PURPOSE: Popliteal artery entrapment and adventitial cystic disease are the main causes of claudication in young patients. Adventitial cystic disease is a rare vascular pathology mostly affecting the popliteal artery but other localisations have been reported. Diagnosis and therapeutic management of adventitial cystic disease and particularly percutaneous aspiration are presented.
MATERIALS AND METHODS: The authors report six cases (four men and two women; mean age: 55) of CAD of the popliteal artery (n = 4) and unusual cases in the femoral artery (n = 1) and in the femoral vein (n = 1) explo

A case of cystic adventitial degeneration (CAD) of the popliteal artery is presented. Histologically the cyst appeared localised in the adventitia, outside the elastic lamina. A lectin-histochemical study evaluated the following peroxidase conjugated lectins: Peanut agglutinin, Concanavalin A, Ulex Europaeus (UEA I) and Wheat Germ Agglutinin. The lack of UEA I reactivity excluded an arterial origin of the cyst. Moreover, the lectin binding pattern of CAD appeared to be similar to the reactivity pattern of normal synovia.

Cystic adventitial disease is very rarely described in the common femoral artery, the usual location being the popliteal artery. CT in this patient shows the extension of an enlarged bursa of the psoas muscle reaching the cyst in the arterial wall.

The popliteal fossa is located at an important place of the osteo-articular system near the knee of course but also in strong relationship with the neighboured vascular, muscular and nervous structures. The symptomatology of the Baker's cyst is well known but its physiopathology is less obvious. It is important to remain aware of some unusual conditions that must prompt a differential diagnosis reaching from functional articular conditions over vascular and nervous problems to major tumoral problems. The patient's history and a rigorous clinical examination must remain the main stone of the me

Cystic adventitial disease is a rare form of non-atherosclerotic stenosis and one of the main causes of claudication in young and middle-aged men. Approximately 200 case reports are available in the literature to date. It is generally located in the popliteal artery, although it may be found in other arteries and even in veins. The aetiology is still unclear: most authors believe that the cyst may originate from a synovial ganglion close to the adjacent hip joint capsule. Patients affected by adventitial cystic degeneration are often young male non-smokers with intermittent calf claudication.

Two patients were admitted to our department because of recent aggravation of claudication in the leg, which was exacerbated by walking. They were diagnosed as having a Baker cyst or acute thrombosis in the popliteal fossa at another hospital. There was no evidence of ischemia, and the ankle brachial index was normal. Computed tomography and magnetic resonance imaging were performed, revealing a cystic mass of the popliteal artery (PA). Intraoperatively, the cystic lesion was found within the adventitia of the PA; based on the biopsy findings, both patients were diagnosed as having adventitial

Angiosarcoma is a rare neoplasm comprising 1 to 2% of soft tissue sarcoma. This tumor has been associated with previous irradiation, exposure to toxins and the presence of foreign bodies. A case report of an epithelioid angiosarcoma that developed at the site of an arterial femoro-popliteal bypass using autologous vein is described. The initial presentation looked like a painful popliteal cyst. Chronic fibrosis secondary to the thrombosis could play a role in the tumorogenesis of this uncommon tumor. This case illustrates that sarcoma may be a late complication of vascular bypass and may have …

Cystic adventitial disease of the popliteal artery is an unusual condition of uncertain etiology in which a mucin-containing cyst forms in the wall of the popliteal artery and causes symptoms of intermittent claudication. The patient is typically a young non-smoking male, whose arteries otherwise are normal. Arteriography may show a smooth-walled narrowing or a non-specific complete occlusion, but may also be normal. Non-invasive imaging techniques may be helpful. We report two cases of cystic adventitial disease. Both were successfully operated on with resection of the diseased arterial segme

Masses about the knee are most commonly benign cysts. The diagnosis can often be made with a history and physical exam, but radiographs and MRI are sometimes required, and histologic evaluation is occasionally necessary. Popliteal (Baker's) cysts are often indicative of arthritis; treatment for the underlying pathology may reduce swelling and permit resorption. Meniscal cysts indicate an underlying tear; treatment is resection and cyst excision. Symptomatic ganglion cysts generally require surgical excision. Treatment for bursitis is conservative. The less-common synovial chondromatosis, pigme

Popliteal cysts or Baker's cysts in childhood is a rare disease usually found by the parents. The purpose of this study is to study the recurrence rate after primary surgical resection and conservative treatment of the cysts. We retrospectively found 20 children with Baker's cysts treated at Ribe Amts Hospitals during the period 1992-1998. Thirteen patients were treated conservatively and seven had a surgical resection of the cysts performed. Of the 13 conservatively treated patients the cysts was still present in six patients (46%). In seven patients the cysts had disappeared within nine mont…

INTRODUCTION AND IMPORTANCE: Cystic adventitial disease (CAD) is a rare vascular disorder marked by occlusion stemming from the development of a cystic mass within the outer (subadventitial) layer, with a predominant impact on the popliteal artery. The significance of the case presented herein lies in shedding light on a distinct clinical manifestation involving a 40-year-old man who exhibited sporadic calf claudication during ambulation. This instance contributes to the broader understanding of CAD and its diverse clinical presentations, emphasizing the need for further exploration and awaren

Synovial cysts are fluid-filled masses lined with synovium and located within or about joints. The main symptoms are pain and/or neurological deficits. They can be intraneural or extraneural or develop between or within muscles. Synovial cysts that arise at a distance from a joint raise diagnostic challenges. We report three cases of synovial cysts of the proximal tibiofibular joint, including an intramuscular cyst responsible for paralysis of the anterolateral leg muscles. Tibiofibular synovial cysts are less common than popliteal cysts, and their pathophysiology is poorly understood. Pressur…

Cystic adventitial disease (CAD), a rare arterial disorder, can cause localized arterial stenosis or obstruction. A 55-year-old man presented with a 2-month history of left lower leg pain and paleness when bending the left knee. The patient was diagnosed with CAD of the left popliteal artery based on imaging examinations. Surgery was performed with the patient placed in the prone position using an S-shaped skin incision, and the left popliteal artery was exposed. A simple incision of the cyst wall was made. There was no sign of recurrence at 1 year postoperatively.

An 87-year-old male was admitted with intermittent claudication of the left calf. We performed lower extremity angiography, which revealed stenosis of the left popliteal artery. Intravascular ultrasound (IVUS) image correctly identified the cystic appearance of visualized extravascular hypodensity, causing extrinsic compression of the lumen. We diagnosed the condition as cystic adventitial degeneration (CAD) of the popliteal artery. We operated a resection of a cyst with the artery and replaced the autovein graft (saphenous vein). After surgery, the patient was free of symptoms. CAD is a rare

Two hundred and thirty-four cases of cystic adventitial disease were collected from over two hundred world literatures and were classified into two groups: first, typical cystic adventitial disease of the popliteal artery, and second, that of the stem vessels in extremities other than the popliteal artery. The nomenclatures, clinical and pathological features, and etiology were reviewed. From the etiological point of view, the group in which the cyst was connected with joint capsule was identified as the joint group. All cases reported are listed in the two tables except those cases listed alr

Calf pain is a common complaint in the primary care setting. An understanding of the relationship of calf pain to the popliteal cyst and pseudothrombophlebitis syndrome and a systematic approach to evaluation may avoid a mistaken diagnosis of thrombophlebitis and unnecessary anticoagulation therapy.

This strange lesion has been described as colloid degeneration, adventitial cyst, adventitial cystic disease, cystic degeneration of the popliteal artery. The disease was also observed in other localizations and the first case, which was described in 1946 by Atkins and Key (I) concerned an iliac artery. Bizard (2) in 1978 reported a case of the common femoral artery. Ejrup and Hiertonn (4) described the first popliteal localization in 1954. Bergan in 1970 reported 40 cases and collected 115 operated cases in Rutherford's Vascular Surgery (5). Usually a young patient complaints from a recent se
