Effect of CO2 on the perilymphatic oxygen tension in cats.

A microelectrode with a tip of 100micron permitting recording of the oxygen tension in the perilymphatic space according to the polarographic principle and having a minimal drift of 1-25% per hour has been developed. The effects of apnea, hypo- and hyperventilation as well as those of inhalation of pure oxygen, and CO2 upon the perilymphatic PO2 have been measured by placing the electrode in the perilymph through the fenestrated stapedial footplate of 87 adult cats. The correlation between the arterial PO2 and the perilymphatic PO2 is so close that even hypo- or hyperventilation in presence of

Cerebrospinal Fluid Leak During Stapes Surgery: The Importance of Temporal Bone CT Reconstructions in Oblique Anatomically Oriented Planes.

Stapes gusher is a massive flow of perilymph and cerebrospinal fluid leak that fills the middle ear immediately after surgical opening of the labyrinth, such as during stapedectomy. Stapes gusher usually occurs as the result of a congenital malformation that causes an abnormal communication between the perilymphatic space and the subarachnoid space involving the internal auditory canal or the cochlear duct. To date, the potential risk of stapes gusher cannot be assessed preoperatively, as there are not pathognomonic signs suggestive of this complication. However, high-resolution computed tomog

Cannula-based drug delivery to the guinea pig round window causes a lasting hearing loss that may be temporarily mitigated by BDNF.

Sustained local delivery of drugs to the inner ear may be required for future regenerative and protective strategies. The round window is surgically accessible and a promising delivery route. To be viable, a delivery system should not cause hearing loss. This study determined the effect on hearing of placing a drug-delivery microcatheter on to the round window, and delivering either artificial perilymph (AP) or brain-derived neurotrophic factor (BDNF) via this catheter with a mini-osmotic pump. Auditory brainstem responses (ABRs) were monitored for 4 months after surgery, while the AP or BDNF

Magnetic resonance imaging for Ménière's disease: correlation with tone burst electrocochleography.

The newly developed use of magnetic resonance imaging of the human inner ear, on a 3 Tesla scanner with intratympanically administered gadolinium, can now reliably distinguish perilymph from endolymph and visually confirm the presence or absence of endolymphatic hydrops. Transtympanic tone burst electrocochleography is an established, and under-utilised evoked response electrophysiological test for hydrops, but it relies on a symptom score to indicate the likelihood of hydrops being present. The current diagnostic criteria for Ménière's disease make no allowance for any in vivo test, making di

Immunologic responses in experimental cytomegalovirus labyrinthitis.

To better understand the pathogenesis of cytomegalovirus labyrinthitis, a guinea pig model was created. Following inoculation at several sites (cardiac, perilymph, and endolymphatic sac) in both seronegative and seropositive animals, the immunologic, histologic, and electrophysiologic responses were measured. Seronegative animals uniformly showed progressive hearing loss with marked inflammation and degeneration of neural elements. In animals inoculated into the endolymphatic sac, an associated endolymphatic hydrops developed in addition to deafness. Seropositivity protected the hearing, but e

Praziquantel degradation in marine aquarium water.

This study has aimed to determine the anatomical site of labyrinthine fistula in patients of chronic suppurative otitis media at our centre. Labyrinthine fistulae (LF) are caused by abnormal communications between the inner ear and surrounding structures resulting in perilymph leakage and hearing loss. Labyrinthine fistula represents as erosive loss of the enchondral bone overlying the semicircular canals without loss of perilymph. The manifestations of fistula like vertigo, hearing loss vary in severity and complexity, commonly ranging from very mild to incapacitating. Cholesteatoma induced f

Lasers for otosclerosis: CO2 vs. Argon and KTP-532.

The concept of using laser energy to perform stapedotomy and stapedectomy revision is an attractive one. Precision of vaporizing a perfectly round 0.6- to 0.8-mm hole in the stapes footplate, regardless of its thickness or degree of fixation, would introduce an elegant simplicity to a sometimes difficult operation while eliminating mechanical trauma to the inner ear. When revising a previously failed stapedectomy, lasers should enable the otologic surgeon to atraumatically vaporize the obliterating oval window tissue and thus precisely diagnose the cause of the failure. A laser stapedotomy cou

Calcium gradients in inner ear endolymph.

Recent studies suggest that endolymphatic hydrops resulting from the ablation of the endolymphatic duct and sac in guinea pigs may be caused by a disturbance of endolymph calcium homeostasis. A similar disturbance of calcium homeostasis could represent the underlying cause of Ménière's disease. In this study, we mapped the calcium concentrations and electrical potentials throughout the endolymphatic system in normal guinea pigs. Large concentration differences exist between different compartments, including a more than twofold increase along the length of the cochlea. The electrochemical poten

Revision stapedectomy.

During the past decade, the evolution of surgical techniques and philosophy for revision surgery after stapedectomy has accelerated. Lasers, new surgical techniques, and new prostheses now permit the precise identification and reliable correction of the conductive problem, while reducing the risk of postoperative sensorineural hearing loss that plagued nonlaser revision techniques.

Perilymphatic pressure measurement in Meniere's disease.

The aim of this study was to evaluate the perilymphatic pressure, by means of the MMS-10 Tympanic Displacement Analyzer (TDA), in patients with Meniere's disease (MD). Measurements were performed in 37 patients with MD and in 14 normal-hearing subjects. Data were collected from 3 groups: healthy ears of normal-hearing subjects, unaffected ears of patients with MD, and affected ears of patients with MD. Analysis of the results obtained with the TDA showed no significant differences between the 3 groups. Several hypotheses could explain this lack of difference: 1) perhaps indirect measurement of

Non-invasive perilymphatic pressure measurement in patients with Menière's disease.

The MMS-10 Tympanic Displacement Analyser in a new device to measure the perilymphatic pressure in humans. This instrument was used in 25 patients with Menière's disease (28 affected ears) and a group of 50 young normal hearing subjects. No significant differences were found in perilymphatic pressure measurements between the groups. Although measurement parameters showed large inter-individual variation, the intra-individual correlation was good. In patients with Menière's disease, no relationship was found between perilymphatic pressure, hearing threshold, blood pressure, gender or age.

Hormonal modulation of inner ear fluids.

In the cochlea, hormones such as antidiuretic hormone and adrenocorticosteroid hormones are supposed to modulate the endolymph osmolality acting on the labyrinthine water permeability, on the one hand, and on the Na+, K(+)-ATPase, on the other hand. To test the hypothesis that these hormones are involved in the inner ear fluids homeostasis, the electrochemical composition of cochlear fluids was studied in control Long Evans rats, Brattleboro rats that are genetically deprived of antidiuretic hormone, and in adrenalectomized Long Evans rats. The results demonstrated that: i) in Brattleboro rats

Vestibular pathophysiologic changes in experimental perilymphatic fistula.

An animal model of experimental perilymphatic fistula (EPLF) was developed in the guinea pig in order to study vestibular pathophysiology. In experimental animals, 4 microL of perilymph was suctioned from one cochlea via the round window membrane. Changes in vestibular function were as follows. 1) During the acute stage (5 hours postoperatively), spontaneous nystagmus directed toward the normal side was noted in 57.4% of the EPLF animals. This lasted less than 24 hours. 2) One week postoperatively, direction-fixed positional nystagmus toward the lesioned ear was present in 22.7% of the EPLF an

The measurement and manipulation of intralabyrinthine pressure in experimental endolymphatic hydrops.

Three to four months after unilateral surgical ablation of the endolymphatic duct and sac, endolymphatic and perilymphatic pressures were measured in both the normal and hydropic ears of 11 guinea pigs. In normal ears, endolymphatic pressure always approximated perilymphatic pressure. Endolymphatic pressure exceeded perilymphatic pressure in all ears with hydrops, except one in which these pressures were equal. The effect of postural inversion on inner ear pressures were studied in both normal and hydropic inner ears. Normal ears showed endolymphatic and perilymphatic pressure to rise equally

An ipsilateral cochlear efferent loop protects the cochlea during intense sound exposure.

One suggested physiological function of the efferent nerve fibers innervating the cochlea is that they protect the cochlea against the effects of intense sound exposure. In order to test this hypothesis, we studied the effects of intense sound in the presence and in the absence of strychnine which blocks the efferent nerve fibers. The results show that in presence of strychnine an ipsilateral intense sound has a greater effect on the cochlea than in the absence of strychnine. We conclude that the ipsilateral cochlear efferents may act as protectors against intense sound exposure.

The effects of indomethacin on inner ear fluids and morphology.

Metabolic disturbances may affect inner ear fluid homeostasis in various ways. The membrane sodium-potassium pump must play a role in this homeostasis, and as this pump derives energy from A.T.P. it may also depend somewhat on the presence of prostaglandins, through their stimulatory effect on the adenylate cyclase system. Salicylates suppress prostaglandins; and Indomethacin is even more potent in this respect. Could the temporary ototoxic effects of salicylates and Indomethacin be due to prostaglandin suppression? Further, could energy system problems involving prostaglandins be related to t
