OBJECTIVES: This study aims to evaluate prospectively the safety and efficacy of extracorporeal shock wave lithotripsy (ESWL) in Chinese patients.
METHODS: A total of 214 patients with painful chronic pancreatitis and pancreatic stones who underwent ESWL followed by endoscopic retrograde cholangiopancreatography from March 2011 to February 2012 in Changhai Hospital were enrolled. The main pancreatic duct clearance rate and complications were recorded prospectively. Symptoms, weight, quality of life, and pancreatic function were assessed before and after ESWL and endotherapy.

PURPOSE OF REVIEW: Pancreatic exocrine insufficiency (PEI) has long been associated with intrinsic pancreatic disease, yet emerging evidence shows it is under-recognized in patients with nonspecific gastrointestinal symptoms. This review summarizes prevalence data, evaluates the performance and utility of faecal elastase-1 (FE-1), highlights the co-existence of PEI with other luminal conditions, and explores the presence of early pancreatic dysfunction among a subset of patients with irritable bowel syndrome (IBS)-type symptoms. Updates from the 2025 European guidelines are discussed.

A 46-year old man developed subcutaneous fat necrosis of the right upper thigh in association with pancreatitis. The panniculitis established by biopsy resolved 4 years later. Subcutaneous nodular fat necrosis is a rare dermatological manifestation of pancreatitis or pancreas carcinoma which may be clinically indistinguishable from erythema nodosum. Biopsy of the skin lesion reveals a characteristic histological picture with necrotic fat cells surrounded by lymphocytes, histiocytes and plasmacells.
