Percentage of free serum prostate-specific antigen: a new tool in the early diagnosis of prostatic cancer.

Prostate-specific antigen (PSA) is a protease able to bind to serum antiproteases as alpha 1 antichymotrypsin (ACT). Free PSA (FPSA) corresponds to the fraction of total PSA (TPSA) which is unbound to ACT. Specific detection of the FPSA seems to be a valuable tool in the distinction between prostatic cancer (PCa) and benign prostatic hyperplasia (BPH). Our aim was to evaluate retrospectively the FPSA/TPSA ratio in comparison to TPSA or FPSA determination, using two new immunoradiometric assays (PSA-RIACT and FPSA-RIACT, CIS bio international, Gif Sur Yvette, France) in the early diagnosis of P…

Postmortem Serum Prostate-Specific Antigen as a Potential Marker for Prostatic Disease: A Forensic Exploratory Study.

INTRODUCTION: Prostate-specific antigen (PSA) is widely used clinically to diagnose and monitor prostate cancer (PCa) and benign prostatic hyperplasia (BPH). Although PSA has long been used in forensic science for semen identification, its utility as a postmortem serum biomarker has not been systematically evaluated in a forensic autopsy context. This study explored whether postmortem serum PSA levels may reflect underlying prostatic pathology in forensic autopsy practice. METHODS: A total of 101 male autopsy cases (PCa, n = 3; BPH, n = 16; non-prostatic malignancies, n = 36; controls, n = 46…

Prostate specific antigen enhances the innate defence of prostatic epithelium against Escherichia coli infection.

BACKGROUND: This study investigated whether the increase in serum prostate specific antigen (PSA) typically seen during male urinary tract infection (UTI) is incidental or reflects an innate defence mechanism of the prostate. The protective roles of the whey-acid-motif-4-disulphide core (WFDC) proteins, secretory leukoproteinase inhibitor (SLPI) and WFDC2, in the prostate were also examined. METHODS: UTI recurrence was assessed retrospectively in men following initial UTI by patient interview. PSA, SLPI, and WFDC2 gene expression were assessed using biopsy samples. An overview of the health effects of isoflavones with an emphasis on prostate cancer risk and prostate-specific antigen levels.

Soybean isoflavones possess hormonal and nonhormonal properties that may reduce the risk of coronary heart disease, osteoporosis, and certain cancers, and alleviate hot flashes in menopausal women. Among the various cancers whose risk may be reduced by isoflavones, there is particular interest in prostate cancer. Eleven trials have examined the effects of isoflavones on serum prostate-specific antigen (PSA) levels. The dose of isoflavones in these trials from supplements or soy protein ranged from 60 to 900 mg/day (typical Japanese intake is 30-50 mg/day), subject number/group ranged from 8 to…

Total and free serum prostate specific antigen levels during the first month of acute prostatitis.

OBJECTIVES: The level of total prostate specific antigen (PSA) rises in acute prostatitis. The PSA fraction responsible for this increase and the evolution of the free/total PSA ratio are little known at the present time. We therefore carried out a prospective study of the evolution of total and free PSA levels and the free-to-total PSA ratio for one month after diagnosis of acute prostatitis. PATIENTS AND METHODS: Between January 1999 and February 2001, 31 consecutive patients (mean age 51 years) were treated for acute prostatitis. No patient had a history of prostate cancer or of recent pro

From prostate-specific antigen to precision: The future of prostate cancer diagnosis with artificial intelligence, biomarkers, and imaging.

BACKGROUND: Prostate cancer (PCa) diagnosis has historically relied on prostate-specific antigen (PSA) testing. Although PSA screening significantly reduces mortality rates, it is limited by its low specificity and the risk of overdiagnosis and overtreatment. These limitations highlight the need for more accurate diagnostic approaches that can be combined with PSA testing. Emerging technologies, such as artificial intelligence (AI), novel biomarkers, and advanced imaging techniques, offer promising avenues to enhance the accuracy and efficiency of PCa diagnosis and risk stratification. PET/CT imaging of prostate cancer in the era of small molecule prostate specific membrane antigen targeted tracers.

Staging and restaging of prostate cancer is crucial for treatment planning and prognosis. Accurate localization is of high relevance for a tailor-made therapy and an early detection of unknown metastatic spread can lead to a survival benefit. Evidence based guidelines that are currently in use were established using data from conventional imaging (such as magnetic resonance imaging (MRI), computed tomography (CT) and bone scintigraphy). Prostate-specific membrane antigen (PSMA) positron emission tomography (PET) is rapidly evolving with promising results. However, up to now there is little con

Rise in prostate-specific antigen in men with untreated low-grade prostate cancer is slower during spring-summer.

To test the hypothesis that the rate of rise in prostate-specific antigen (PSA) is slower during the spring-summer than during the rest of the year, we used PSA data from a prospective single-arm cohort study of men who had been followed to characterize a watchful observation protocol with selective delayed intervention for clinically localized, low-to-intermediate grade prostate adenocarcinoma. The rate of PSA increase was calculated as the visit-to-visit slope of log (PSA) against time, from 1 calendar-quarter visit to the next. The nonparametric Friedman test confirmed differences in rate o…
