Diagnostic distribution of non-traumatic upper limb disorders: vibrotactile sense in the evaluation of structured examination for optimal diagnostic criteria.

BACKGROUND: Upper limb disorders (ULDs) are common, and so are the difficulties in specific diagnoses of these disorders. Prior studies have shed light on the nerves in the diagnostic approach beside disorders related to muscles, tendons and joints (MCDs). OBJECTIVE: The study aimed to compare the distribution of upper limb disorders, and the vibration perception threshold (VPT) in different diagnostic groups according to 1) A-criteria: the SALTSA consensus criteria, including MCDs and four peripheral neuropathies, and 2) B-criteria: including MCDs and 10 different neuropathy diagnoses--re-de

Double Supinator Branch Transfer to Posterior Interosseous Nerve May Confer Superior Hand Opening Restoration Outcomes in Brachial Plexus and Spinal Cord Injury Compared to Single Branch.

BACKGROUND AND OBJECTIVES: Cervical spinal cord injury (SCI) and lower trunk brachial plexus injury (BPI) commonly result in hand paralysis. The supinator to posterior interosseous nerve (SPIN) transfer is a well-established surgical approach that can achieve restoration of hand opening with similar outcomes in SCI and BPI. Most often, the radial nerve has 2 branches to the supinator. Studies have reported utilization of either single or double supinator branches as donors for the SPIN transfer. The aim of this study was to study whether using one or both supinator branches can affect recovery

Humeral shaft fracture: systematic review of non-operative and operative treatment.

INTRODUCTION: Humeral shaft fractures can be treated non-operatively or operatively. The optimal management is subject to debate. The aim was to compare non-operative and operative treatment of a humeral shaft fracture in terms of fracture healing, complications, and functional outcome. METHODS: Databases of Embase, Medline ALL, Web-of-Science Core Collection, and the Cochrane Central Register of Controlled Trials (CENTRAL) were systematically searched for publications reporting clinical and functional outcomes of humeral shaft fractures after non-operative treatment with a functional brace o

['The man with the withered hand': iconography of the saturnine palsy?].

INTRODUCTION: The synoptic gospels tell about a man who cannot extend his hand, that appeared dried. The description is consistent with radial palsy. Christian artists depicted this gospel story producing a rich material, unexplored from the medical point of view. AIM: To analyze the mentioned iconography verifying the depicted lesions and their possible causes. DEVELOPMENT: Six representations of the evangelical text pertaining to the period comprehended between 10th and 17th centuries were selected. Four belong to illuminated gospels, one is a mosaic and the other one is a fresco. In three

Prognostic Value of Nerve Ultrasound and Electrophysiological Findings in Saturday Night Palsy.

BACKGROUND AND PURPOSE: We report on the prognostic role of the cross-sectional area (CSA) enlargement and conduction block (CB) in radial neuropathy (Saturday night palsy [SNP]). METHODS: Reference CSA values were defined in 30 healthy subjects. Twenty-four patients with SNP underwent evaluation (Thessaloniki Hypesthesia Score [THS], Medical Research Council [MRC], ultrasound, electrophysiology). All patients were followed up 3 months after initial presentation. RESULTS: During initial evaluation, 13 patients showed pathological CSA (pCSA) and 11 normal CSA (nCSA). Fourteen patients showed …

The radio-radial nerve transfer for elbow extension restoration in C5 to C7 nerve root injury.

Extension of the elbow is required to oppose gravity; however, activation of the triceps brachii is frequently underestimated during the surgical planning for brachial plexus injuries. This report aims to describe a novel technique of distal nerve transfer designed for elbow extension reconstruction in patients sustaining a C5-C7 nerve root injury. We report a patient sustaining a brachial plexus injury with triceps palsy and preserved finger extension motion; after careful intraneural dissection of the radial nerve, a fascicle innervating the extensor digitorum communis muscle was sectioned,

[Radial nerve palsy associated with humeral shaft fractures - early exploration or expectant procedure? An analysis concerning current strategies of treatment].

BACKGROUND: With a prevalence from 11.8 to 18% no fracture is as often associated with nerve damage as the humeral shaft fracture. Whether the radial nerve should be surgically explored in association with a palsy in humeral shaft fractures in order to exclude an interposition or discontinuity is being discussed controversially. The aim of this study was to assess the strategies for primary care of radial nerve palsies associated with humeral shaft fractures in Germany. MATERIAL AND METHODS: In a standardised survey 495 traumatological and 134 neurosurgical clinics were interviewed regarding

Delayed radial nerve palsy due to entrapment of the nerve in the callus of a distal third humerus fracture.

A 45-year-old male patient was referred for management of radial nerve palsy. His past medical history revealed that he had been injured in a car accident and broken his left humerus 4 months ago. Primary stabilization of the fracture has been achieved by the application of a long-arm plaster cast. His medical reports displayed that he had experienced no clinical signs of radial nerve palsy at that time. After the cast was removed, he noticed that he could not extend his wrist. Surgical exploration revealed that the radial nerve was encased inside the callus. He had probably not realized the w

Neurolysis with Amniotic Membrane Nerve Wrapping for Treatment of Secondary Wartenberg Syndrome: A Preliminary Report.

BACKGROUND: Entrapment of the superficial sensory branch of the radial nerve (SRN) commonly results in debilitating pain of the dorsoradial wrist. Symptom relief following SRN neurolysis is often incomplete or temporary due to recurrent perineural scarring. METHODS: We performed a retrospective review with prospective follow-up of all patients with SRN neuropathy who were treated with neurolysis and nerve wrapping using an amnion-based allograft adhesion barrier over a one-year interval. Measured outcomes included pain rated by Visual Analog Scale (VAS) and Quick Disabilities of the Arm, Shou

Ozoralizumab-Induced Multifocal Demyelinating Neuropathy in a Patient With Rheumatoid Arthritis.

Tumor necrosis factor-α (TNF-α) inhibitors can induce demyelinating polyneuropathy, but this has not been reported with ozoralizumab. We report a case of a 57-year-old male with rheumatoid arthritis who developed paresthesia in the left hand, bilateral wrist drop, and reduced grip strength after starting ozoralizumab. Nerve conduction studies showed bilateral radial nerve conduction blocks consistent with demyelination. Symptoms persisted after discontinuation of ozoralizumab but resolved after two courses of intravenous immunoglobulin therapy, with complete clinical and electrophysiological r

Brachial neuropathy after brachial artery-antecubital vein shunts for chronic hemodialysis.

Peripheral mononeuropathies may complicate distal arteriovenous fistulas for chronic renal dialysis. We observed three diabetic patients who developed pain, paresthesias, and weakness in the distribution of the median, ulnar, and radial nerves shortly after construction of proximal brachial artery-antecubital vein fistulas. EMG confirmed multiple distal nerve injuries. All three patients improved after shunt banding or ligation. Twenty additional patients with proximal shunts were examined for risk factors for brachial neuropathy. Although all patients had severe atherosclerosis and many had p

Appropriately Selected Nerve in Suspected Leprous Neuropathy Yields High Positive Results for Mycobacterium leprae DNA by Polymerase Chain Reaction Method.

Identification of Mycobacterium leprae DNA by polymerase chain reaction (PCR) is a reliable and an affordable method to confirm leprosy. DNA from 87 nerve samples (61 from paraffin blocks and 26 fresh samples) was extracted. Mycobacterium leprae DNA was amplified by PCR from 80/87 (92%) specimens. Patients were seen over a period of 11 years (2007-2019), and leprosy was diagnosed based on clinical and characteristic histopathology findings. The clinical diagnostic possibilities were as follows: leprous neuropathy in 73/80 (91.3%), mononeuritis multiplex of unknown etiology in four (5.0%), vasc

Humeral shaft fracture and radial nerve palsy in Korean soldiers: focus on arm wrestling related injury.

INTRODUCTION: Humeral shaft fractures can lead to radial nerve injury and may require surgery and rehabilitation. We determined the causative events of humeral fracture, including arm wrestling, in young Korean soldiers and examined whether humeral fracture is related to demographic characteristics and the presence of radial nerve palsy. METHODS: We reviewed 7.5 years (July 2012 to June 2019) of medical records covering patients who had experienced a humeral shaft fracture after entering military service and had received surgery for open reduction and internal fixation. Data were obtained on

Factors associated with radial nerve palsy after operative treatment of diaphyseal humeral shaft fractures.

BACKGROUND: The radial nerve is at risk after diaphyseal humeral fracture or surgery to repair the fracture. We hypothesized that there are no factors associated with iatrogenic radial nerve palsy and, secondarily, that there are no factors associated with traumatic radial nerve palsy or radial nerve palsy of any type. METHODS: We analyzed 325 adult patients who underwent operative treatment of a diaphyseal humerus fracture at 6 hospitals between January 2002 and November 2014 to determine factors associated with a radial nerve palsy. We excluded patients with pathologic fractures, fractures

The radial nerve bridging plate technique for humeral shaft fractures: an approach facilitating plate removal without impeding fracture union - a case series.

BACKGROUND: Removing a plate of the humeral shaft with the radial nerve on top of the plate is challenging and carries a high risk for iatrogenic radial nerve palsy (iRNP). This study aimed to assess the incidence of iRNPs following plate removal of a radial nerve bridging plate (RNBP) and to evaluate outcomes of this RNBP in treatment of a humeral shaft fracture. METHODS: This was a single-center retrospective case series. Patients treated between May 2013 and December 2025 with a RNBP for a humeral shaft fracture or nonunion were included. The primary outcome measure was the incidence of iR
