Lymphedema of the hand following a fracture of the distal radius is a disabling, but rare complication. Although the pathogenesis of this mechanism is poorly understood, extensive review of the literature suggests that an underlying cause of this lymphedema is psychogenic in nature. After numerous therapeutic modalities including intense physiotherapy, sympathetic nerve blocks and hospitalization, our patient still continued to persist with this debilitating disease. This article presents a case of Colles fracture complicated by nonpitting edema in a 62-year-old woman in whom psychogenic cause

We report on a seventy-year-old woman who developed a non-union of the distal radius with infection of the ulnocarpal joint after having sustained a fracture. The fistula, the distal ulna and the ulnocarpal joint were resected. Two months later we removed the non-union and carried out open reduction and internal fixation of the radius with a palmar plate. Good functional and cosmetic results were achieved.

OBJECTIVE: To describe the clinical features and outcome of a series of patients with complete motor and sensory ulnar nerve palsy associated with a fracture of the distal radius.
DESIGN: Retrospective case series.
SETTING: Level 1 trauma center.
PATIENTS/PARTICIPANTS: Five adults with acute complete motor and sensory ulnar nerve palsy associated with fracture of the distal radius were treated during a 2 year period. There were 3 men and 2 women, with an average age of 42 years (range, 33 to 56 years). All 5 distal radius fractures were high energy and widely displaced. Three patients had a…

INTRODUCTION: Fractures of the distal radius are the most frequent fractures of longbones during childhood and adolescence. A rare mechanism … injuries. 61(28.6 %) of them resulted from playing soccer, with 48 (78.7 %) after a fall. 13 distal radius fractures were identified as a goalkeepers injury by a sharply shot ball. We found 6 buckle fractures, 4 complete fractures, 2 physeal separations Salter II and 1 metaphyseal greenstick fracture. At the time of the last follow up investigation all injured had …

Entrapment of the flexor tendon after fracture of both forearm bones is very rare. This report describes entrapment of the middle finger flexor digitorum superficialis (FDS) tendon alone in the radius fracture site. A 13-year-old boy fractured both forearms and had closed reduction and a long-arm cast. After the cast was removed he had full middle finger motion with the wrist flexed but was unable to extend the proximal interphalangeal or metacarpophalangeal joints and could extend only the distal interphalangeal joint of the middle finger with the wrist in the neutral or extension position. We treated a patient with a rare combination of ipsilateral fractures of the distal and proximal ends of the radius. A woman aged 52 years had simultaneous fractures of the distal and proximal ends of the radius (radial head and neck) after she fell from a high place. The fracture of the radial head was treated by open fixation with a cancellous bone screw, and the fractured distal end of the radius was treated by bone graft, with the subsequent application of an external fixator. At the 1-year follow up, the patient had minor residual limitation of forearm pronation and elbow joint extension,

Percutaneous and limited open fixation of fractures of the distal radius is an important method of treatment for many unstable fractures such as unstable dorsal bending fractures, shearing fractures of the radial styloid and lunate facet, and simple articular fractures. The quality of the reduction is monitored with image intensification and the tactic of the reduction is based on manipulation of the fracture fragments by longitudinal traction, percutaneous manipulation, and in some instances by direct manipulation through small incisions. The role of arthroscopy remains uncertain and may repr

First a short survey of the variety of experiments concerning absorbable implants is given. The result of those experiments, especially with large implants are still quite unsatisfactory. For small osteosynthesis the development of Polydioxanon-Pins seems to open new perspectives of therapy. Also some selected indications in the field of hand surgery as well as maxillary surgery are mentioned. Promising characteristics of PDS-Pins, such as gentle application, complete absorbability, no late infections and no removal of metal plates, made us implant these pins in eight patients aged 35 to 58 su

Fortunately, most bone forearm fractures in children heal with minimal functional disability. My approach to management of the most common injuries is presented. By following the guidelines outlined in this article, the generalist as well as the most sophisticated upper extremity surgeon should be able to avoid pitfalls and complications when managing forearm fractures in children.

Simultaneous fracture of the scaphoid bone occurred in 26 (4%) of the 650 injuries of the distal end of the radius seen at our institution. These injuries occur after a fall on the outstretched hand with a pronated wrist joint and extended hand. In a 4-year period (1983-1987), 26 simultaneous fractures of the distal end of the radius and the scaphoid bone were seen. Typically, the fracture of the radius had minimal or slight displacement, and the fracture of the scaphoid bone occurred always as a transverse thin line without displacement. The simultaneous fracture of the scaphoid bone was ofte

Nineteen cases of nerve complications following closed fractures of the lower end of the radius (17 lesions of the median nerve, 4 of which were associated with lesions of the ulnar nerve, and 2 isolated lesions of the ulnar) were observed. A therapeutic approach may be suggested for lesions of the median nerve and/or the ulnar nerve: immediate lesions: immediate reduction, gently if possible and avoiding the position of volar flexion and ulnar deviation. Early secondary lesions associated with immobilization in flexion and ulnar deviation: modify the position and complementary pinning when ne

Ulnar-sided injuries of the wrist have received more attention recently for their potential negative impact on the outcome of distal radius fractures. Radiographs and medical records were retrospectively reviewed for 166 distal radius fractures treated during a 1-year interval. Distal radius fractures were classified according to the AO system, and accompanying ulnar styloid fractures were evaluated for both size and displacement. Each distal radius fracture was also evaluated for radiographic and clinical evidence of distal radioulnar joint instability. The distribution of ulnar styloid fract…

There is not enough evidence in the literature to support the use of any classification system for distal radius fractures (DRF) in adults. However, there is a need for identification of more homogeneous groups of patients with DRF so that the extent of preoperative workup and sophistication of treatment would best match the needs of the patient. The authors propose an innovative method to analyse and stratify acute DRF in adults. A one-page chart includes criteria related to the patient (P), the energy of the accident (A) and the pathology of the fracture (F). Analysis of the pathology includ…
