Inflammatory diseases in hematology: a review.

Hematopoietic cells are instrumental in generating and propagating protective inflammatory responses to infection or injury. However, excessive inflammation contributes to many diseases of the blood, bone marrow, and lymphatic system. We review three clinical categories of hematological inflammatory diseases in which recent clinical and translational advances have been made. The first category is monogenic inflammatory diseases. Genotype-driven research has revealed that previously mysterious diseases with protean manifestations are characterized by mutations that may be germline (e.g., defici

[Heavy chain deposition disease: report of a case and review of the literature].

A 35-year old woman was admitted to our hospital because of systemic edema. Laboratory examination revealed that she had Bence Jones protein(lambda), hypocomplementemia, and proteinuria(0.5 g/day). Renal biopsy showed nodular glomerular lesions with IgG heavy chain and C3 deposition, but not other immunoglobulin and light chains. An extensive study about the character of circulating and deposited IgG demonstrated that the complexes of CH2-CH3 lacking CH1 domain was an important factor in the glomerular heavy chain deposition.

The potential role of mass spectrometry for the identification and monitoring of patients with plasma cell disorders: Where are we now and which questions remain unanswered?

Mass spectrometry (MS) techniques provide a highly sensitive methodology for the assessment and monitoring of paraproteins compared to standard electrophoretic techniques. The International Myeloma Working Group (IMWG) recently approved the use of intact light chain matrix-assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF MS) in lieu of immunofixation in the clinical assessment of patients and the assessment of patients enrolled on clinical trials. The increased sensitivity of these assays may help to detect and monitor monoclonal proteins (MP) in many patients w…

[Therapeutic apheresis: selective methods].

Therapeutic apheresis is a term that describes numerous different procedures that remove pathological substances from the body. They are used in cases when conservative treatment measures fail, and may be useful as an adjuvant method for treatment of different diseases which have a common feature pathogenic protein substance (paraproteins, antibodies...) or pathogenic substance attached to proteins (toxins), that has to be removed from the body. These substances could be removed by nonselective (plasmapheresis and therapeutic plasma exchange), semiselective (cascade apheresis) or selective met…

Pseudohypobicarbonataemia: A Rare Case of Enzymatic Assay Interference From Monoclonal Paraproteinaemia.

Pseudohypobicarbonataemia is a rare but important diagnostic pitfall that can lead to unnecessary investigations and inappropriate treatments if unrecognised. We present a case of a 92-year-old woman with chronic severe hypobicarbonataemia on enzymatic assay, despite normal acid-base parameters on blood gas analysis. Extensive evaluation excluded common causes of metabolic acidosis. Analytical studies identified interference by an IgG lambda paraprotein. Serial dilution showed a non-linear increase in bicarbonate concentration and polyethylene glycol precipitation improved bicarbonate recovery

Severe but reversible pulmonary hypertension in scleromyxedema and multiple myeloma: a case report.

BACKGROUND: Scleromyxedema is a progressive, systemic connective tissue disorder characterized by fibro-mucous skin lesions and increased serum monoclonal immunoglobulin levels. Pulmonary involvement occurs in a subset of patients, though the overall prevalence of pulmonary lesions in scleromyxedema is unknown. Since pulmonary hypertension presumably occurs in these patients due to disease progression and development of additional conditions, treatment of the underlying plasma cell dyscrasia and connective tissue disorder may improve pulmonary hypertension symptoms. CASE PRESENTATION: An elde

New insights on scleromyxedema.

Scleromyxedema is a rare fibromucinous disorders, with several clinical and pathological overlaps with scleroderma and scleredema. Etiopathogenesis remains uncovered, and no explanation has been provided either for the origin of mucin deposition or for the paraprotein role. The disease does not show gender predilection and affects mainly middle-age adults. The course is unpredictable, and prognosis remains guarded for renal, cardiac, and neurologic complications, especially in the setting of dermato-neuro syndrome. A valuable recent progress is the consensus definition of diagnostic criteria a

Acute and chronic polyradiculopathies.

Polyradiculopathies are uncommon peripheral nervous system syndromes that result from a variety of conditions. The clinical manifestations are variable but often include symmetric or asymmetric distal and proximal weakness with a variable degree of sensory loss and reduction or loss of reflexes. The most common cause of an acute polyradiculopathy is acute inflammatory demyelinating polyradiculopathy (also known as Guillain-Barré syndrome); however, other inflammatory, infectious, or neoplastic causes can present with similar features. Chronic polyradiculopathies include chronic inflammatory de

Benign monoclonal gammopathy-associated acute kidney injury: case report and literature review.

BACKGROUND: Benign monoclonal gammopathy (MG) is an MG that is not accompanied by clinical manifestations of plasma cell dyscrasia, such as multiple myeloma, macroglobulinemia, amyloidosis, or other related disorders. Benign MG can progress to malignancy, but seldom causes organ damage. CASE REPORT: We presented a case denied any underlying disease who visited our hospital as acute uremia syndrome. Laboratory data indicated renal failure and abnormal paraprotein in the serum and urine. Renal biopsy indicated acute tubular necrosis and cast in the renal tubule. The plasma cell counts were norm

Bone marrow extracellular large geometric crystals in IgG/lambda MGUS.

We describe the peculiar histopathology of the bone marrow in a case of IgG/lambda MGUS. Striking eosinophilic crystals with a rectangular, rhomboid or square shape lay in the interstitium, sometimes in optically empty spaces, but failed to elicit a foreign body giant cell reaction. Their histochemical properties, immunoreactivity for anti-lambda light chain antiserum, and ultrastructural features strongly supported their relationship with the paraprotein synthesized by the monoclonal plasma cells. The crystals were not observed on bone marrow aspirate smear, suggesting that they had formed du

[Renal plasmacytoma. Report of a new case].

OBJECTIVE: To present a case of extramedullary plasmacytoma localized in the kidney. The clinical features, diagnostic tests, treatment and outcome are discussed. METHODS/RESULTS: A 59-year-old patient presented with a right renal mass and renal failure. The complementary tests showed a lambda monoclonal band in blood and urine, and a left renal biopsy showed changes compatible with myeloma. The definitive diagnosis of plasmacytoma was based on the findings of open renal biopsy since fine needle punction findings were compatible with a carcinoma. The patient received polychemotherapy, but die

[Tubulointerstitial nephropathies].

The term tubulointerstitial nephropathy (TIN) means a renal disease, during which the tubules and interstice form a substrate of the primary injury or a substrate whose involvement is dominating in the disease clinical picture. The incidence of TIN is fairly high, with different etiology and pathogenesis. Drug, virus, bacterial and immune patterns are among the most common types of acute TIN. It is shown that the incidence of chronic drug TIN ranges from 0.2% among urban population to 0.6% among rural population. The incidence and gravity of renal injuries in hyperuricemia depend on the blood

Monoclonal gammopathy of renal significance: An atypical presentation of Waldenström's disease.

Waldenström's disease is a rare lymphoproliferative syndrome in the bone marrow and sometimes in lymphoid organs which secretes high amounts of monoclonal immunoglobulin M into serum. It can remain indolent for years and rarely affects the kidney, with intraglomerular rather than intratubular damage being predominant, in contrast to multiple myeloma. Different studies identified AL amyloidosis as the most frequent renal lesion, followed by cryoglobulinemic glomerulonephritis. Signs and symptoms may be unspecific, as well as renal manifestations, so collaboration between nephrologists, hematolo

Monoclonal Light Chains with alpha 2 mobility on Serum Protein Electrophoresis.

Multiple myeloma (MM) is a neoplasm characterized by malignant proliferation of plasma cells that produce excessive quantities of a single type of immunoglobulin (Ig) called as monoclonal immunoglobulin or M-protein or paraprotein. M-protein produced can be either an intact antibody with both heavy and light-chain components or only light chains or rarely only heavy chains. Presence of M-protein in serum protein electrophoresis (PEP) is useful in the diagnosis, prognosis, and treatment of MM and other plasma cell dyscrasias. These M-proteins are identified commonly in beta and gamma regions an

First case report of pre-mortem biopsy-proven kidney involvement by necrobiotic xanthogranuloma with monoclonal gammopathy.

Necrobiotic xanthogranuloma (NXG) is a rare non-Langerhans cell histiocytosis with characteristic cutaneous features and rare visceral involvement. More than 80% of individuals with this disease have a detectable paraprotein but the precise pathogenesis remains obscure. A 68-year-old man with known cutaneous necrobiotic xanthogranuloma presented with acute kidney injury and imaging suggestive of bilateral perinephric infiltration. Renal biopsy showed a prominent histiocytic infiltration of renal capsule and cortex with necrobiosis and characteristic 'Touton-type' giant cells suggestive of necr

Quantification of bone-marrow plasma cell levels using various International Myeloma Working Group response criteria in patients with multiple myeloma.

We examined the association of residual bone-marrow plasma cells (PCs) and International Myeloma Working Group (IMWG) response assessment in light-chain-only multiple myeloma (LCMM) and intact immunoglobulin multiple myeloma (IIMM) using multicolour flow cytometry (MFC). We identify considerable differences in bone-marrow neoplastic PC levels between IIMM and LCMM for the same IMWG response categories. Furthermore, even in the same IMWG response category, residual neoplastic PC levels differed considerably over a logarithmic scale range However, normalization of the free light-chain ratio is a

Effect of Serum Immunoglobulins on Routine Coagulation Tests: A Comparison of Coagulation Analyzers using Mechanical and Optical Clot Detection.

BACKGROUND: We previously showed that the prothrombin time (PT), but not activated partial thromboplastin time (APTT), is correlated with serum immunoglobulin level in patients with plasma cell neoplasms. METHODS: To determine if the observed effect of serum immunoglobulins on PT was reagent and/or method dependent, PT and APTT were measured in plasma samples obtained from patients referred to the Myeloma Institute using mechanical (STAR Evolution; Diagnostica Stago) and optical clot detection (ACL TOP 500 analyzer; Instrumentation Laboratory) and manufacturer provided reagents.
