Prolonged fever and pyuria: an uncommon manifestation of Q fever.

A patient with Q fever is described who had been ill for a year before the diagnosis was made on the basis of serological data. In addition it was possible to isolate Coxiella burnetii, the causative agent by culture from the urine. This is very exceptional and is to our knowledge only the second case in which this has been achieved. The patient made a full recovery after lengthy treatment with tetracycline. Q fever should be considered in patients with pyrexia of unknown origin, particularly in travellers.

[Q fever as a cause of acute hepatitis accompanying fever].

Q fever is a zoonotic infection caused by Coxiella burnetti, which has been previously regarded as an uncommon infectious disease in Korea but is sporadically reported recently. Common manifestations of acute Q fever usually present as influenza-like illness, pneumonia and occasionally hepatitis. Herein, we report 4 cases of acute Q fever as a cause of acute hepatitis and fever. All patients had fever and non-specific symptoms, and laboratory test showed acute hepatitis. Antibody surveys for many virus infections and bacterial cultures were negative. Finally, they were diagnosed acute Q fever …

Emerging Q Fever Infection in Türkiye.

OBJECTIVES: Q fever, caused by Coxiella burnetii, is an underrecognized zoonotic infection with diverse clinical manifestations. The nonspecific nature and slow progression of Q fever frequently delay diagnosis, sometimes leading to severe or even fatal outcomes. This study describes 32 patients diagnosed with Q fever over an 18-month period in a previously unreported region, highlighting the emerging awareness of this neglected zoonosis. METHODS: Patients with seropositive C. burnetii Phase I/II IgM and/or IgG antibodies between October 2023 and March 2025 (18-month period) were included. Ca…

Q fever transmission mediated by ticks.

We report a patient diagnosed with Q fever who developed an acute hepatitis after tick exposure. Sequence analysis of PCR fragments from the patient´s blood and the tick from the patient (Hyalomma lusitanicum) showed homologous (100% identity) Coxiella burnetii sequences, suggesting direct Q fever transmission by a H. lusitanicum tick.

Q Fever Vertebral Osteomyelitis Complicating Vertebroplasty.

Query (Q) fever is a zoonotic bacterial infection caused by Coxiella burnetii. In a minority of patients, chronic disease can occur after acute infection. Endocarditis and infections of aneurysms or vascular prostheses are the most common forms of chronic Q fever in adults. We report a case of an elderly female patient with chronic Q fever vertebral osteomyelitis at the site of her previous cement vertebroplasty, complicated by paravertebral abscess. Patient treatment required prolonged drainage in addition to the long duration of antibiotic treatment by doxycycline and hydroxychloroquine. Ost…

[Q fever, a zoonosis often overlooked].

Q fever is a zoonosis caused by an intracellular Gram-negative bacteria, Coxiella burnetii. Animals are the main reservoir and transmission to men generally is occurring by inhalation of contaminated aerosols. Acute Q fever generally is benign and usually resolves spontaneously. When symptomatic, the clinical presentation typically includes one of the following three syndromes: a flu-like illness, a granulomatous hepatitis or an atypical pneumonia. Individuals presenting risk factors such as patients with valvular heart diseases and vascular prostheses, as well as pregnant women and immuno-sup…

Q fever endocarditis: an unusual presentation.

Coxiella burnetii, the agent of Q fever, is an obligate intracellular pathogen typically associated with exposure to livestock. This organism may present with acute or chronic manifestations, the most typical chronic manifestation being endocarditis. We report a highly unusual case of Q fever endocarditis acquired in the United States marked by atypical cardiac manifestations.

[Fatigue after Q fever: nothing new].

New guidelines for the diagnosis and treatment of Q-fever related fatigue syndrome have been proposed. However, we argue that Q-fever related fatigue syndrome is only just another description of a chronic fatigue syndrome in which a specific micro-organism is implicated. We feel that development of this guideline comes too soon and may be redundant, as relevant diagnosis and treatment protocols for patients with chronic fatigue, without somatic or psychiatric cause, are already available and current evidence does not support a distinct guideline for fatigue after Q fever. Rather, chronic infec…

Vascular complications of Q-fever infections.

INTRODUCTION: Coxiella burnetii is a strict intracellular pathogen causing Q fever, a worldwide zoonosis with an extensive animal reservoir. Chronic Q fever infections are frequently associated with cardiovascular complications, mainly endocarditis, and also aortic aneurysms and vascular-graft infection. We present four cases of chronic Q fever infections and associated vascular complications, and review the literature to identify major symptoms and assess the prevalence, treatment and outcome in these challenging patients. MATERIALS AND METHODS: The demographic and clinical data of four pati…

Q fever: underdiagnosed in Hong Kong?

We report the first imported case of chronic Q fever with multi-organ involvement seen in Hong Kong. Although the disease is found worldwide, its chronic form is very rare in our locality. Familiarity with the clinical presentation, useful diagnostic tools, and appropriate treatment is necessary for the prevention of the serious morbidity and mortality associated with chronic Q fever. To the best of our knowledge, this article represents the first comprehensive review to compare the local experience with Q fever with international data, and establishes a management approach for this unusual in…

[Infection by Coxiella burnetii (Q fever)].

In spite of being described over 60 years, Q fever is still a little known disease. The exact prevalence is also unknown, but probably the number of cases of Q fever is underestimated. There is much variation in the clinical presentation, including severe forms with a poor prognosis. Acute cases often present as an asymptomatic infection, flu-like syndrome, pneumonia or hepatitis. Presumably, host factors play an important role in the development of chronic disease, which may present as endocarditis with negative blood culture. The diagnosis of Q fever should be considered in cases of fever of…

Australia's national Q fever vaccination program.

A nationally funded Q fever vaccination program was introduced in Australia in 2002. The evaluation of this unique program included measures of program uptake, safety, and notification and hospitalisation rates for Q fever pre- and post-program implementation. Program uptake ranged from close to 100% amongst abattoir workers to 43% in farmers. The most commonly reported adverse event was injection site reaction. Q fever notification rates declined by over 50% between 2002 and 2006, particularly in young adult males, consistent with the profile of the abattoir workforce. Hospitalisation data sh…

Q fever endocarditis; not always expected.

Q fever endocarditis is a chronic disease with protean manifestations. The clinical and serological manifestations of nine patients diagnosed as having Q fever endocarditis during a 19-year period are reviewed. Four patients (44%) required valve replacement due to congestive heart failure. Three of these four patients were diagnosed as having Q fever endocarditis only after elective valve surgery, by histopathological examination of the valve and subsequent serological tests. Prior to surgery they were afebrile and had no other symptom or sign indicative of endocarditis. The antibiotic treatme…
