Fractures of the humerus in the neonatal period.

BACKGROUND: Fractures of the humerus in neonates can pose a diagnostic challenge, especially when the fracture occurs in the proximal or distal epiphysis. OBJECTIVES: To review our experience in the diagnosis and treatment of birth-related humeral fractures. METHODS: Between the years 2001 and 2009, seven newborn patients and two patients treated in the neonatal intensive care unit sustained a fracture of the humerus. Four of the fractures occurred in the humeral shaft, three in the proximal epiphysis and two in the distal epiphysis. In all the newborn patients the diagnosis was made on the

Incontinentia pigmenti in the neonatal period.

Incontinentia pigmenti (IP) is a rare multisystem disease, X linked dominant disorder. As all X linked dominant diseases, it is usually male-lethal. Female newborn admitted to the neonatal intensive care unit on the first day of life was diagnosed as having probable herpetic infection with vesicular skin lesions distributed on upper right limb and inferior limbs. Family history showed that her 22-year-old mother had hypopigmented lesions on the lower limbs and her 13-month-old sister had hyperpigmented lesions on the trunk and limbs. In newborns, herpes infection emerges as the principal diagn

[Periodic fever with increased IgD].

Periodic fever or hereditary inflammatory fevers are characterized by intermittent inflammatory attacks. Many entities are well recognized today such as familial mediterranean fever (FMF) and hyperimmunoglobulinemia D syndrome (HIDS). We report on the case of a 6-year-old boy referred for evaluation of a recurrent fever associated with chest pain, pneumonitis, or pleuritis since the age of 5 years. Laboratory data showed leukocytosis, a high erythrocyte sedimentation rate, and C-reactive protein; however, a permanent high serum level IgD was noted. Stereotypical episodes of fever appeared ever

Spontaneous periodic hypothermia and hyperhidrosis.

We present a patient diagnosed with Shapiro syndrome without corpus callosum agenesis. A 4-year-old-girl was admitted to the hospital with complaints of sweating, cooling, and drowsiness that continued during the last week of her admission. Attacks occurred almost daily, and lasted for about 1 hour. All laboratory findings, as well as Holter and echocardiography results, were normal. Cranial magnetic resonance imaging demonstrated an intact corpus callosum, and electroencephalography obtained during an attack revealed normal findings. However, technetium 99m-labeled hexamethylpropylene amine o

Erythrocyte disorders in the perinatal period.

Anemia is a commonly encountered problem in the fetal and neonatal period, and can lead to significant morbidity and mortality. Intrinsic disorders of the erythrocyte, such as the hemoglobinopathies, enzyme deficiencies, and membrane defects are common causes of neonatal anemia. Genetic diseases that lead to decreased erythrocyte production, such as Diamond-Blackfan anemia, Schwachman-Diamond syndrome, and Congential Dyserythropoietic Anemia, are rare causes of perinatal anemia, but are important to recognize as they are often associated with other congenital abnormalities and require speciali

Pulmonary endothelium in the perinatal period.

The pulmonary circulation is the only system in the body which does not have a dress rehearsal in utero. The pulmonary endothelium plays a pivotal role in ensuring that the resistance falls rapidly after birth so that the lungs can receive and process the entire cardiac output for the first time. It transduces signals triggered by environmental changes to the underlying smooth muscle cells, controlling their reactivity and regulating pulmonary vascular tone. It also clears alveolar fluid. This review addresses the mechanisms involved in these processes and considers failure of adaptation, the

Hereditary periodic fever syndromes.

The hereditary periodic fevers are a group of Mendelian disorders characterized by seemingly unprovoked fever and localized inflammation. Recent data indicate that these illnesses represent inborn errors in the regulation of innate immunity. Pyrin, the protein mutated in familial Mediterranean fever, defines an N-terminal domain found in a large family of proteins involved in inflammation and apoptosis. Through this domain pyrin may play a role in the regulation of interleukin (IL)-1beta, nuclear factor (NF)-kappaB, and leukocyte apoptosis. Cryopyrin/NALP3, another protein in this family, is m

Are there critical periods for musical development?

A critical period can be defined as a developmental window during which specific experience has a greater effect than at other times. Musical behavior involves many skills, including the basic encoding of pitch and time information, understanding scale and harmonic structure, performance, interpretation, and composition. We review studies of genetics, behavior, and brain structure and function in conjunction with the experiences of auditory deprivation and musical enrichment, and conclude that there is more supporting evidence for critical periods for basic than for more complex aspects of mus

[HELLP syndrome in the postpartum period].

The HELLP syndrome was described by Weinstein as a complication of pregnancy induced hypertension (PIH). There are few clinical reports about HELLP during postpartum period. We present the history of 31 years old pregnant patient, who developed in 36 week of her first pregnancy PIH, pregnancy was terminated in 39 week by cesarean section and postpartum period was complicated with HELLP syndrome (class I). The patient was treated surgically (hysterectomy), than continue at ICU. The treatment was successful without any late organ complications.

Chlamydia infections in the neonatal period.

The paper presents current knowledge about the clinical course, diagnostic and treatment of chlamydial infections in the neonatal period. In the neonatal period, infections caused by Chlamydia trachomatis are in the majority of maternal origin. Chlamydia mother to child transmission occurs during vaginal birth. It can also occur but rarely, during caesarean section after premature rupture of membranes. Chlamydia trachomatis infection of the genitourinary system in pregnancy increases the danger of intrauterine growth restriction and foetal death. It rises the perinatal morbidity of foetuses an

Modelling the incubation period of anthrax.

Models of the incubation period of anthrax are important to public health planners because they can be used to predict the delay before outbreaks are detected, the size of an outbreak and the duration of time that persons should remain on antibiotics to prevent disease. The difficulty is that there is little direct data about the incubation period in humans. The objective of this paper is to develop and apply models for the incubation period of anthrax. Mechanistic models that account for the biology of spore clearance and germination are developed based on a competing risks formulation. The m

We have examined 107 women aged 46 to 55 years, 47 among them with complaints of vertigo (Group I), 30 women aged 45 to 50 years with symptoms of menopause diagnosed in Outpatient of Endocrinology Clinic (Group II) and 30 healthy women aged 45-54 (control Group). Blood concentration of sexual hormones (estradiol, LH, FSH, progesterone), sodium and potassium blood level, electronystagmography (ENG), optokinesis, eye tracking test, static and dynamic posturog

[Premorbid period of anorexia nervosa].

Premorbid period of anorexia nervosa was studied in 96 female patients. In 64 cases (group 1) anorexia nervosa was as a separate disease and in 32 cases (group 2)--as a syndrome of endogenous mental disorder. In group 1 premorbid period was characterized by the features of neuropathy and residual organic disorders in combination with accumulation of subjects with personality disorders in the patient's family that promoted development of prolonged neurotic disorders. In group 2 there was hereditary psychosis load, high frequency of obstetrical and other perinatal complications, overvalued ideas

Anaphylaxis during the perioperative period.

Anesthesiologists use a myriad of drugs during the provision of an anesthetic. Many of these drugs have side effects that are dose related, and some lead to severe immune-mediated adverse reactions. Anaphylaxis is the most severe immune-mediated reaction; it generally occurs on reexposure to a specific antigen and requires the release of proinflammatory mediators. Anaphylactoid reactions occur through a direct non-immunoglobulin E-mediated release of mediators from mast cells or from complement activation. Muscle relaxants and latex account for most cases of anaphylaxis during the perioperativ
