Pylorus-preserving pancreaticoduodenectomy for advanced duodenal disease in familial adenomatous polyposis.

BACKGROUND: Although only 5 per cent of patients with familial adenomatous polyposis (FAP) die from duodenal cancer, a recent study indicated that the mortality rate is much higher in patients with Spigelman stage IV disease. This has prompted an increased rate of referral for excisional surgery and an analysis of the results. METHODS: Between January 1994 and June 2002, 16 patients with FAP (mean age 55 years; eight men) were referred to a single surgeon for pylorus-preserving pancreaticoduodenal resection for Spigelman stage IV duodenal adenomatosis. RESULTS: One patient died from multiple…

[Isolated complete duodenal avulsion at the pylorus after blunt abdominal trauma].

We would like to place on record a case of an isolated complete duodenal avulsion distal to the pylorus after blunt abdominal trauma, in a 78-year-old patient. From all relevant literature, this is the first case known to us of duodenal rupture, around its complete circumference, in close proximity to the pylorus, which did not result in the injury of any further organs, in the absence of chronically infectious alterations. A duodenal injury in the case of blunt abdominal trauma is rare and amongst injuries to hollow organs is of no great importance. In isolated cases of duodenal injuries, it …

Long-term postoperative functional evaluation of pylorus preservation in Imanaga pancreatoduodenectomy.

Our purpose was to determine whether pylorus-preservation in the Imanaga (PpPDI) method minimizes postoperative impairment of gastrointestinal function. Nine patients who had undergone PpPDI (postoperative years: 5.7 +/- 2.6) and nine patients who had undergone conventional Imanaga pancreatoduodenectomy (PDI) (postoperative years: 6.8 +/- 2.0) were evaluated for symptoms, nutritional parameters, and physiologic function of the biliary tract and residual stomach using gastric emptying and hepatobiliary scintigraphy. The body weight recovered to 99.3% +/- 3.8% of pre-illness body weight in PpPDI…

Long-term survival in pancreatic cancer: pylorus-preserving versus Whipple pancreatoduodenectomy.

BACKGROUND: This study compared long-term survival in pancreatic or periampullary cancer treated with Whipple pancreatoduodenectomy (PD) and pylorus-preserving pancreatoduodenectomy (PPPD). METHODS: Two hundred twenty-one patients with pancreatic head or periampullary cancer were treated. Prognostic variables included age, gender, type and period of operation, and tumor stage. In the ductal adenocarcinomas variables also included tumor and node status, type of lymphadenectomy, pathologic grade, and presence of microscopic residual tumor. The end point was death as a result of neoplastic recur…

Pancreatic enzyme activity after a pylorus-preserving pancreaticoduodenectomy reconstructed with pancreaticogastrostomy.

This study was initiated to clarify whether the main hydrolytic enzymes of the pancreas are activated or inactivated when secreted into the stomach of patients who had undergone a pylorus-preserving pancreaticoduodenectomy (PPPD) and were given a pancreaticogastrostomy (PG) for the reconstruction. Seventeen such patients, 15 cancer patients and two pancreatic patients, who underwent PPPD-PG reconstruction were postoperatively followed up for 3 or more years to investigate the influence of the gastric acid on the p-type amylase and lipase activity. Results revealed that when the pH was < 3.0, b…

Gastric carcinoma presenting as double-channel pylorus on upper gastrointestinal series.

A double-channel pylorus or gastroduodenal fistula is a rare entity frequently acquired in association with penetrating peptic ulcer disease. The diagnosis is made by upper gastrointestinal series or by endoscopy. We report a 59-year-old man who presented with gastric outlet obstruction whose upper gastrointestinal series was typical of benign peptic ulcer disease demonstrating antral ulcers and a double-channel pylorus. At operation, gastric carcinoma infiltrating the pyloric channel and dividing it into two lumens was found. No gastroduodenal fistula was found. The "gastroduodenal fistula" o…

Pyloric-sphincter studies in peptic-ulcer patients: pylorus in peptic ulcer.

Pyloric pressure was assayed by a manometric procedure in the basal state and after intraduodenal infusion of HCl. 13 control subjects, 11 patients with benign gastric ulcer, 8 with duodenal ulcer, and 2 with coexistent gastric and duodenal ulcers were studied. Mean resting pyloric pressure in gastric-ulcer patients (5.17 +/- 0.71) mm Hg) was significantly lower than controls (9.40 +/- 0.85 mm Hg) and did not increase after HCl perfusion into the duodenum. Mean basal pyloric pressure in duodenal-ulcer patients (11.30 +/- 1.57 mm Hg) did not differ significantly from controls and increased afte

Pylorus-Preserving Gastrectomy for Gastric Cancer: Current Evidence and Future Directions.

Pylorus-preserving gastrectomy (PPG) is a function-preserving surgery for clinically T1N0M0 (cT1N0M0) early gastric cancer (EGC) located in the middle third of the stomach, at least 4 cm proximal from the pylorus. By preserving the peri-pyloric tissues, including the hepatic branch of the vagus nerve and the infrapyloric vessels, PPG maintains physiological pyloric function without compromising oncologic safety. Several randomized controlled trials (RCTs) have demonstrated the functional advantages of PPG. In particular, the Korean multicenter RCT (KLASS-04) reported a lower incidence of galls…

Pylorus preservation after total pancreatectomy: a systematic review and meta-analysis.

BACKGROUND: Total pancreatectomy (TP) is a major surgical procedure often associated with significant morbidity. Pylorus preservation (PP) during TP aims to minimize postoperative complications, particularly delayed gastric emptying (DGE), while maintaining gastrointestinal function. However, their impact remains controversial. This study systematically reviewed and analyzed the outcomes of TP with and without pylorus preservation. METHODS: A systematic review and meta-analysis were conducted following the PRISMA and AMSTAR 2 guidelines. Randomized controlled trials and controlled clinical tr…

EndoFLIP evaluation of the pylorus during minimal invasive Ivor-Levis esophagectomy.

BACKGROUND/AIMS: During esophagectomy for malignancy, the anterior and posterior branches of the vagus nerve are transected in order to achieve surgical radicality. This leads to loss of central nervous system-control of the pylorus which may lead to delayed gastric emptying. We aimed to investigate the feasibility of the EndoFLIP technique for assessment of pyloric biomechanical properties in patients undergoing esophagectomy. METHODS: A feasibility study in six patients undergoing surgery was conducted. EndoFLIP measurements were carried out preoperative (Pre-op), after surgical resection (…

Long-Term Outcomes of Pylorus-Preserving Gastrectomy for Early Gastric Cancer.

BACKGROUND: In the short term, pylorus-preserving gastrectomy (PPG) has been reported to have advantages over distal gastrectomy (DG) with regard to postprandial symptoms and dumping syndrome. We aimed to evaluate the quality of life after PPG for early gastric cancer in the long term in comparison to that after DG. METHODS: Twenty-six patients who underwent gastrectomy (11 PPG, 15 DG) for early gastric cancer at Osaka University Hospital participated and were followed for more than 4 years. Body weight changes, nutritional status, dual-phase scintigraphy findings, endoscopic survey results, …

Effect of pyloric dilatation on gastric emptying after pylorus-preserving pancreaticoduodenectomy.

BACKGROUND/AIMS: Pylorus-preserving pancreaticoduodenectomy (PPPD) is the standard treatment for periampullary and pancreatic head tumors. Delayed gastric emptying (DGE) is the most common (ranging from 15-45%) but not life threatening complication and impairs patient recovery and prolongs the hospital stay after PPPD. The precise pathomechanism of DGE is still unclear. The aim of this study was to evaluate whether the method of pyloric dilatation performed at the time of PPPD could improve gastric emptying. METHODOLOGY: Forty patients underwent PPPD for pancreatic or periampullary lesions fr…
