A newly recognized skeletal dysplasia with rhizomelic limbs and retinitis pigmentosa.

A girl with severe pre- and post-natal short stature, low-pitched voice, retinitis pigmentosa, photophobia, short neck, broad thorax, platyspondyly, rhizomelic shortening of the long bones, bilateral subluxation of the hips, advanced maturation of the epiphyses, and apparently normal intellectual development is described. The girl's parents are first cousins. Two subsequent pregnancies had ended in spontaneous abortion with polyhydramnios and severe growth retardation. To the best of our knowledge, this association had not been previously reported, and may be considered a newly recognized synd…

Ophthalmological and biological features of the oculorespiratory syndrome after influenza vaccination.

We report the ophthalmological and laboratory findings of 6 patients who, after influenza vaccination, were affected by oculorespiratory syndrome (ORS), complaining of red eyes, photophobia, blurred vision, palpebral edema, ocular pain and itching, and conjunctival secretions. The conjunctivae were mildly hyperemic with few follicles, but the ophthalmological examination findings were otherwise normal. Patients had lymphopenia and decreased levels of the total hemolytic complement and the third and fourth component of the complement. We conclude that ORS causes conjunctivitis and seems to invo…

Canine mycotic keratoconjuntivitis caused by Acremonium kiliense.

Mycotic keratoconjuntivitis caused by Acremonium kiliense in a German Shepherd bitch was diagnosed with the aid of laboratory tests. The dog presented with photophobia, tearing, corneal edema and reduction of the visual capacity. A thick white layer partially covered the right eye. The left eye showed irritation and small brown stains which were diagnosed as pigmentary keratitis. The initial treatment consisted of 2% yellow mercury oxide. Natamycin was used as final treatment. Seven days later, the natural brightness of the eye as well as the visual capacity were restored.

Lower eyelid laxity and ocular symptoms.

We measured lower eyelid length in 288 healthy subjects to establish a norm and then in 95 patients with complaints of intractable photophobia, foreign-body sensation, chronic tearing, or chronic recurrent conjunctivitis. The difference in length was statistically significant (34.25 +/- 2.43 mm for healthy men vs 36.51 +/- 2.59 mm in male patients [P - .001] and 32.97 +/- 2.46 mm for healthy women vs 33.95 +/- 2.9 mm for female patients [P = .01]). There appears to be a cause-and-effect relationship between a flaccid lower eyelid and these ocular symptoms.

[Varicella zoster meningitis with associated vasculitis in a two-month post-partum woman].

Reactivation of varicella zoster virus occurs more often in the elderly or immunocompromised persons. During pregnancy increased anti-inflammatory cellular response promotes tolerance of foetal antigens. Post-partum a shift towards an inflammatory response may facilitate the reoccurrence of latent infections. Varicella-associated vasculitis can lead to ischaemic lesions in the brain. We report a case of a two-month post-partum woman suffering from headache, nausea, vomiting, photophobia and radicular pain with varicella zoster meningitis and an ischaemic lesion in the splenium of corpus callos…

Migraine.

Migraine is a chronic neurovascular disorder characterised by recurrent attacks of moderate-to-severe head pain, accompanied by transient but disabling symptoms such as aura, nausea, vomiting, phonophobia, and photophobia, reflecting the involvement of multiple systems. Migraine affects and disables a large proportion of the global population. The pathogenesis of migraine involves the interplay of genetic and epigenetic factors across complex neurosensory and neurovascular networks. Decades of research have elucidated key components of these pathways, driving the development of mechanism-based…

Coccidioidomycosis Causing Hydrocephalus.

Coccidoidomycosis is caused by Coccidoides imitis and C. posadasii infection. Coccidioidomycosis is also known as San Joaquin Valley fever or desert rheumatism. It is only seen in the Southwest United States (Arizona, New Mexico, California, Texas, Nevada, and Utah), and Central and South America. This infection is acquired by the inhalation of fungal spores in the air. The most severe extrapulmonary coccidioidomycosis is coccidioidomycosis meningitis, in which patients present with headaches, photophobia, altered mental status, and hearing difficulties. This is a case report of a person with …

Developments in the treatment of benign essential blepharospasm.

PURPOSE OF REVIEW: To review new developments in the medical and surgical treatment options for benign essential blepharospasm (BEB). RECENT FINDINGS: Botulinum toxin injections remain the mainstay treatment for BEB with several formulations currently commercially available. Reports in the medical literature support photochromatic modulation for the symptoms of photophobia, as well as oral medications and surgical myectomy for control of the motor signs of eyelid protractor spasm. SUMMARY: Although there remains no cure for BEB, several treatment options are available to effectively manage t…

Acute Central Vision Loss in an IV Drug User.

This report describes the case of a 21-year-old heroin user who presented with a 6-day history of decreased vision in her right eye, preceded by 1 week of headache and tender scalp nodules, neck stiffness, and photophobia. A broad infectious workup for acute vision loss was completed, and she was ultimately presumed to have acquired toxoplasmic chorioretinitis (ocular toxoplasmosis). We review the initial workup for chorioretinitis, and the epidemiology, diagnosis, and treatment of ocular toxoplasmosis. Intravenous drug users may be at increased risk of acquired ocular toxoplasmosis.

[A rare manifestation of sarcoidosis].

This article reports the case of a 14-year-old boy who was presented in the case conference with symptoms of decreased visual acuity, scintillating scotomas and photophobia. Physical examination revealed right facial paralysis, parotid gland swelling, high fever and poor general condition. Ophthalmoscopy revealed anterior and posterior uveitis including macular edema and chorioretinal infiltrates. Angiography revealed a dense pattern of hyperfluorescent lesions and these observations resulted in the diagnosis of Heerfordt syndrome. Under systemic prednisolone therapy, symptoms were reduced and…

Vogt-Koyanagi-Harada syndrome in a 6-year-old Hispanic boy.

A 6-year-old Hispanic boy presented to the ophthalmology department with complaints of pain, photophobia, and blurry vision of both eyes. He was found to have bilateral granulomatous panuveitis, cataracts, and high intraocular pressures. He later developed multiple asymptomatic, ovoid, hypopigmented patches over the mid-lumbosacral back. Biopsy of lesional skin was significant for low melanocyte counts and a mild lymphocytic infiltrate. The patient was diagnosed with Vogt-Koyanagi-Harada syndrome (VKH). This article reviews the literature regarding the cutaneous presentation of VKH.

[Sinus thrombosis in a healthy female taking oral contraceptives].

A 19 year-old female presented with sudden onset of severe headache and photophobia. Clinical examination, blood tests and lumbar puncture were unremarkable. The patient was taking drospirenone/ethinyl estradiol oral contraceptives. An unenhanced computed tomography (CT) was performed with findings suspicious for sinus thrombosis. A confirmatory CT venography was made, which was diagnostic for sinus thrombosis. Thrombophilia screening was performed and showed normal values. Estrogen-containing hormonal contraceptives was the only risk factor, and this therapy has venous thrombosis as a rare sid…

Recurrent meningitis attributable to herpes simplex virus-2 in a child.

A boy manifested episodes of recurrent meningitis that were attributed to herpes simplex virus-2 infection. He presented no concurrent or previous history of involvement of the genitourinary system. He exhibited headaches, dizziness, photophobia, loss of balance, and vomiting. He underwent three episodes of aseptic meningitis. The herpes simplex virus-2 etiology was confirmed by polymerase chain reaction of the cerebrospinal fluid in the last two episodes. After the third occurrence, he was treated with acyclovir. Five years have elapsed since then, without the recurrence of aseptic meningitis…

The big brown eyes of Samuel Pepys.

Samuel Pepys (1633-1703) is known for writing the finest diary in the English language. He was a man of remarkable accomplishments who transformed the English Navy, was president of the Royal Society, and was a member of the British Parliament. He survived the Great Plague and imprisonment in the Tower of London. During the years when he was writing the diary, Pepys began to experience great pain in his eyes when reading and writing and from photophobia, which caused him to give up writing the diary. Pepys also had an ultimately unjustifiable fear of blindness.

Tension headache.

BACKGROUND: Tension headache and migraine are opposite ends of a benign recurrent headache spectrum. OBJECTIVE: To provide an approach to the diagnosis of benign recurrent headache. DISCUSSION: Tension headache is characterised by symmetry, non-disabling severity and the absence of vascular features (throbbing quality, nausea and photophobia). The differentiation between tension headache and attacks with more vascular features has important therapeutic implications as attacks with significant vascular features tend to respond better to antimigrainous approaches rather than those employed in …

Ewing's tumor metastatic to the clivus, simulating meningitis: case report.

Ewing's sarcoma, which is regarded as one of the most lethal primary bone tumors, lies in the domain of the orthopedic surgeon because it occurs most commonly in the shaft of the long bones, especially in the lower extremities. Pain, leukocytosis, fever, anemia, and an elevated erythrocyte sedimentation rate are commonly seen. We are presenting a case of Ewing's sarcoma of the left greater trochanter with metastasis to the clivus producing a bilateral 6th nerve palsy. The presence of fever, nuchal rigidity, and photophobia simulated meningitis. The rapid evolution of radiological signs will be…
