Image-guided treatment of Budd-Chiari syndrome: a giant leap from the past, a small step towards the future.

Budd-Chiari syndrome (BCS) is a relatively rare vascular disease characterized by hepatic outflow tract obstruction, and image-guided endovascular treatment, namely percutaneous angioplasty, stenting, and transjugular intrahepatic portosystemic shunt (TIPS), has proven to be effective treatment modalities to alleviate symptoms and markedly improve the prognosis of the disease. Specifically, a step-wise approach is recommended, i.e., angioplasty and stenting are the prioritized choice for patients with membranous obstruction and short-length stenosis, whereas TIPS is the option for patients who

Giant gastric stromal tumor mimicking as a posterior mediastinal mass: A case report and literature review.

RATIONALE: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal tract. Mediastinal GISTs are rare and mostly arise from the esophagus. PATIENT CONCERNS: A 68-year-old woman with dysphagia who presented with a huge posterior mediastinal mass. DIAGNOSES: The patient was diagnosed with a GIST through chest computed tomography (CT)-guided core biopsy of the mass. INTERVENTIONS: Complete excision including the tumor, lower part of the esophagus and fundus of the stomach were performed. OUTCOMES: On follow-up after 48 months, the patient is curre

Efficacy of Two Radiologic-Assisted Prophylactic Gastropexy Techniques.

OBJECTIVE: To describe the technique and clinical outcome of prophylactic percutaneous radiologic-assisted gastropexy (PRG) and radiologic-assisted incisional gastropexy (RIG) in client-owned dogs. STUDY DESIGN: Prospective, nonrandomized clinical trial. ANIMALS: Fourteen client-owned, large, and giant breed dogs. METHODS: Four dogs underwent PRG with fluoroscopic guidance to place 2 T-fasteners into the pyloric antrum and secure it to the right body wall. Ten dogs underwent RIG, which was a modification of PRG. For RIG, the T-fasteners were first used to approximate the pyloric antrum and

Balloon frontal sinuplasty for intracranial abscess in a pediatric acute sinusitis patient.

A case of balloon frontal sinuplasty in a 12-year old male with intracranial abscess from acute sinusitis is presented. The patient experienced photophobia, fever, headache, nausea and vomiting. Frontal sinusitis with intracranial abscess was diagnosed on imaging. The patient was taken to the operating room for drainage with left frontal balloon sinuplasty. The patient showed immediate clinical improvement, did not suffer from any complications of surgery and was further managed with long term intravenous antibiotics. We believe that balloon frontal sinuplasty is potentially safe and effective

CT-guided transforaminal cervical and lumbar epidural injections.

Transforaminal injections are widely used. Serious complications including strokes and paraplegia have been reported after transforaminal injections of corticosteroids, and the Afssaps (2011) has issued a warning about their use [1]. The needle must be positioned in the posterior aspect of foramen, and its correct placement validated by an injection of contrast product. It is preferable to choose cortivazol (Altim(®)) as the corticoid for injection. This procedure is simple, reproducible, and durably effective in 60 to 70% of cases. Complications and adverse effects are rare but potentially se

Swiss population exposure to radiation by interventional radiology in 2008.

The aim of this study was to investigate the radiation exposure of the Swiss population to interventional procedures. A nationwide survey was conducted in Switzerland. The annual effective dose per capita due to interventional procedures was found to be 0.14 mSv, corresponding to 12% of the total dose. Coronary angiography and percutaneous coronary interventions were found to be the most frequent and the most irradiating interventional procedures, accounting for 52% of the total examination frequency and 64% of the dose delivered to the population. Switzerland stands at the same level as other

[A characteristic of angiographic cone-beam CT].

Angiographic cone-beam CT, called DynaCT by SIEMENS, is a 3D imaging tool reconstructed from projection data by a rotational C-arm with a flat panel detector. It can visualize low-contrast objects such as soft tissue or small vessels as well as high-contrast structures such as enhanced vessels or bone. We need to understand its image characteristics and dose distribution during 200 degree rotation around a patient. In this research, we evaluated fundamental characteristics and dose effectiveness for optimized clinical images. DynaCT, including soft tissue information and isochronal voxel data

Interventional management of massive hemothorax due to inadvertent puncture of an aberrant right subclavian artery.

We report a case of massive hemothorax due to inadvertent puncture of an aberrant right subclavian artery during central venous access. Iatrogenic laceration at the origin of the right internal thoracic artery was successfully treated with coil embolization of the internal thoracic artery followed by stent-graft placement into the subclavian artery. Due to its elongated and abnormal course, an aberrant right subclavian artery may predispose to inadvertent puncture during vein catheterization and should be recognized as a potential threat for such procedures. Our case emphasizes that ultrasound

Deterministic effects in interventional radiology.

Since the development of interventional radiology, the number and complexity of procedures has increased significantly and continues to grow. Interventional radiology procedures offer substantial health care benefits. However, associated with the increasing complexity as well as a lack of quality control programmes and specific training in radiation protection, there is an increase in the occurrence of deterministic effects in both patients and staff. There is a growing literature of case reports describing deterministic effects. A review of some case reports and the response of various intern

The arthroscopic management of intra-articular distal radius fractures.

Intra-operative arthroscopy and fluoroscopy provide improved visualisation and guide the restoration of intra-articular distal radial fractures while minimising the operative dissection required for their stabilisation. Radial styloid fractures, distal radial fractures with dorsal, palmar or combined ulnar-sided "die punch" fragments, palmar and dorsal Barton's fractures, and various three- and four-part intra-articular fractures without significant bone loss or defect are especially suited for this technique. The experienced arthroscopist may wish to apply the technique to more severely commi

[Therapeutic embolization of the spleen].

Spleen embolization is a method of percutaneous occlusion of instrasplenic vascular network, by which is achieved a partial embolization of arterial branches with the convenience of controlled circulation reduction. Reduction achieved in this way corrects clinical and laboratory symptoms of hypersplenism and improves hematologic status in the diseases of lymphopoietic tissues where splenectomy is otherwise the therapeutic solution. This method had been applied experimentally, and afterward clinically since 1973, but with numerous complications and incidents. The authors had discussed those com

Percutaneous vertebroplasty: state of the art.

Vertebroplasty is an effective new radiologic procedure consisting of the percutaneous injection of a biomaterial, usually methyl methacrylate, into a lesion of a vertebral body. This technique allows marked or complete pain relief and bone strengthening in most cases. The principal indications for vertebroplasty are osteolytic metastasis and myeloma, painful or aggressive hemangioma, and osteoporotic vertebral collapse with debilitating pain that persists despite correct medical treatment. Radiography and computed tomography must be performed in the days preceding vertebroplasty to assess the

[Interventional radiology in thoracic emergencies].

There are numerous clinical situations in which interventional angiography fully reveals its two-fold diagnostic and therapeutic value. The present review focuses attention on the use of such procedures in certain thoracic emergencies. Indications, diagnostic results and therapeutic advantages are examined together with possible complications. Pulmonary embolism is a serious circulatory condition that is often difficult to diagnose because of the lack of specificity of its accompanying symptoms. In these cases the role of the angiographic radiologist is often three-fold: diagnosis, therapy (po

[Interventional radiology in the treatment of hemoptysis].

Massive haemoptysis, usually related to lesions of bronchial arteries, is a life-threatening complication of chest lesions. Embolization of bronchial arteries demonstrates immediate favourable results in at least 90% of cases. The main complications of embolization of bronchial arteries are medullary strokes. Consequently it is mandatory to visualize the anterior spinal artery in order to avoid its embolization. Massive haemoptysis of pulmonary arterial origin is far less common. It is also feasible to perform the embolization of the abnormal pulmonary artery in order to control the haemoptysi

[First clinical experience of radio-endovascular functional thyroidectomy in the treatment of diffuse toxic goiter].

To suppress Thyroid pathologic activity in diffuse toxic goitre the authors have for the first time resorted to roentgenoendovascular functional thyroiedctomy. This method consists in vascular isolation of the thyroid by superselective catheterization of the left and right thyroid arteries followed by their material occlusion. For embolization a wide spectrum of nonlyzed synthetic, organic, and inorganic materials were used. The operation was carried out in 32 patients with stages III-IV diffuse toxic gotre. The advantages of this technique over subtotal thyroidectomy are emphasized. A stable
