Congenital and neonatal pneumonia.

The greatest risk of death from pneumonia in childhood is in the neonatal period. It is estimated that pneumonia contributes to between 750000-1.2 million neonatal deaths annually, accounting for 10% of global child mortality. Congenital and neonatal pneumonias are often a difficult disease to identify and treat, with clinical manifestations often being non-specific. Many of the normal lung defences are compromised in the fetus and neonate, leading to an increased susceptibility to infection. The aetiology and epidemiology of congenital and neonatal pneumonias will depend on the clinical setti…

Mefloquine-induced eosinophilic pneumonia.

Mefloquine has been widely used for prophylaxis and treatment of patients with chloroquine-resistant malaria; the drug is usually well toler … hylaxis for an 8-week trip to South Africa. A thorough work-up led to the diagnosis of eosinophilic pneumonia caused by the mefloquine. Her condition improved after the drug was discontinued. To our knowledge, this is the first report of mefloquine-induced eosinophilic pneumonia. Clinicians should be aware of this rare, potential adverse effect of mefloquin …

[Fever, pneumonia and eosinophilia].

We present the case of a 71-year old man with acute eosinophilic pneumonia. By ruling out other common etiologies and by a positive lymphocyte transformation test the non-steroidal anti-inflammatory drug mefenamic acid was identified as the possible causative agent or this potentially life threatening lung disease. The clinical presentation, diagnosis, and treatment of acute eosinophilic pneumonia as a pulmonary manifestation of a drug-induced allergic reaction are discussed.

[Idiopathic chronic eosinophilic pneumonia].

INTRODUCTION: Idiopathic chronic eosinophilic pneumonia (ICEP) is one of the hypereosinophilic lung diseases. BACKGROUND: ICEP is a rare disease of unknown cause that combines non-specific respiratory and general symptoms with predominantly peripheral radiological infiltrates. The presence of blood and alveolar eosinophilia points strongly to the diagnosis. ICEP is very sensitive to systemic corticosteroids but relapses are common following stopping treatment or reducing the dose. A background of asthma is commonly found and many patients with ICEP develop severe asthma which, together with t…

Idiopathic endogenous lipoid pneumonia.

Lipoid pneumonia is a rare pulmonary disorder having no classical radiological appearance. We report a 33-year-old male, ex-smoker who was referred to us with history of cough, mild mucoid expectoration and progressively increasing dyspnoea since one year. He was investigated at local hospital and was treated with 30 mg prednisolone per day for 6 months for sarcoidosis without any response. On examination, he was normal except for fine basal crepitations in chest. Pulmonary function test (PFT) revealed mild airway obstruction. High resolution computerised tomographic scan (HRCT scan) revealed …

Pneumocystis: not just pneumonia.

Once known exclusively as the agents of severe pneumonia in immunocompromised individuals, Pneumocystis spp. are now being associated with asymptomatic carriage in hosts that do not have profound immune debilitation. In the absence of a cultivation system, polymerase chain reaction and histological studies have identified Pneumocystis in neonatal populations, in pregnant women and in other patients that have chronic underlying disease processes. These findings in humans and in experimental animal models indicate the presence of potential reservoirs of infection, and provide insights into the t…

[Pneumocystis pneumonia in infants].

The goal of this study is to present the clinical and evolutive features of Pneumocystis infection (PCP) in infants admitted in our clinic. We summarise these aspects from 17 cases (10 male and 7 female infants), admitted between 1st January 2004 and 31st May 2005. PCP infection is rare. It represents 1,5/1000 children (17 cases of 11328 total patients) admitted in our hospital. The risk factors for PCP were age between 6 weeks and 6 months (average 3,38 months) low birth weight (average = 2428 grams), low weight for age, prolonged hospital admission (88,23% of the 17 infants were abandoned in

Clarithromycin-induced eosinophilic pneumonia.

Clarithromycin (CAM) has been widely used for the treatment of respiratory infection. Macrolides are generally well tolerated and their adve … ed with the withdrawal of CAM. Histopathologic examinations confirmed the diagnosis of eosinophilic pneumonia. This is the first report of CAM-induced eosinophilic pneumonia.

Severe community-acquired pneumonia.

Although several pneumonia severity criteria have been firmly established, the exact definition of severe community-acquired pneumonia (CAP) remains elusive. Mortality from CAP remains high, reaching 50% in some series. The particular role of and spp. in severe CAP has been defined more clearly. Microbial diagnosis in the individual patient remains a difficult task. Despite promising new diagnostic tools, concerns about possible mixed origins preclude a change from the currently advocated broad-spectrum approach of antimicrobial treatment. Although there is some evidence that guidelines may op…

Immunologic responses to pneumonia.

Pneumonia in the critically ill surgical patient often results from the bombardment of a previously normal pulmonary system with therapeutic foreign bodies, hospital pathogens, and impairment of the host defenses. Despite its long history as a significant clinical problem, a woefully inadequate amount of study has been directed toward therapy. We created an experimental model of a differential pulmonary infection using a strain of Klebsiella pneumoniae. We then compared the progressively affected pneumonic process versus the normal parenchyma. We measured neutrophil and monocyte complement ant…

[Idiopathic chronic eosinophilic pneumonia].

Idiopathic chronic eosinophilic pneumonia is a rare disease first described by Carrington 30 years ago. The cause is unknown. As illustrated by the case described in this report, most cases occur in asthmatic patients in the fifth decade of life. Cardinal features are respiratory symptoms, altered general status, laboratory evidence of inflammation, blood eosinophilia in most cases, and x-ray images showing the presence of infiltrates in both lungs. Diagnosis can be confirmed by detection of eosinophils in broncho-alveolar lavage fluid. Extrapulmonary involvement is uncommon and is suggestive …

Lipoid pneumonia: CT findings.

OBJECTIVE: To assess the CT findings of lipoid pneumonia. METHODS: Chest radiography and CT performed in six patients with proven lipoid pneumonia were reviewed by two observers. Diagnosis was confirmed by biopsy (five cases) or bronchoalveolar lavage (one case). The clinical history of taking oily substance could be obtained retrospectively in all patients. RESULTS: Chest radiography showed bilateral air space consolidation in three cases, irregular mass-like lesions in two, and a reticulonodular pattern in one case. Computed tomography demonstrated diffuse parenchymal consolidation in thre…
