The diagnostic problems which arise when fever occurs in a patient returning from black Africa are more and more frequently encountered because of the multiplication of rapid connections with this continent. Analysis of the main etiologies leads the author to review most of the specifically tropical diseases. However, cosmopolitan diseases should not be underrated. The author emphasizes the high incidence of pernicious malaria, liver amebiasis, and typho-paratyphoid fevers, along with the necessity of keeping in mind the new viral diseases (Lassa, Marburg, Ebola).

AIM: To quantify the proportion of selected notified diseases in NSW attributable to overseas travel and assess the quality of data on travel-associated risk factors, to inform prevention strategies.
METHODS: 2010 and 2011 notification data for dengue, hepatitis A, hepatitis E, malaria, paratyphoid fever, shigellosis and typhoid fever were extracted from the NSW Notifiable Conditions Information Management System and analysed for travel-associated risk factors.
RESULTS: Where place of acquisition was known, the proportion of cases for whom the disease was acquired overseas ranged from 48.7% …

Enteric fever caused by Salmonella enterica serovars Typhi and Paratyphi A and, to a lesser extent, S. Paratyphi B and C, remains a significant cause of mortality and morbidity in resource-constrained settings. Typhoid conjugate vaccines (TCVs) protect against S. Typhi but no vaccine to date protects against paratyphoid fever. There are several bivalent S. Typhi/Paratyphi A products in development; however, the low incidence of paratyphoid fever in many settings limits the feasibility of phase 3 efficacy studies. Two bivalent vaccines adding the S. Paratyphi A-specific O:2 lipopolysaccharide c…

We report on two children with paratyphoid fever and rare cardiac complications (endocarditis and pericarditis) during an outbreak of Salmonella paratyphi A infection in Bangkok, Thailand, in 1996. Both of the patients had underlying congenital heart disease. Two cases in the literatures of endocarditis and five cases of pericarditis caused by Salmonella paratyphi were reviewed. These rare cardiac complications should be considered among persons who reside in an endemic area of enteric fever or during disease outbreaks, especially in children with underlying heart diseases.

We evaluated the reliability of a passive bacterial agglutination test to detect Salmonella typhi somatic antigen(s) in the sera of patients with typhoid fever. It was positive in 32 of 33 bacteriologically proven typhoid fever cases. Among 13 patients with a presumptive diagnosis of typhoid fever, 11 were positive by passive bacterial agglutination. The serum of one patient with paratyphoid A was also positive. Among 50 febrile patients without typhoid fever, one was persistently positive during the course of illness; 49 were negative. The sensitivity, specificity, and accuracy indices of the

Salmonella enterica serovar Paratyphi A is the main causative agent of paratyphoid fever in many Asian countries. As paratyphoid is spread by the fecal-oral route, the most effective means of controlling S. Paratyphi A infection is through the availability of clean water supplies and working sanitation services. Because sanitation facilities improve slowly in these poor areas and antibiotic resistance is severe, the development of a safe and effective vaccine remains a priority for controlling the spread of paratyphoid disease. In this study, we investigated the strategy of heterologous O-anti…
