Sarcoma can present as locally advanced disease involving pleura for which extra-pleural pneumonectomy (EPP) may be the only surgical option to ensure adequate local control. Pulmonary arteriovenous malformations are infrequent lesions diagnosed rarely in the neonatal period. A case of arteriovenous malformation involving an entire lung in a newborn is described. Because of an untractable cyanosis with heart failure, an emergency left total pneumonectomy had to be done at 18 hours of life. Cyanosis disappear immediately, and the newborn recovered normal cardiac function.

A newborn girl underwent a right pneumonectomy for cystic adenomatoid malformation. She subsequently developed compression of the trachea and left mainstem bronchus by a combination of aortic arch, pulmonary artery, and ductus arteriosus. She died while planning aortic suspension, and the anatomic relationships were confirmed by autopsy. This problem is potentially treatable by a variety of means.
