55-year-old woman diagnosed with adult-onset osteopetrosis at age 33 after imaging demonstrated dense and marbled long bones, sclerotic medullary canals, vertebral endplate thickening, and hepatosplenomegaly. Bone marrow biopsy revealed preserved megakaryocytes, yet she experienced recurrent episodes of thrombocytopenia that responded to oral steroids and ultimately resolved following splenectomy, sup…

BACKGROUND: High fluoride ion (F(-)) levels are found in many surface and well waters. Drinking F(-)-contaminated water typically explains endemic skeletal fluorosis (SF). In some regions of Asia, however, poor quality "brick tea" also causes this disorder. The plant source of brick, black, green, orange pekoe, and oolong tea, Camellia sinensis, can contain substantial amounts of F(-). Exposure to 20 mg F(-) per day for 20 yr of adult life is expected to cause symptomatic SF. High F(-) levels stimulate osteoblasts and enhance bone apposition but substitute for OH(-) groups in hydroxyapatite cr
