Intestinal Histopathology due to an Acanthocephalan in Two Corvid Species from Northern Italy.

Carnivorous birds maintain parasites in the sylvatic cycle and have a role in their diffusion. The histopathology and ultrastructure of the intestine of 29 Hooded Crows (Corvus corone cornix) and 51 Eurasian Magpies (Pica pica), from the Province of Ferrara (Northern Italy), naturally infected with Sphaerirostris picae (Acanthocephala), were investigated. In both bird species, the prevalence of infection was around 10%, and the intensity of the infection in the Hooded Crows ranged from two to 12 acanthocephalans per host, whereas in the Eurasian Magpies it ranged from one to nine worms per bir…

Posterior inferior cerebellar artery aneurysms: operative strategies based on a surgical series of 27 patients.

AIM: Posterior inferior cerebellar artery (PICA) aneurysms are uncommon. The natural history and management of these aneurysms remains poorly understood. Surgical treatment of PICA aneurysms is challenging in view of their close neurovascular relationship. MATERIAL AND METHODS: Data relating to clinical presentation, radiological findings, operative approaches and outcomes were analyzed in 27 consecutive patients surgically treated for PICA aneurysms in a single hospital between January 1991 and December 2010. RESULTS: Headache was the most common presentation (100%) followed by gait ataxia …

3D Computer graphics simulation to obtain optimal surgical exposure during microvascular decompression of the glossopharyngeal nerve.

The affected artery in glossopharyngeal neuralgia (GPN) is most often the posterior inferior cerebellar artery (PICA) from the caudal side or the anterior inferior cerebellar artery (AICA) from the rostral side. This technical report describes two representative cases of GPN, one with PICA as the affected artery and the other with AICA, and demonstrates the optimal approach for each affected artery. We used 3D computer graphics (3D CG) simulation to consider the ideal transposition of the affected artery in any position and approach. Subsequently, we performed microvascular decompression (MVD)…

Revascularization of the Anterior Inferior Cerebellar Artery Using Extracranial and Intracranial Donors: A Morphometric Cadaveric Study.

INTRODUCTION: Anterior inferior cerebellar artery (AICA) aneurysms are rare, accounting for 0.2%-1.3% of all intracranial aneurysms. The standard treatment is often endovascular embolization or neck clipping; however, sacrifice of the parent vessel is sometimes necessary. Addition of revascularization procedures is a subject of controversy. The occipital artery (OA) has been used as a donor for bypass, but recently there has been a trend toward intracranial-intracranial approaches. The posterior inferior cerebellar artery (PICA)-AICA side-to-side bypass may serve as a safe alternative. OBJECT…

Ruptured Posterior Inferior Cerebellar Artery Aneurysms: Integrating Microsurgical Expertise, Endovascular Challenges, and AI-Driven Risk Assessment.

Background/Objectives: Posterior inferior cerebellar artery (PICA) aneurysms are one of the most difficult cerebrovascular lesions to treat and account for 0.5-3% of all intracranial aneurysms. They have deep anatomical locations, broad-neck configurations, high perforator density, and a close association with the brainstem, which creates considerable technical challenges for either microsurgical or endovascular treatment. Despite its acceptance as the standard of care for most posterior circulation aneurysms, PICA aneurysms are often associated with flow diversion using a coil or flow diversi…

Effect of the anastomosis between the posterior inferior cerebellar artery and the superior cerebellar artery on outcomes of acute basilar artery occlusion after endovascular treatment.

BACKGROUND: The effects of secondary collateral compensation on outcomes remain unclear in patients with acute basilar artery occlusion (BAO) after endovascular treatment (EVT). This study aimed to evaluate the benefits of the anastomosis between the posterior inferior cerebellar artery (PICA) and the superior cerebellar artery (SCA) in BAO after EVT. METHODS: This cohort study was conducted using data from the Endovascular Treatment for Acute Basilar Artery Occlusion Study Registry. Patients with acute BAO and treated with EVT were included. The primary outcome was a modified Rankin Scale sc…

C2-P2 Bypass: Technical Assessment of Petrous Carotid Artery to Posterior Cerebral Artery Interpositional Bypass Through the Combined Transcochlear-Subtemporal Approach as a Part of Microsurgical Treatment for Dolichoectatic Vertebrobasilar Artery Aneurysms.

BACKGROUND AND OBJECTIVES: Managing dolichoectatic vertebrobasilar artery aneurysms requires a multifaceted approach. Revascularization of the posterior circulation with a high-flow bypass is part of the flow reversal paradigm. Performing a robust high-flow bypass and addressing the aneurysm through the same approach smooths the operative intervention. This study assessed the anatomic feasibility of accessing the basilar trunk and aneurysm simultaneously to revascularize the posterior circulation using a petrous internal carotid artery (pICA)-posterior cerebral artery (PCA) interpositional byp…

Treatment of intracranial vertebral artery dissecting aneurysms involving the posterior inferior cerebellar artery origin.

BACKGROUND: Vertebral artery dissecting aneurysm (VADA) involving the origin of the posterior inferior cerebellar artery (PICA) is a complex disease entity in which the dual goals of preventing future rebleeding and maintaining perfusion of the lateral medulla must be considered. We present an illustrative case and review the literature surrounding treatment strategies. CASE DESCRIPTION: We report a patient presenting with extensive subarachnoid hemorrhage due to rupture of an intracranial VADA involving the PICA origin. After consideration of the patient's cerebral vasculature and robustness…

A ruptured distal posterior inferior cerebellar aneurysm. Our case and review of the literature.

We present a case of ruptured distal posterior inferior cerebellar artery (PICA) aneurysm, and review the literature and discuss the treatment strategy. A 77-year-old woman presented with the sudden onset of severe headache, nausea and vomiting. Computed tomography revealed an intraventricular hemorrhage, predominantly in the fourth ventricle and hydrocephalus with a thin subarachinoid hemorrhage (SAH). Angiography revealed an aneurysm arising at the turning point of the vessel, from the telovelotonsillar segment of the right PICA. On the 17th day after the onset, repeated angiography revealed…

Origin, course and distribution of the cerebellar arteries and of their branches are described. Anatomical drawings of the territory of thes … locculus and the neighbouring lobules of cerebellum. When the posterior inferior cerebellar artery (PICA) is hypoplastic, AICA takes over the territory usually supplied by the lateral branch of the PICA. The PICA always gives rami to the group of arteries supplying the dorsal medullary territory, but rarely participates to the supply of the lateral med …

[Contralateral suboccipital approach for clipping of an unruptured vertebral artery-posterior inferior cerebellar artery aneurysm].

Numerous approaches have been used to access aneurysms of the vertebral artery(VA)-posterior inferior cerebellar artery(PICA)complex for microsurgical clipping. Here, we report the case of a patient with an unruptured aneurysm of the left VA-PICA complex that was successfully treated using a contralateral suboccipital approach. Computed tomography angiography demonstrated a small saccular aneurysm arising from the lateral aspect of the left V4 segment just distal to the PICA origin. The aneurysm deviated to the right from the midline at the level of the jugular tubercle on angiographic evaluat…

Disrupted and migrated microcatheter in the vertebrobasilar artery system in endovascular embolization of cerebellar AVM: failure of endovascular and microneurosurgical retrieval.

Microcatheter disruption and migration is a rare etiology of complications that is directly attributable to preoperative embolization of arteriovenous malformations (AVMs). A 16-year-old male presented with cerebellar hemorrhage. Angiography showed an AVM in the right cerebellar hemisphere fed by the posterior inferior cerebellar artery (PICA) and superior cerebellar artery (SCA). During the procedures of preoperative embolization, a microcatheter was spontaneously disrupted before N-butyl-2-cyanoacrylate (NBCA) injection. The catheter migrated 13 cm into PICA, basilar artery (BA) and posterio…

Distal posterior inferior cerebellar artery saccular and giant aneurysms: report of two new cases and a comprehensive review of the surgically-treated cases.

Aneurysms of the distal posterior inferior cerebellar artery are rare comprising less than 0.5 to 3% of all aneurysms. The authors report two cases of distal PICA aneurysms: a case of a ruptured saccular aneurysm arising on the tonsillomedullary segment of the PICA and a case of a giant aneurysm arising on junctional site of lateral-medullary and tonsillo-medullary segments. In the first case subarachnoid and intraventricular hemorrhage was diagnosed by CT and four vessel-angiography visualized the aneurysm which was clipped successfully. In the second case MRI was suggestive for posterior fos…
