The sound of engagement: assessing the feasibility and acceptability of an AI-generated personalized podcast as a between-session resource for therapy.

INTRODUCTION: Consistent client engagement with between-therapy session activities (i.e., homework) is a strong predictor of positive psychotherapy outcomes. Because traditional homework activities aren't always tailored to the individual, they can fail to provide the support patients need to translate session insights into real-life change. The integration of digital tools into weekly psychotherapy presents an opportunity to overcome these limitations through personalized blended care. METHODS: This study describes the development and initial validation of Talkcast, a novel AI-powered tool t

A miniaturized chemiluminescent thrombin generation assay to assess the effect of APCC in persons with hemophilia A with inhibitors: an observational study.

BACKGROUND: Monitoring of bypassing agents in persons with hemophilia A with inhibitors is complicated, especially when multiple treatment modalities are combined. The thrombin generation assay (TGA) might address these issues but is hampered by limited availability and standardization. OBJECTIVES: To assess the performance of a novel microfluidic chemiluminescent TGA (microfluidic-TGAlum) to monitor the effect of anti-inhibitor coagulant complex (APCC) in persons with hemophilia A with inhibitors. METHODS: The microfluidic-TGAlum was used in factor VIII (FVIII)-deficient plasma samples with

Does personalization in exposure assessment change ambient air pollution exposure-response relationships? A panel study.

Accurate exposure assessment is crucial to understand linkages between ambient air pollution and cardiopulmonary disease. Air quality monitors (AQM) are widely used, but do not account for personal behaviors. We compare the exposure-response relationships between ambient air pollution (PM2.5 and O3) and cardiopulmonary biomarkers in a panel study using both stationary AQM and Exposure Model for Individuals (EMI). Participants (n = 28) underwent 3-5 sessions totaling 134 visits. Participants underwent spirometry and blood sampling. PM2.5 and O3 concentrations were calculated for each visit (lag

Prospective evaluation of comprehensive geriatric assessments in multidisciplinary bladder cancer care and implications for personalized vulnerability phenotyping.

PURPOSE: Frailty predicts adverse outcomes in bladder cancer (BC). Current guidelines endorse completion of Comprehensive Geriatric Assessments (CGAs) in older adults prior to treatment election to objectively measure frailty, however, these are rarely performed in urologic practice due to inadequate resources. We hypothesized CGA implementation would be feasible and identify multifaceted vulnerabilities beyond standard risk assessments in a multidisciplinary BC clinic and developed a novel method to visualize "vulnerability phenotypes" to guide supportive interventions. METHODS: Adults with

Computational Phenomapping of Randomized Clinical Trial Participants to Enable Assessment of Their Real-World Representativeness and Personalized Inference.

BACKGROUND: Assessing the generalizability of randomized clinical trials (RCTs) to real-world patients remains challenging. We propose a multidimensional metric to quantify the representativeness of an RCT cohort in an electronic health record (EHR) population and estimate real-world effects based on individualized treatment effects observed in the RCT. METHODS: We identified 65 clinical prerandomization characteristics of patients with heart failure with preserved ejection fraction within the TOPCAT (Treatment of Preserved Cardiac Function Heart Failure with an Aldosterone Antagonist Trial)

Spatial distribution and ecological risk assessment of pharmaceuticals and personal care products across China's coastal waters.

Pharmaceuticals and Personal Care Products (PPCPs) constitute water pollutants of emerging concern due to their extensive occurrence and potential ecological impacts. In this nationwide-scale study of China's coastal systems, we integrated high-resolution spatial analysis (288 water samples across four marine regions) with multivariate source apportionment to reveal the distribution, drivers, and risks of 30 antibiotics and 14 other PPCPs. Antibiotics ranged from N.D. to 564 ng/L (sulfonamides dominant), while other PPCPs spanned N.D.-961 ng/L (phenoxy carboxylic acids predominant). Spatial pa

Toxocariasis as an Elderly Zoonosis: Seroprevalence, Neurocognitive Assessment, and Associated Risk Factors in Persons 50 Years and Older.

Toxocariasis, a geohelminthiasis caused by Toxocara canis and Toxocara cati nematodes, has an estimated 19% seroprevalence worldwide. Although children have been considered more prone to infection, adults and the elderly may also be considered at risk. Accordingly, the present study aimed to assess the associated risk factors for Toxocara spp. seropositivity in a population of 290 individuals over 50 years old, attended by the Public Health System in western São Paulo state. Socioepidemiological information was obtained by a semi-structured questionnaire, a blood (serum) sample, and a neurocog

Machine learning for personalized risk assessment of HIV, syphilis, gonorrhoea and chlamydia: A systematic review and meta-analysis.

BACKGROUND: Machine learning (ML) shows promise for sexually transmitted infection (STI) risk prediction, but systematic evidence of its effectiveness remains fragmented. METHODS: We systematically searched six electronic databases, three preprint archives and conference proceedings (January 2010-April 2024). Studies reporting quantitative performance metrics for supervised ML-based STI risk prediction models were included. We used a bivariate random-effects model to estimate pooled sensitivity, specificity and area under the curve (AUC). The risk of bias was assessed using the Prediction mod

The International Evidence-based Guidelines for the Assessment and Management of Polycystic Ovary Syndrome: An Exemplar of Person-centred Care.

In this opinion piece, we appraise the International Evidence-based Guideline for the Assessment and Management of Polycystic Ovary Syndrome 2023 from a person-centric perspective. We discuss how the authors balance evidence with empathy and offer excellence in clinical decision-making while ensuring the empowerment of the affected individual. We note how they skilfully use powerful words and phrases to capture the essence of person-centred care. Finally, we suggest how these guidelines can be strengthened and how they can be used to create a template for guidance on the management of other ch

A Personalized Data Dashboard to Improve Compliance with Ecological Momentary Assessments in College Students: Protocol for a Microrandomized Trial.

BACKGROUND: Ecological momentary assessments (EMA) are ideal for capturing the dynamic nature of young adult substance use behavior in daily life and identifying contextual risk factors that signal higher-risk episodes. These methods could provide a signal to trigger real-time intervention delivery. Study compliance and engagement are common barriers to participation but may be improved by personalizing messages. This study compares compliance outcomes between one group of young adults receiving standard (generic) prompts at each assessment and another group that received additional personaliz

Assessing personalized responses to anti-PD-1 treatment using patient-derived lung tumor-on-chip.

There is a compelling need for approaches to predict the efficacy of immunotherapy drugs. Tumor-on-chip technology exploits microfluidics to generate 3D cell co-cultures embedded in hydrogels that recapitulate simplified tumor ecosystems. Here, we present the development and validation of lung tumor-on-chip platforms to quickly and precisely measure ex vivo the effects of immune checkpoint inhibitors on T cell-mediated cancer cell death by exploiting the power of live imaging and advanced image analysis algorithms. Using a quantitative assessment of propulsion biomechanics in wheelchair racing to guide the design of personalized gloves: a case study.

This study with a T-52 class wheelchair racing athlete aimed to combine quantitative biomechanical measurements to the athlete's perception to design and test different prototypes of a new kind of rigid gloves. Three personalized rigid gloves with various, fixed wrist extension angles were prototyped and tested on a treadmill in a biomechanics laboratory. The prototype with 45° wrist extension was the athlete's favourite as it reduced his perception of effort. Biomechanical assessment and user-experience data indicated that his favourite prototype increased wrist stability throughout the propu

Psychometric assessment of the Runyankole-translated Marlowe-Crowne Social Desirability Scale among persons with HIV in Uganda.

BACKGROUND: Social desirability can negatively affect the validity of self-reported measures, including underreporting of stigmatized behaviors like alcohol consumption. The Marlowe-Crowne Social Desirability Scale (SDS) is widely implemented and comprised of Denial and Attribution Domains (i.e., tendencies to deny undesirable traits or attribute socially desirable traits to oneself, respectively). Yet, limited psychometric research has been conducted in sub-Saharan Africa, where the prevalence of unhealthy alcohol consumption is high as well as religiosity and hierarchical social norms. To ad

Evaluation of a deep learning-enabled automated computational heart modelling workflow for personalized assessment of ventricular arrhythmias.

Personalized, image-based computational heart modelling is a powerful technology that can be used to improve patient-specific arrhythmia risk stratification and ventricular tachycardia (VT) ablation targeting. However, most state-of-the-art methods still require manual interactions by expert users. The goal of this study is to evaluate the feasibility of an automated, deep learning-based workflow for reconstructing personalized computational electrophysiological heart models to guide patient-specific treatment of VT. Contrast-enhanced computed tomography (CE-CT) images with expert ventricular

Validation of a Spanish Adaptation of the Gambling Symptom Assessment Scale (G-SAS) in Persons with Recent History of Gambling.

Gambling is becoming increasingly frequent and problematic, especially due to the explosion of online alternatives. Evaluating the severity of gambling symptomatology is therefore more important than ever. However, innovations in the gambling field have generally focused on its treatment rather than its evaluation. The Gambling Symptom Assessment Scale (G-SAS) is a well-established measure of gambling-related symptomatology (e.g., gambling urges, gambling-related thoughts and behaviours, and interpersonal functioning). The aim of this study is to validate a Spanish adaptation of the G-SAS so t
