Application of the Arantius-first approach in a patient with right-sided ligamentum teres.

BACKGROUND: Laparoscopic anatomical liver resection is usually guided by the Glissonian pedicle and hepatic veins as landmarks [1]. However, these landmarks and approaches may not be directly applicable in the presence of anatomical variations. Right-sided ligamentum teres (RSLT) is a rare anatomical variation, occurring in approximately 0.43% of patients [2]. The Arantius-first approach is a useful method for laparoscopic left hemi-hepatectomy in normal anatomy, taking advantage of the laparoscopic caudal view [3, 4]. Although established in normal anatomy, its applicability in anatomical var

Electrocardiographic markers of right heart strain and their association with arrhythmias in patients with COPD.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is frequently associated with cardiovascular complications, including right heart dysfunction and cardiac arrhythmias. Chronic hypoxia and pulmonary vascular remodeling may lead to structural and electrical cardiac changes. Electrocardiography (ECG) may provide simple markers of right heart strain that could help identify COPD patients at increased arrhythmic risk. OBJECTIVE: To evaluate the association between ECG markers of right heart strain and arrhythmias and to identify independent predictors using multivariable analysis. METHODS

Right Temporal Lobe Tuberculoma Presenting as Seizures in a Patient With Pulmonary Tuberculosis.

BACKGROUND Central nervous system (CNS) manifestations of tuberculosis (TB) account for approximately 1-2% of all TB cases worldwide. Tuberculomas, which are granulomatous lesions of the CNS diagnostically confirmed through biopsy, can mimic more common intracranial pathologies, including abscesses and neoplasms. This report describes a patient with pulmonary TB who presented with seizure and was ultimately diagnosed with a right temporal lobe TB mimicking a brain abscess. CASE REPORT A 30-year-old woman with alcohol use disorder, polysubstance use, and pulmonary TB previously treated with rif

A complex case of right heart masses in a leukemia patient: a case report.

BACKGROUND: A patient with acute myeloid leukemia (AML) presented with a cardiac mass of unknown nature. This case underscores the importance of careful monitoring and a multidisciplinary approach in managing and differentiation of rare cardiac complications in leukemia patients. It aims to improve diagnostic accuracy and therapeutic outcomes in similar challenging scenarios. This case report discusses a 33-year-old male who was initially diagnosed with Acute Myeloid Leukemia (AML). During medical check-ups before allogeneic hematopoietic stem cell transplant (allo-HSCT), cardiac ultrasound re

Determinants of Right Heart Hemodynamic Derangement in Patients With and Without Tricuspid Regurgitation.

BACKGROUND: The determinants of tricuspid regurgitation (TR) hemodynamic severity remain to be established. We explored the hemodynamic correlates of right atrial (RA) pressure and stroke volume (SV) in patients with and without TR, and assessed the reliability of the indirect Fick method in relation to TR severity. METHODS: In this observational study, right ventricular (RV) 3-dimensional echocardiography (3DE) was obtained simultaneously with direct Fick right heart catheterization. RVSV3DE and SVRHC were combined to determine the TR regurgitant fraction (RegFr=RVSV3DE-RVSVRHC/RVSV3DE). RA

Patients' Right to Know and Family Influence in Cancer Diagnosis Disclosure in Pakistan.

Non-disclosure of cancer diagnosis is a common practice in Pakistan, with families asking for it to shield loved ones from despair. While meant to protect, this can take away a patient's autonomy to make decisions, prepare for what lies ahead, and spend their time as they wish. Without knowing the truth, patients may undergo difficult treatments without understanding their purpose, leading to confusion and loss of trust. Honest communication, given with care, can preserve dignity and allow people to face illness on their own terms. In many collectivist cultures worldwide, similar practices exi

Isolated Double-Chambered Right Ventricle in an Adult Patient: Diagnosis to Management.

Double-chambered right ventricle (DCRV) is a rare congenital heart defect characterised by the division of the right ventricular cavity into two chambers by anomalous muscle bundles. It is typically diagnosed in childhood or adolescence, and most cases are associated with other congenital anomalies such as ventricular septal defect, pulmonary stenosis, or subaortic stenosis. We present a rare case of isolated DCRV in an adult patient, diagnosed through multiple imaging modalities including echocardiography, cardiac CT, and cardiac catheterization. The patient successfully underwent surgical re

Cone repair and right ventricular resection in an adult patient with Ebstein's anomaly.

Ebstein's anomaly is a rare congenital heart defect characterized by tricuspid valve malformation and right ventricular myopathy. The "cone procedure," introduced by Dr. José Pedro da Silva, represents a major advancement in the repair of Ebstein's anomaly, achieving near-anatomical tricuspid valve reconstruction. We describe the case of a 23-year-old-female with Carpentier type B Ebstein's anomaly, severe tricuspid regurgitation and Wolff-Parkinson-White syndrome, who underwent cone reconstruction combined with right ventricular resection. Detailed preoperative imaging guided the procedure, e

Right colic artery pseudoaneurysm in a patient with rheumatoid arthritis: A case report.

To describe the clinical presentation, diagnosis, and endovascular treatment of a right colic artery (RCA) pseudoaneurysm in a patient with rheumatoid arthritis, highlighting its rarity as a complication of rheumatoid vasculitis. A woman in her early 50s with rheumatoid arthritis presented with a three-day history of epigastric pain, vomiting, and melena. Examination revealed anemia, purpuric rashes, and mild abdominal tenderness. Computed tomography (CT) angiography identified an RCA pseudoaneurysm. Following resuscitation and blood transfusion, selective superior mesenteric artery angiograph

Specific cardiovascular morphology on computed tomography angiography in patients with right isomerism.

BACKGROUND: Children with right isomerism usually accompany complex congenital heart disease, which is associated with pulmonary atresia, common atrioventricular valve, and total anomalous pulmonary venous connection. This study aimed to explore specific cardiovascular morphology associated with right isomerism on computed tomography angiography (CTA). METHODS: We retrospectively reviewed CTA images to assess the atrio-ventricular connections and the spatial relationship between the systemic and pulmonary channels in children with right isomerism. RESULTS: We studied 33 patients (17 females)

Successful management of HeartMate 3 in a patient with arrhythmogenic right ventricular cardiomyopathy.

The management of right heart failure during durable left ventricular assist device (LVAD) support remains an unsolved issue so far. We had a 44-year-old male patient who was diagnosed with arrhythmogenic right ventricular cardiomyopathy and received HeartMate 3 LVAD (Abbott, USA) implantation as a bridge-to-transplant indication. The pump speed was adjusted as low as 4500 rpm to avoid the left ventricular narrowing and interventricular septal leftward shift. Riociguat was administered to decrease the afterload of the right ventricle and increase the preload of the left ventricle, in addition

Patient Perceptions of Emerging Gene Therapies for Arrhythmogenic Right Ventricular Cardiomyopathy.

BACKGROUND: No disease-specific therapy currently exists for arrhythmogenic right ventricular cardiomyopathy (ARVC), a progressive cardiogenetic condition conferring elevated risk for ventricular arrhythmias, heart failure, and sudden cardiac death. Emerging gene therapies have the potential to fill this gap. However, little is known about how adults with ARVC, or any other inherited cardiomyopathy or arrhythmia syndrome, appraise the risks and benefits of gene therapy research and which considerations may influence their decisions about clinical trial participation. METHODS: Twenty adults wi

To flip or not: Case series of coronary angioplasty in patients with right-sided heart.

Coronary angioplasty in patients with a right-sided heart may be difficult due to challenges in engaging the coronary arteries, interpreting angiogram, and further delivering intracoronary therapies. We present our experience of percutaneous coronary intervention in two cases and propose a practical algorithm to approach cardiac catheterization in these patients.

Right ventricular phenotyping in incident patients with idiopathic pulmonary arterial hypertension.

BACKGROUND: Right ventricular (RV) imaging has not a definite role in risk stratification of pulmonary arterial hypertension (PAH) patients. We tested the hypothesis that echocardiography-derived phenotypes, depicting different degrees of RV remodeling and dysfunction, may provide additional prognostic information to current risk stratification tools. METHODS: Consecutive incident PAH patients aged ≥18 years, diagnosed between January 2005 and December 2021, underwent clinical assessment, right heart catheterization, standard echocardiography. Simple echocardiographic variables were combined

Right Ventricular Energy Failure Predicts Mortality in Patients With Pulmonary Hypertension.

Right ventricular (RV) failure has a significant adverse impact on pulmonary hypertension (PH) prognosis. None of the currently used parameters directly assess whether RV fails to provide enough energy output to propel the blood through diseased pulmonary vascular system. Furthermore, most of the current parameters are affected by the volume status of the patient. We aimed to explore whether RV energy failure has a predictive power for mortality on top of the established prognostic risk parameters in patients with PH. We screened 723 cases from our database. A total of 3 sets of binary regress

Undetected language deficits in left or right hemisphere post-stroke patients.

Previous studies have reported that widely used tests for aphasia identification are unable to detect the subtle language deficits of left hemisphere brain damaged (LHBD) individuals. Similarly, the language disorders of individuals with right hemisphere brain damage (RHBD) usually remain undetected, due to the lack of any specialized test for the evaluation of their language processing skills. The aim of the present study was to evaluate the language deficits of 80 individuals suffering from the effects of either a LHBD or RHBD stroke, who were diagnosed as having no aphasia or language defic

A giant thrombus in the right atrium of a patient with acute promyelocytic leukemia M3.

Acute promyelocytic leukemia is a special type of acute myeloid leukemia. Patients with this disease are at high risk of complications. Right atrial thrombosis is a rare but potentially serious complication. A 55-month-old girl with acute promyelocytic leukemia M3 was in her last phase of treatment. Radiologic examination revealed an echo structure in the right atrium that was still present after 6 weeks of anticoagulation treatment with enoxaparin. Cardiac surgery was performed to remove the mass, which was found to be a calcified thrombus. Although this is a rare occurrence, recognition of t

Evolution and triggers of defibrillator shocks in patients with arrhythmogenic right ventricular cardiomyopathy.

INTRODUCTION: Implantable cardioverter-defibrillators (ICDs) can prevent sudden cardiac death due to ventricular arrhythmias in patients with arrhythmogenic right ventricular cardiomyopathy (ARVC). The aim of our study was to assess the cumulative burden, evolution and potential triggers of appropriate ICD shocks during long-term follow-up, which may help to reduce and further refine individual arrhythmic risk in this challenging disease. METHODS: This retrospective cohort study included 53 patients with definite ARVC according to the 2010 Task Force Criteria from the multicentre Swiss ARVC R

Suction thrombectomy of catheter-associated right atrial thrombus in two patients.

Catheter associated right atrial thrombus (CRAT) is an uncommon but potentially life-threatening condition. There are no established guidelines for management, and treatment ranges from systemic anticoagulation and thrombolysis to open surgery. While there have been reports on use of suction thrombectomy for right atrial thrombi, the feasibility and outcomes of suction thrombectomy in CRAT have not been described. These two cases describe a successful off-label use of two devices (Triever 20, Inari Medical, Irvine, CA, and AlphaVac, AngioDynamics, Latham, NY) for thrombectomy in CRAT. Both pat

Subcostal right ventricular free wall strain in patients with pulmonary hypertension.

UNLABELLED: Right ventricular longitudinal strain (RVLS) is frequently used as a measure of right ventricular systolic function. Abnormal RV strain is associated with poor prognosis in patients with pulmonary hypertension (PH); however, the measure is not always easy to obtain in patients with poor apical acoustic windows. OBJECTIVE: This study aims to analyze the RVLS and determine if there is a difference when measured from the apical and subcostal views. METHODS: In this cross-sectional study, we analyzed 22 adult outpatients (= 18 years old), 81% female, mean age 49.9 ± 17.3 years, with
