Linear osteochondromatosis in a cat.

A domestic shorthair cat was presented with quadriparesis and lumbar hyperesthesia that progressed over 4 months. There were linear and amorphous radiopaque masses throughout the soft tissue surrounding the long bones, vertebral bodies, ribs, pelvis, and scapula. The diagnosis of osteochondromatosis was confirmed by histopathology. Unlike previously reported patients with osteochondromatosis, most of the calcified masses in this cat were not connected to the periosteum; some were linear and were arranged parallel to the long bones involved.

Computed tomographic appearances of sternocostoclavicular hyperostosis.

Computed tomographical analysis of sternocostoclavicular hyperostosis was performed in 27 patients. In the earliest stage hyperostosis occurred around the cartilaginous portion of the first ribs. The sternoclavicular joint space was preserved even in the late stage III of the disorder. These findings suggest that sternocostoclavicular hyperostosis develops around the costal cartilage including periosteum, perichondrium, and the ligamentous structures, and that the sternoclavicular joint is not primarily involved. It is also suggested that perichondritis and periostitis play important roles in …

[Anatomicoclinical types of mandibular fractures in children (author's transl)].

The author describes the particular type of fragility zones found in children's mandibles, which vary during growth, and discusses the anatomicoclinical types of fractures observed from the earliest age (symphysial disjunctions) until the end of mixed dentition. Emphasis is placed on one particular type, described in a previous communication: the paramedian form, which can be explained by the atomical position of certain dental germs, as well as the importance of reconstitution of the periosteum in displaced sub-condylar fractures.

Modification of Marshall-Marchetti-Krantz operation.

In a classic article published in 1949 Marshall, Marchetti, and Krantz demonstrated that stress incontinence in women without uterine prolapse could be corrected by a simple vesicourethral suspension. Beginning in 1960 one of the authors (W.E.C.) became concerned about suturing the urethra to the periosteum of the pubis. It occurred to him that use of the upper sutures only to pull up the bladder and hold its anterior wall to the back of the rectus muscles might be just as effective, and this has proved to be so.

Fibrous interposition causing valgus deformity after fracture of the upper tibial metaphysis in children.

The development of genu valgum in a child after a fracture of the upper metaphysis of the tibia with a medial gap is due to the interposition of a flap of fibrous tissue consisting of pes anserinus and periosteum avulsed from the lower fragment. The ensuing biomechanical disturbance induces bowing of the shaft and asymmetrical growth at both ends of the bone. Four cases of established deformity are reported, together with two cases of fresh fracture successfully treated by surgical clearance of the fibrous tissue from the gap.

Surgical treatment of solitary bone cysts.

A special method of subperiosteal segmental resection is described as an operative treatment for solitary bone cysts in long bones. This procedure exploits the high osteogenetic potency of the periosteum in adolescence. An autogenous or homologous fibula bone graft locked between the ends of the cyst serves to maintain the length of the bone fragments and acts as a guiding rod for continued subperiosteal new bone formation. There is full reconstruction of bone stability and function at both the cyst region and the donor region (fibula).

Circumscribed Myositis Ossificans of the Masseter Muscle.

The authors report a case of myositis ossificans traumatica of the masseter muscles. A 21-year-old man developed masseter myositis after orthognathic surgery to treat facial asymmetry. The myositis may have been triggered by factors such as calcification of a hematoma, periosteum implantation after surgical trauma, and local soft tissue injury. An intraoral incision was made to excise the ectopic bone, resulting in resolution of the patient's symptoms including limited oral opening.

Severe skin necrosis in adult: an unusual complication of fulminant meningococcal sepsis.

We report on a 20-year-old female high-school student with fulminant meningococcal sepsis presenting as severe skin necrosis in lower extremities. The wound surface affected 2% of her total body surface area and was covered with black eschar. The adipose tissue beneath the eschar was degenerated and turned to necrosis as to deep fascia and periosteum of lower tibia. Excision of necrotic eschar, then covered with flaps on both legs resulted in a successful outcome.

[Corrective osteotomy using Hoffmann II external fixators for extra-auricular malunion of the distal radius].

The authors present the technique and clinical results of a corrective osteotomy for malunion following extra-articular fractures of the distal radius. External fixation allows restoration of the length of the radius with minimal damage to the periosteum. It allows early mobilization of the radiocarpal joint. The position of the distal pins provides intraoperative visual control of the angular correction. The minimal invasive approach decreases the risk of iatrogenic lesion of an extensor tendon or a sensitive branch of the radial nerve. This technique is easy and reliable.

Functional Outcomes of Local Pedicle Graft Interpositional Arthroplasty in Adults With Severe Freiberg's Disease.

BACKGROUND: Conservative treatment or debridement is generally sufficient for Freiberg's disease grades I and II but operative intervention for the late stages of the disease process (III-V) is more challenging. Debridement alone is not sufficient and various forms of arthroplasty have been put forward. We have evaluated the outcomes of patients treated with an interpositional arthroplasty technique using a pedicle graft of periosteum and fat made into a "Rollmop" spacer for severe Freiberg's disease. No results have previously been reported for this technique. METHODS: Twenty-five consecutiv…

Surgical Management of a Rare Complication of Mastoidectomy: A Retroauricular Cutaneous-Mastoid Fistula.

Our goal was to describe a case of a rare complication that developed as a result of multiple surgical interventions over the years due to chronic mastoiditis - specifically, a pathological connection between a retroauricular skin defect and the mastoid cavity - and its surgical management. Based on subjective and objective findings, a surgical intervention was planned, including radical cavity obliteration and closure of the cutaneous-mastoid fistula using the temporomastoid periosteum. The surgery was successfully completed, resulting in improved hearing and the elimination of the infectious…

Percutaneous femoral derotational osteotomy for excessive femoral torsion.

Femoral derotational osteotomy is an acceptable treatment for excessive femoral torsion. The described procedure is a minimally invasive single-incision technique based on an intramedullary saw that enables an inside-out osteotomy, preserving the periosteum and biological activity in the local bone and soft tissue. After the osteotomy is complete and correction is achieved, an expandable intramedullary nail is used to achieve immediate stability, without the need for locking screws. Indications, tips, and pitfalls related to this novel osteotomy technique are discussed.

Coincidence of conchachoanal polyp and complete mulberry hypertrophy of inferior concha.

We report two unique cases of 50- and 28-year-old Turkish men with coincidence of conchachoanal polyp and mulberry hypertrophy of the inferior concha. We operated on both cases with endoscopic stripping of the polypoid tissue while preserving the periosteum and healthy mucosa of the inferior nasal concha. The polypoid tile of the left inferior nasal concha was resected in the case with mulberry hypertrophy at only the tail of the concha. Both cases reported significant relief of symptoms after the operations.

Spinous process fractures associated with Baastrup disease.

Patients with Baastrup disease may experience pain owing to irritation of the periosteum or adventitial bursae between abutting spinous processes. We are not aware of any reports in the literature that describe stress fractures of the spinous fractures in patients with Baastrup disease. In this case report, we present two cases of spinous process fractures in lumbar vertebrae associated with Baastrup disease and describe their appearance with conventional radiography, bone scintigraphy, computed tomography (CT) and magnetic resonance (MR) imaging.

Blepharochalasis syndrome.

Patients with chronic forms of blepharochalasis often develop eyelid deformities characterized by blepharoptosis and prolapse of the orbital fat and lacrimal gland. Some individuals have an acquired form of blepharophimosis, secondary to the dehiscence of the canthal tendons. In this late stage of the condition, the tendons still adhere to the periosteum of the orbital rims and loss of fixation occurs at the distal attachment between the tendons and the eyelid tissues. This results in a horizontally shortened palpebral fissure and a rounded deformity of the lateral canthal angle. Surgery remai…

[Periosteal reactions in bone tumors (author's transl)].

Morphology of solid, lamellar and spicular periosteal reactions of benign and malignant primary and metastatic bone tumors in x-ray-pictures with regard to common and specific characteristics of different bone tumors is presented. The specific behaviour of the periosteum itself and of the subperiosteal region as an expression of the reactive biodynamics in skeletal neoplasia is demonstrated; and the diagnostic value of the radiograph is explained. The possibilities but also the limitations of sophisticated radiologic image analysis in establishing a differential diagnosis and a diagnosis of bo…

[The operative cavity in the treatment of congenital atresia (author's transl)].

In the treatment of the various malformations of the external and middle ear (atresia of the bony canal, malformed ossicles, reduced volume of the middle ear in varying degrees, atypical course of the facial nerve), the indirect approach to the middle ear via the antrum appears to be the safest operative procedure for a tympanoplasty in cases of congenital atresia. A wide open external canal is required for a permanently successful hearing result. A special double plasty using skin and periosteum for the anterior canal wall has proved to be the method of choice.
