Use of noninvasive versus invasive ventilation in patients hospitalized with acute exacerbation of heart failure: a Nationwide Readmissions Database study.

BACKGROUND: Respiratory compromise requiring ventilatory support is common in acute decompensated heart failure (ADHF). Noninvasive ventilation (NIV) may improve outcomes and reduce mortality compared with invasive mechanical ventilation (IMV). This study evaluated the association between ventilation modality and 30-day all-cause readmission. METHODS: Using data from the 2016-2017 Nationwide Readmissions Database, adults aged 18 years or older who were hospitalized with a principal diagnosis of ADHF and received ventilatory support were identified. The primary outcome was 30-day all-cause rea

Temporal Validation of a Machine Learning Readmission Model in Patients With Heart Failure With Preserved Ejection Fraction and Chronic Kidney Disease.

BACKGROUND: The coexistence of heart failure with preserved ejection fraction and chronic kidney disease is associated with high readmission risks. Existing tools, such as the Meta-Analysis Global Group in Chronic Heart Failure (MAGGIC) score, rely on linear modelling, which limits their ability to capture nonlinear interactions. We developed a machine learning model to predict readmission and assessed its performance against temporal data drift. METHODS: In this retrospective study, patients were temporally stratified into derivation (n = 750) and independent validation (n = 130) cohorts. Fe

Development and external validation of a prediction model for 90-day readmission in elderly patients with COPD complicated by pulmonary heart disease.

BACKGROUND: Older individuals developing chronic obstructive pulmonary disease (COPD) and concurrent pulmonary heart disease (PHD) have a higher early readmission probability post-discharge. But validated and interpretable models for forecasting 90-day COPD/PHD-related readmission among such individuals remain scarce. METHODS: This retrospective two-center study recruited ≥ 65-year-old patients developing COPD and concurrent PHD from Affiliated Hospital of North Sichuan Medical College (development cohort) as well as Hospital of Integrated Traditional Chinese and Western Medicine of Dazhou Ci

Parkinson's Disease Patients Face Higher 90-Day Readmission, Reoperation, and Infection Risk Following Total Knee Arthroplasty.

BACKGROUND: Parkinson's disease (PD) is an increasingly common comorbidity in patients undergoing total knee arthroplasty (TKA), yet its impact on readmissions remains poorly defined. METHODS: Using the Nationwide Readmissions Database 2020-2022, we identified elective, primary TKA cases. Patients with oncologic diagnoses, COVID-19-related admissions, acute fractures, and contralateral knee arthroplasty were excluded. Only the first readmission within 90 days was analyzed. Propensity score matching (1:1) was performed for demographics, comorbidities, hospital factors, and primary payer. The p

Severity of Peripheral Artery Disease Predicting Clinical Outcomes and 30-day Nonelective Readmission in Patients Admitted for Atrial Fibrillation.

BACKGROUND: While peripheral artery disease (PAD) is a recognized risk factor for other macrovascular diseases, including coronary artery disease, the impact of PAD severity in patients hospitalized for atrial fibrillation (AF) remains underexplored. Using a nationally representative cohort of patients admitted for AF, we examined clinical outcomes in those with different PAD severities and compared them to patients without PAD. METHODS: Data were obtained from the 2016 to 2022 Nationwide Readmissions Database to query all patients admitted for AF. Patients were categorized into 3 groups: No

Chronic liver disease is associated with greater risk of hospital readmission in patients with heart failure: a nationwide database study in the US.

AIMS: There is limited evidence regarding the association between chronic liver disease (CLD) and the risk of readmission in patients with heart failure (HF). METHODS AND RESULTS: We utilized data from the 2019 Nationwide Readmissions Database (NRD). An index hospitalization was defined as a hospitalization for HF among patients aged ≥18 years with an alive discharge. We categorized patients into two groups, with and without CLD, and compared 30-day and 90-day all-cause and HF-specific readmission. Multiple logistic regression analyses were used to estimate the association between CLD and rea

Impact of community-acquired invasive Escherichia coli disease on mortality and readmissions in elderly patients: a multi-center study in Germany.

BACKGROUND: Extraintestinal pathogenic Escherichia coli (ExPEC) causes invasive E. coli disease (IED), resulting in significant morbidity and mortality, particularly among the elderly. IED can present as bacteremia or sepsis and poses serious health risks for adults aged ≥ 60 years, compounded by increasing antimicrobial resistance. However, real-world data on IED's clinical burden and risk factors in Europe are sparse. This study systematically reviews medical charts across 11 hospitals in Germany to address this gap by evaluating the impact of IED on patient outcomes, mortality rates, and ho

Can we improve early readmission after hip fracture of the adult? A retrospective analysis of 57.544 patients from SNHFR.

Hip fractures in the older persons are associated with high morbidity and mortality rates, with a growing incidence due to an aging population. Early readmission increases dependence and healthcare costs, and identifying the factors associated with readmission could improve care. This study aims to identify factors associated with 30-day readmission following hip fracture in patients aged 75 and older, as well as to explore the relationship between various clinical variables. A multicentric, retrospective observational study was conducted using data from the National Hip Fracture Registry (NHF

Characteristics and Outcomes of Hospitalized Patients With Heart Failure and Sarcoidosis: A Propensity-Matched Analysis of the Nationwide Readmissions Database 2010-2019.

BACKGROUND: Sarcoidosis is associated with a poor prognosis. There is a lack of data examining the outcomes and readmission rates of sarcoidosis patients with heart failure (SwHF) and without heart failure (SwoHF). We aimed to compare the impact of non-ischemic heart failure on outcomes and readmissions in these two groups. METHODS: The US Nationwide Readmission Database was queried from 2010 to 2019 for SwHF and SwoHF patients identified using the International Classification of Diseases, 9th and 10th Editions. Those with ischemic heart disease were excluded, and both cohorts were propensity

Outcome of an elective readmission policy in patients receiving acute myeloid leukaemia consolidation therapy and implications for an outpatient management programme.

Safe outpatient management of acute leukaemia consolidation cycles may enable substantial savings in admission costs. Safety involves the prompt administration of antibiotics in patients with neutropenic fever. Our unit in a metropolitan tertiary referral hospital analysed a cohort of patients spanning a 10-year period, with two key observations: (i) a high proportion of patients living a substantial distance from hospital and (ii) the high incidence and generally prompt onset of fever after severe neutropenia, suggesting this broad applicability of this approach is unfeasible without addressi

Trends and Outcomes of Transcatheter Aortic Valve Implantation in Patients With Peripheral Arterial Disease: Insights From the National Readmissions Database.

Due to shared risk factors, many patients with severe aortic stenosis (AS) also have concomitant peripheral arterial disease (PAD). The readmission rates and long term clinical outcomes of these patients following transcatheter aortic valve implantation (TAVI) in a large sample has not been well defined. The National Readmissions Database (NRD) (2015-2019) was used to identify patients undergoing TAVI. TAVI in patients with PAD was compared with those without PAD using a propensity score matched (PSM) analysis to obtain adjusted odds ratios (aOR) with 95% confidence intervals (CI) of net adver

Descriptive epidemiology of hospitalized patients with bacterial nosocomial pneumonia who experience 30-day readmission in the US, 2014-2019.

INTRODUCTION: Nosocomial pneumonia (NP) remains associated with excess morbidity and mortality. The effect of NP on measures such as re-admission at 30 days remains unclear. Moreover, differing types of NP may have varying impacts on re-admissions. METHODS: We conducted a multicenter retrospective cohort study within the Premier Research database, a source containing administrative, pharmacy, and microbiology data. We compared NP patients readmitted with pneumonia (RaP) as the principal diagnosis to those readmitted for other reasons (RaO) with respect to the type of NP (ventilator-associated
