Upper Extremity Effort Thrombosis.

A 23-year-old, right hand-dominant, male active-duty Marine self-referred to physical therapy with dull right anterior shoulder pain. The physical therapist referred the patient for a diagnostic ultrasound to rule out effort thrombosis of the axillary and subclavian veins. Same-day Doppler ultrasonography revealed an occlusive thrombus in the middle subclavian and axillary veins. J Orthop Sports Phys Ther 2020;50(9):532. doi:10.2519/jospt.2020.9585.

Cerebral response to electric stimulation of the colon and abdominal skin in healthy subjects and patients with irritable bowel syndrome.

BACKGROUND: Visceral hyperalgesia may play an important part in the pathophysiology of the irritable bowel syndrome (IBS). We investigated the neuronal afferent pathways in healthy volunteers and IBS patients by recording evoked potentials (EPs) elicited by electrical stimulation of the colon and abdominal skin inside and outside the referred pain area. METHODS: Six healthy subjects and nine IBS patients met the inclusion criteria. Morphology and topography of EPs to painful electrical stimuli were estimated in the rectosigmoid junction and on the skin inside/outside the referred pain areas. …

Unexpected Pain: Thoracolumbar Disc Herniation and Its Neuralgic Shadows.

Clinicians should consider thoracolumbar neuroforaminal discopathy in patients with atypical inguinal or abdominal pain, especially when common causes are excluded. MRI is crucial for accurate diagnosis. Temporary relief from nerve blocks may suggest spinal pathology. Surgical intervention, such as discectomy, can provide definitive treatment and effectively resolve referred pain.

Spinal lesions simulating hip joint disorders.

In three patients pain in the groin and thigh which was initially attributed to either a malfunctioning hip prosthesis or osteoarthritic hip, was shown to arise in the spine. All patients had positive myelograms and one had electromyographic evidence of nerve root compression. Referred pain from the spine must always be considered in the differential diagnosis of pain thought to be arising from a hip replacement.

Referral to Pain Specialists for Treatment of Intractable Mandibular Pain Caused by Osteonecrosis of the Jaw: A Case Series Report.

Pain is a problematic symptom in patients with osteonecrosis of the jaws (ONJ). Effective pain management in patients with advanced ONJ still remains an unresolved issue. This case series report presents three patients who were referred to the pain clinic for treatment of intractable pain caused by ONJ. Two patients received mandibular nerve blocks and achieved pain relief. After referral to the pain clinic, these two patients underwent segmental mandibulectomy for ONJ. In the third patient, the effect of pain control was limited. Appropriate cooperation between the oral and maxillofacial surg

The effect of transcutaneous electrical nerve stimulation on ocular pain.

The effect of transcutaneous electrical nerve stimulation (TENS) on pain originating in the eye was studied in 10 patients. All three patients subjected to TENS during panretinal photocoagulation reported marked reduction in pain. Three of five patients who had persistent pain following scleral buckling and/or vitrectomy reported partial or complete relief of pain during the application of TENS. One patient had equivocal relief of pain when TENS was used during retinal cryopexy; 1 patient with acute pain following paracentesis and 1 patient with persistent pain following cyclocryotherapy had n

Percutaneous Transforaminal Endoscopic Radiofrequency Ablation of the Sinuvertebral Nerve in an Olympian with a Left L5 Pedicle/Pars Interarticularis Fracture-Associated Left L5-S1 Disk Desiccation.

BACKGROUND: Irritation of the sinuvertebral nerve by a posterior or posterolateral disk desiccation can cause somatic referred pain that can mimic a lumbar radiculopathy. We present a case of a patient presenting with this condition and the positive result in pain improvement after endoscopic radiofrequency ablation of the sinuvertebral nerve. CASE DESCRIPTION: An 18-year-old Olympic runner presented to our clinic with back pain and left leg pain in a clear L5 distribution. He did not have a history of trauma. His imaging did not demonstrate any lesion causing compression of the left L5 nerve…

Somatosensory and clinical profiles of patients with spine-related and clinical framework-based neck-arm pain.

Spine-related neck-arm pain is heterogeneous and may present on a spectrum between nociceptive and neuropathic pain. A recently developed mechanism-based clinical framework for spine-related pain distinguishes between spinally referred pain without neurological deficits (somatic referred pain, heightened nerve mechanosensitivity, radicular pain), with neurological deficits (radiculopathy), and mixed-pain presentations. This study investigated differences in somatosensory and clinical profiles of patients with unilateral spine-related neck-arm pain grouped according to the clinical framework. P…

Cognitive factors are associated with disability and pain, but not fatigue among physiotherapy attendees with persistent pain and fatigue.

OBJECTIVES: Most research exploring the relationship between cognitive factors and pain, disability and fatigue in patients with persistent pain/fatigue has been performed in multi disciplinary environments. It is unclear whether these associations are consistent in other contexts. This study therefore aimed to establish the relationships between these factors in patients with persistent pain/fatigue referred for physiotherapy treatment. DESIGN: Cross-sectional observational study assessing the association between cognitive factors (self-efficacy and catastrophizing) and levels of pain, disab

BACKGROUND: Chronic neck pain can be a difficult problem to evaluate and treat, as it can have several different causes. We studied a series of patients with neck pain near the superomedial aspect of the scapula that was referred pain from inflammation of the shoulder secondary to chronic impingement. We postulate that some patients with specific clinical findings and neck pain can benefit from treatment of shoulder impingement. METHODS: We conducted a retrospective review of the charts of thirty-four patients with neck pain who met three criteria for the diagnosis of shoulder impingement syn…

Chest pain in children and adolescents: epigastric tenderness as a guide to reduce unnecessary work-up.

Our objective was to evaluate and highlight the significance of epigastric tenderness in children and adolescents with chest pain. In a 26-months period, patients who were referred for pediatri cardiology evaluation at Shiraz University of Medical Sciences with chief complaint of chest pain were studied. Patients with epigastric tenderness were evaluated endoscopically by gastroenterologist. Patients who had positive findings on endoscopy were appropriately treated. Since there were no serious findings on cardiac evaluation, no other treatment was necessary. The patients were re-evaluated 4 we

Intrapartum referral from primary to secondary care in the Netherlands: a retrospective cohort study on management of labor and outcomes.

BACKGROUND: In the Dutch maternity care system, primary care midwives provide care to low-risk women and refer to obstetricians if risks or complications occur. We examined reasons for referral, management of labor, and maternal and neonatal outcomes among women who were referred during labor. METHODS: In a retrospective cohort study, descriptive analyses were performed on data obtained from patient records. Six purposively chosen hospitals in The Netherlands participated in the study from June 2011 to February 2012. The study population included 600 pregnant women who were referred during la

Chest pain in general practice or in the hospital emergency department: is it the same?

OBJECTIVE: The aim of the present study was to provide a description of the impact of setting on the diagnostic case mix that is identified in consecutive patients presenting with chest pain. METHODS: A cross-sectional study was carried out of patients presenting with chest pain according to setting: general practice, self-referred, referred or arriving by ambulance at the hospital emergency department (ED). GPs from 25 general practices situated in the Flemish part of Belgium were recruited, and the hospital involved was a major teaching hospital in the same area. A total of 320 patients in

Evaluating the patient with low back pain.

In the UK, low back pain is the most common cause of disability in young adults and every year 6-9% of adults consult their GP about back pain. A thorough history and examination is required to exclude an alternative diagnosis, such as pain arising from the hip or trochanteric bursa and to categorise patients as having: serious spinal pathology, nerve root/radicular pain or non-specific back pain. Inflammatory back pain is often missed, particularly in the early stages when examination may be normal. The primary features are pain arising in patients under 40, thoracolumbar or sacroiliac pain a

Temporomandibular joint disorders in patients referred for third molar extraction.

BACKGROUND: Third molar removal has been implicated as a precipitating event for temporomandibular joint disorders. The aim of this study was to determine what proportion of patients had pre-existing pain and dysfunction that could be attributed to the temporomandibular joints. METHODS: Sixty patients referred for third molar removal were clinically examined and a history of their presenting complaint recorded at the initial consultation visit. Patients were then diagnosed and categorized. RESULTS: Of the total number of patients examined, 13.3 per cent showed signs and symptoms of temporoma
