BACKGROUND: Approximately 4 to 34 percent of infants born with neonatal brachial plexus palsy do not recover spontaneously and require surgery. Despite the increasing availability of microsurgical nerve repair, the authors hypothesize that this condition remains undertreated and that uninsured children and children with public insurance are less likely to receive treatment than those with private insurance.
METHODS: The authors used a national sample of inpatient hospital discharge data from the Healthcare Cost and Utilization Kids Inpatient Databases for the years 1997, 2000, 2003, and 2006.

Birth injuries to the brachial plexus have not disappeared in spite of improvements in obstetrics. Many of these palsies spontaneously recover but some will require a surgical approach. Over the last 30 years, the author has explored and repaired 1026 brachial plexus injuries in infants who had not recovered biceps function after three months. Repair is always feasible and its results far better than spontaneous progression. The results are evaluated after 20 years of follow-up for some patients. Their evaluation confirms the importance of early plexus surgery.

Eleven brachial plexus lesions were repaired using end-to-side side-to-side grafting neurorrhaphy in root ruptures, in phrenic and spinal accessory nerve neurotizations, in contralateral C7 neurotization, and in neurotization using intact interplexus roots or cords. The main aim was to approximate donor and recipient nerves and promote regeneration through them. Another indication was to augment the recipient nerve, when it had been neurotized or grafted to donors of dubious integrity, when it was not completely denervated, when it had been neurotized to a nerve with a suboptimal number of fib

Seventy-three children with slow recovery after obstetric lesion of the brachial plexus (biceps function returning after 3 months of age) and with relatively favourable neurophysiological investigation were followed until a mean age of 4.3 years. Predictions for C6 and C7 were confirmed in 92% and 96% of the cases, respectively. Predictions for C5 were confirmed in a smaller proportion of cases (78%). The inability to record nerve action potentials for C5, and the high frequency of secondary shoulder pathology are reasons for this. In all, 33 of the 73 children came to operation for medial rot

We present the main findings of the 5th National Audit Project (NAP5) on accidental awareness during general anaesthesia (AAGA). Incidences were estimated using reports of accidental awareness as the numerator, and a parallel national anaesthetic activity survey to provide denominator data. The incidence of certain/probable and possible accidental awareness cases was ~1:19,600 anaesthetics (95% confidence interval 1:16,700-23,450). However, there was considerable variation across subtypes of techniques or subspecialities. The incidence with neuromuscular block (NMB) was ~1:8200 (1:7030-9700),

OBJECTIVE: Shoulder dystocia (SD) and brachial plexus palsy (BPP) are complications of childbirth that can result in significant long-term sequelae. The purpose of the present study was to analyze risk factors in cases of SD and BPP.
METHODS: We performed a retrospective study of laboring women who delivered a singleton, term, live-born infant at the Los Angeles County + University of Southern California Medical Center from 1995 to 2004. Multivariable logistic regression models were used to analyze risk factors among SD cases with and without BPP.
RESULTS: Of the 13,998 deliveries that met i

Among the late consequences of obstetrical brachial plexus palsy is winging of the scapula, a functional and aesthetic deformity. This article introduces a novel surgical procedure for the dynamic correction of this clinical entity that involves the dynamic transfer of the contralateral trapezius muscle and/or rhomboid muscles and anchoring to the affected scapula. In more severe cases of scapula winging, the contralateral latissimus dorsi muscle may also need to be transferred to achieve dynamic scapula stabilization. The outcomes of this novel surgical procedure were analyzed in relation to

Gorham-Stout disease is a rare disorder of bone loss and proliferation of lymphatic and vascular tissue (lymphangiomatosis). A 30-year-old nulliparous woman with Gorham-Stout disease presented at 8weeks of gestation with a fused cervical spine. At 31weeks she developed basilar invagination and neurological symptoms that were managed with a neck brace. Anesthetic considerations were those of airway compromise, development of severe preeclampsia and Kasabach-Merritt coagulopathy. Elective tracheostomy was declined. She presented two days before a planned cesarean delivery at 35weeks in preterm l

Elective microsurgery of the peripheral nervous system concerns nerve decompression, neurolysis, nerve repair, and forms of nerve reconstruction with grafts or tubes. Management skills for the peripheral nerve too often rely on antiquated assessment techniques and depend upon traditional electrodiagnostic testing. Quantitative sensory testing is an approach to the evaluation of peripheral nerve function that is available now to all physicians interested in managing peripheral nerve problems. This article introduces quantitative sensory testing, with particular regard to the management of perip

PURPOSE: To determine the value of magnetic resonance (MR) imaging in diagnosing traumatic pseudomeningocele.
MATERIAL AND METHODS: Five newborn infants with brachial plexopathy secondary to traumatic delivery underwent MR imaging of the spine and serial neurologic examinations, and the findings were compared.
RESULTS: Cervical MR images showed focal collections of cerebrospinal fluid lateral to the spinal cord and extending into the neural foramina. These were believed to represent pseudomeningoceles in four infants, all of whom have persistent major neurologic deficits. The infant with nor

A 38-year-old wheelchair-bound primigravida with transverse myelitis presented at 38 weeks of gestation for elective caesarean section. Transverse myelitis, which is characterised by bilateral inflammation of the spinal cord and myelin destruction, is associated with myopathy, autonomic dysreflexia and pulmonary aspiration. Regional anaesthesia was contraindicated in this case as the patient had undergone two previous lumbar spinal fusion procedures. Rocuronium 1.2 mg/kg was used to facilitate rapid intubating conditions. The caesarean section proceeded uneventfully, but even after administrat

We present 7 children with obstetric brachial plexus palsy treated by transferring two motor fascicles out of the ulnar nerve to the biceps nerve. Three were male, and 4 were female. The left-side brachial plexus was affected in 4 patients, and the right side in 3 patients. All children had vaginal delivery; two of them presented with shoulder dystocia. The average birth weight was 4300 g (range, 3620-5500 g). Average age at time of operation was 16 months (range, 11-24 months). The indication for the operation was absent active elbow flexion with active shoulder abduction against gravity in 4

The denervated limb of the child with obstetrical brachial plexus injury is theoretically at risk of contact burns. A prospective study was designed to document the incidence and clinical presentation of these burn injuries in a series of 127 birth palsy cases. The study group included 90 Erb's and 37 total plexus palsy cases. Accidental burns occurred in 11% of total palsy cases and 1% of the Erb's palsy group. All injuries were contact burns and confined to the denervated limb. When the circumstances of the burn injury where investigated, it was concluded that injury to the denervated limb w

The fetus is subjected to mechanical forces during labor and delivery, which may result in traumatic injuries. Such injuries include intracranial hemorrhage, spinal cord lesions, cephalhematoma, cranial or peripheral nerve palsies, intraabdominal organ rupture, or bony fractures. Risk for perinatal trauma and mortality is increased in primigravidas, multiple gestations, abnormal presentations, maternal-fetal disproportion, oligohydramnios, forceps or vacuum extractions, and internal version maneuvers. Very-low-birth-weight neonates (<1500 g) are at high risk due to ease of deformity of the cra

The aim of this study was to review and to analyze treatment patterns of early and late obstetric brachial plexus palsy. Eighty-one children with early and late obstetric brachial plexus palsy were treated in the Department of Pediatric Orthopedics and in the Postintensive Care Unit within the period 1988-2002. Children were classified into 2 groups according to age: Ist group (67 newborns) was treated conservatively, and IInd group (14 children with late obstetric brachial plexus palsy with deformity) underwent operative treatment. Active hand movements and innervation were evaluated before a
