We describe a case of fatal gas gangrene of the uterus after an apparently normal term pregnancy in a 22-year-old woman. The patient died less than 15 h after the onset of symptoms, which consisted of a single episode of vomiting and increasing hypogastric pain. Hypotension, tachycardia and renal failure were the dominant clinical features. The pathogenesis, diagnosis and treatment of uterine gas gangrene are discussed.

A report is given in a case of appendicitis after delivery in childbed. This was the third case of appendicitis within 17 657 deliveries in the gestation of the district hospital in Sangerhausen in the period from 1960 to 1975. The chronic relapsed appendicitis produced a paralytic Ileus. By using correspondent measures (infusional therapy, appendectomy, adhesion solutions) the patient could be cured without any complications. Postpartum superficial and deep venous thrombosis were found in 22.3% and 5.3% of women with varicose veins who delivered vaginally, or by cesarean section. The preventive use of low doses of heparin (5,000 U every 12 h injected subcutaneously) in 116 parturients with varicose veins reduced significantly the incidence of postpartum venous thrombosis. None of the 26 women with previous thrombosis who received preventive treatment, developed such an event.

Acute puerperal inversion of the uterus is an uncommon but potentially fatal obstetric complication. In the 15 year period ending December 31, 1974, 11 inversions have been managed at the University of Virginia Hospital. All were recognized immediately and manually replaced and there was no significant postpartum morbidity. Successful management appears to depend largely upon prompt recognition, which can be achieved by routine postpartum examination of the vagina and cervix and manual exploration of the uterus.

In addition to bipolar cycloid episodes, infective delirium and eclamptic psychosis, each of which has characteristic clinical features and course, brief episodes of delirium and stupor are also seen in the immediate aftermath of parturition. Several mothers have had similar episodes developing later in the first 10 days, and some have cyclical disorders with an unusual time base. Bipolar/cycloid disorders can start on day 1 or even earlier.
