Hungary health system review.

Hungary has achieved a successful transition from an overly centralized, integrated Semashko-style health care system to a purchaser provider split model with output-based payment methods. Although there have been substantial increases in life expectancy in recent years among both men and women, many health outcomes remain poor, placing Hungary among the countries with the worst health status and highest rate of avoidable mortality in the EU (life expectancy at birth trailed the EU27 average by 5.1 years in 2009). Lifestyle factors especially the traditionally unhealthy Hungarian diet, alcohol

Association between physical activity and vitamin D: A narrative literature review.

This narrative review of the medical literature assessed whether outdoor and indoor physical activity would increase the plasma levels of vitamin D. Synthesis of this liposoluble vitamin is mainly mediated by sunlight on the skin, where it is activated to perform its main action, which is to control the serum levels of calcium as soon as the element is absorbed in the intestines, assisting in the regulation of bone metabolism. Physical activity is any body movement that results in energy expenditure, while outdoor physical activity refers to physical activity carried out at public parks or oth

Ultrasound at scintigraphic "intermediate probability of pulmonary embolism".

For diagnosis of pulmonary embolism (PE), ventilation/perfusion lung scintigraphy is routinely used; approximately one-third of the patients will have the diagnosis "intermediate probability of PE" (inconclusive). In this group only about 33% are found to have pulmonary emboli if examined with pulmonary angiography. To evaluate the diagnostic, therapeutic, and economic consequences of ultrasound of the legs as a complementary diagnostic investigation to "intermediate probability", 72 consecutive patients were investigated with bilateral ultrasound of the proximal deep veins of the legs and pul

Alcohol use disorder is a chronic disease.

Alcohol use disorder (AUD) is a chronic, relapsing brain disease with profound health, societal, and economic consequences. Alcohol misuse not only leads to AUD, but it is also a driver of multimorbidity, exacerbating a wide range of chronic comorbidities, including cancer. Despite being formally recognized over six decades ago as a medical condition, AUD remains one of the most prevalent and costly public health issues in the United States. Alcohol misuse contributes to more than 90,000 deaths annually in the United States, with hundreds of billions of dollars lost annually due to healthcare

The spatial effects and influencing factors of inter-provincial health resource allocation efficiency in China.

BACKGROUND: Chinese government has been adjusting its strategies in response to rapid domestic and international changes to devise effective health policies and enhance public health. However, with the rapid socio-economic development and the COVID-19 outbreak, many researchers have identified issues of inefficiency and uneven distribution in health resource allocation within China. Therefore, how to scientifically allocate and efficiently use Chinese health resources has become an urgent issue. METHODS: The super-efficiency SBM model and global Malmquist model were used to measure and dynami

Performance of primary health care in São Paulo state, Brazil, during the period 2010-2019.

This article presents the results of an analysis of the performance of primary health care in São Paulo state over the last decade against a backdrop of financial crisis and health funding cuts. We conducted a time series analysis (2010-2019) of performance indicators across the following dimensions based on an adapted conceptual framework: health service performance, health system, and determinants of health. Annual percentage change was calculated for each indicator using a log-linear model. Performance across the indicators was generally positive; however, there was a decline in performance

Evaluation and Management of Patients With Stable Angina: Beyond the Ischemia Paradigm: JACC State-of-the-Art Review.

Coronary heart disease is a chronic, systemic disease with a wide range of associated symptoms, clinical outcomes, and health care expenditure. Adverse events from coronary heart disease can be mitigated or avoided with lifestyle and risk factor modifications, and medical therapy. These measures are effective in slowing the progression of atherosclerotic disease and in reducing the risk of thrombosis in the setting of plaque disruptions. With increasing effectiveness of prevention and medical therapy, the role of coronary artery revascularization has decreased and is largely confined to subgro

Nontrivial resource requirement in the early stage for containment of epidemics.

During epidemic control, containment of the disease is usually achieved through increasing a devoted resource to reduce the infectiousness. However, the impact of this resource expenditure has not been studied quantitatively. For disease spread, the recovery rate can be positively correlated with the average amount of resource devoted to infected individuals. By incorporating this relation we build a novel model and find that insufficient resource leads to an abrupt increase in the infected population size, which is in marked contrast with the continuous phase transitions believed previously.

Patterns of psychotropic drug dispensation in Portugal amidst the COVID-19 pandemic and beyond.

The increasing prevalence of mental health disorders has been matched with growing psychotropic drug consumption rates around the world. Assessing psychopharmacoepidemiological trends and their determinants is essential to guide medical care delivery and public health policies. However, nation-wide studies on up-to-date consumption patterns are scarce and generally disregard relevant pharmacological, medical, socio-demographic, and economic covariates. Previous studies on the Portuguese case, known for critically high consumption rates of benzodiazepines and antidepressants, are limited to the

Integrated analysis of a compendium of RNA-Seq datasets for splicing factors.

A vast amount of public RNA-sequencing datasets have been generated and used widely to study transcriptome mechanisms. These data offer precious opportunity for advancing biological research in transcriptome studies such as alternative splicing. We report the first large-scale integrated analysis of RNA-Seq data of splicing factors for systematically identifying key factors in diseases and biological processes. We analyzed 1,321 RNA-Seq libraries of various mouse tissues and cell lines, comprising more than 6.6 TB sequences from 75 independent studies that experimentally manipulated 56 splicin

The costs and financing needs of delivering Kenya's primary health care service package.

INTRODUCTION: For many Kenyans, high-quality primary health care (PHC) services remain unavailable, inaccessible, or unaffordable. To address these challenges, the Government of Kenya has committed to strengthening the country's PHC system by introducing a comprehensive package of PHC services and promoting the efficient use of existing resources through its primary care network approach. Our study estimated the costs of delivering PHC services in public sector facilities in seven sub-counties, comparing actual costs to normative costs of delivering Kenya's PHC package and determining the corr

The costs of monitoring trachoma elimination: Impact, surveillance, and trachomatous trichiasis (TT)-only surveys.

BACKGROUND: Although trachoma causes more cases of preventable blindness than any other infectious disease, a combination of strategies is reducing its global prevalence. As a district moves toward eliminating trachoma as a public health problem, national programs conduct trachoma impact surveys (TIS) to assess whether to stop preventative interventions and trachoma surveillance surveys (TSS) to determine whether the prevalence of active trachoma has rebounded after interventions have halted. In some contexts, programs also conduct trachomatous trichiasis (TT)-only surveys. A few costing studi
