Sudden cardiac death in Air Force recruits. A 20-year review.

We reviewed the clinical and autopsy records of the 19 sudden cardiac deaths that occurred among the 1,606,167 US Air Force healthy, medically screened recruits (90% male; 17 to 28 years old) during a 42-day basic training period between 1965 and 1985. Sixteen (all male) died suddenly of underlying structural heart disease, whereas no anatomic cause of death was identified in the remaining three. Thirty-two nonsudden, noncardiac deaths occurred during the same period, and only two had structural heart disease. Strenuous physical exertion was associated with sudden death in 17 of 19 cases (0.01…

Atypical Presentation of Exertional Rhabdomyolysis in Older Adults: Two Case Reports and Literature Review.

BACKGROUND Rhabdomyolysis (RM) is caused by drugs, trauma, infection, and high-intensity exercise. As a subtype, exertional rhabdomyolysis (ER) is commonly triggered by high-intensity exercise and clinically manifests as myalgia, dark urine, and other symptoms. Auxiliary tests typically show elevated creatine kinase (CK) and myoglobin (Mb) levels. Aggressive rehydration is the primary treatment for RM. After fluid replacement therapy, serum CK levels usually decline rapidly. CASE REPORT We describe 2 patients with ER who displayed atypical symptoms induced by short-term high-intensity physical

Effects of preferred music on internal load in adult recreational athletes: a systematic review and meta-analysis.

INTRODUCTION: When exercising to preferred music (PM), participants found more satisfaction and less typical exercise-related fatigue, which made it easier and more enjoyable to maintain the physical activity (PA) until the exercise goals were achieved. The purpose of this review and meta-analysis was to determine whether changes on internal training load in adult recreational athletes were modified by listening to PM and non-preferred music (NPM), during different PA. EVIDENCE ACQUISITION: A music-focused search was performed on the Google Scholar, PubMed, and Web of Science databases to ide

Rebound Hyperthermia in Exertional Heat Stroke.

Exertional heat stroke (EHS) is a life-threatening condition requiring rapid reversal of hyperthermia to prevent poor health outcomes. Current treatment protocols aim for a cooling rate of 0.15 C/min using various modalities. This case report details a 22-year-old male who, despite initial successful cooling measures, experienced rebound hyperthermia, necessitating the use of endovascular cooling (EVC). The patient collapsed during a 19.3 km (12-mile) ruck march in Fort Moore, Georgia, with an initial core temperature of 41.6ºC. Conventional cooling methods, including ice sheets and chilled in

An Investigation of Surface EMG Shorts-Derived Training Load during Treadmill Running.

The purpose of this study was two-fold: (1) to determine the sensitivity of the sEMG shorts-derived training load (sEMG-TL) during different running speeds; and (2) to investigate the relationship between the oxygen consumption, heart rate (HR), rating of perceived exertion (RPE), accelerometry-based PlayerLoadTM (PL), and sEMG-TL during a running maximum oxygen uptake (V˙O2max) test. The study investigated ten healthy participants. On day one, participants performed a three-speed treadmill test at 8, 10, and 12 km·h-1 for 2 min at each speed. On day two, participants performed a V˙O2max test.

Divergent effects of oral cannabis oil extracts marketed as C. indica or C. sativa on exertion of cognitive effort in rats.

The main psychoactive compound within the cannabis plant, Δ9-tetrahydrocannabinol (THC), is thought to drive both the sensation of "high" and the cognitive impairments associated with cannabis consumption. Researchers keen to understand how cannabis impairs cognition have, therefore, studied the behavioral effects of systemic injections of THC in animal models. However, cannabis contains multiple other cannabinoids which may critically modulate the resulting cognitive effects. Users also typically eat or smoke cannabis, leading to concern over the translational validity of pure THC injections.

[CME ECG 67/Answers: Arrhythmia on Exertion].

CME ECG 67/Answers: Arrhythmia on Exertion Abstract. Ventricular tachycardias are potentially life-threatening cardiac arrhythmias with a heart rate >100 beats/min, originating from the specific conduction system below the His or the ventricular myocardium. The morphology of the surface ECG can provide valid information about the underlying mechanism and the associated cardiac disorder. The according pathomechanism is of paramount importance for further management. This article is intended to provide an insight into the various causes and treatment options as well as the differential diagnosis

Psuedo-Cor Triatriatum in an Elderly Patient With Dyspnea of Exertion: An Undescribed Condition Characterized by 3-Dimensional Transesophageal Echocardiography.

A 66-year-old gentleman with no prior cardiac history presented with dyspnea on exertion and chest pain for 1 month. His workup included a transthoracic echocardiogram, which demonstrated findings suggestive of cor triatriatum (C-TAT) with uncertain degree of hemodynamic obstruction. In addition, mild left ventricular systolic dysfunction and segmental wall motion abnormalities suggestive of coronary artery disease were noted. The patient then underwent transesophageal echocardiography (TEE) to define better the structural characteristics and hemodynamic significance of the C-TAT, left and rig

Muscle Conditions Affecting Sport Horses.

Optimal function of skeletal muscle is essential for successful athletic performance. Even minor derangements in locomotor muscle function will impact power output, coordination, stamina, and desire to work during exercise. In this review, the presenting clinical signs, differential diagnoses, approach to diagnostic testing and treatment of muscle atrophy and weakness, focal muscle strain, and exertional myopathies are discussed. Exertional myopathies include polysaccharide storage myopathies, recurrent exertional rhabdomyolysis, malignant hyperthermia, and myofibrillar myopathy.

Pushing, pulling and manoeuvring an industrial cart: a psychophysiological study.

One of the most frequent manual occupational tasks involves the pushing and pulling of a cart. Although several studies have associated health risks with pushing and pulling, the effects are not clear since occupational tasks have social, cognitive and physical components. The present work investigates a real case of a pushing and pulling occupational task from a manufacturing company. The study initially characterizes the case in accordance with Standard No. ISO 11228-2:2007 as low risk. An experiment with 14 individuals during three modalities of pushing and pulling was performed in order to

Colon perforation as a critical complication of exertional heat stroke.

Although reduced intestinal blood flow causing barrier dysfunction and endotoxemia is well documented in the pathogenesis of heat stroke (HS), complications of the gastrointestinal tract are less appreciated in HS patients. Herein, we report the case of a young man with exertional HS complicated with colon perforation. Acute abdomen, bloody diarrhea, dilated bowel loop, and unexplained shock should be considered as warning signs of colonic ischemia and consequent perforation. Early recognition is the key factor for improving the outcome of HS patients complicated with colon perforation.

Myoglobinuria caused by exertional rhabdomyolysis misdiagnosed as psychiatric illness.

BACKGROUND: Rhabdomyolysis is severe and acute skeletal muscle damage resulting in sarcolemma disruption. During injury, intracellular muscle contents are released into the plasma. The consequences may cause hypovolemia, electrolyte abnormalities, compartment syndrome, or even acute renal failure and dialysis. CASE REPORT: We present the history of a patient in whom exertional rhabdomyolysis was misdiagnosed. A 20-year-old male police recruit was admitted to a psychiatric hospital because of complaints about black urine and severe thigh pain. Serum creatinine kinase (CK) was significantly ele

Comparison of physiological and subjective strains of two protective coveralls in two short physically simulated demanding tasks.

A total of 15 physically active male students carried out a normalised task (T(N)) and a task of access by ladder and scaffolding (T(L)) in two impermeable protective coveralls and a reference sports wear to compare the physiological and subjective strains. Heart rate (HR) was recorded and sweat loss was checked. Subjective evaluations of comfort parameters, acceptable exposure durations and physical exhaustion were recorded at the end of the tasks. Results show that both protective clothing, compared to the sport wear, increase significantly HR, RPE and CR10 in both tasks. However the strains

Oxidative stress, antioxidant status, and the contracting diaphragm.

Reactive oxygen species, including free radicals, are produced through a number of biochemical reactions, often as a consequence of aerobic metabolism. A system of antioxidant enzymes and scavenger substrates provides protection of membrane lipids, proteins, and DNA. An imbalance between production of reactive oxygen species and antioxidant protection results in "oxidative stress." Oxidative stress is believed to contribute to numerous pathological conditions including atherosclerosis, obstructive lung disease, aging, and fatigue of skeletal muscles including the diaphragm. Strenuous exercise,
