Tubulovenous communication in myeloma kidney. Tubular prolapse and tubular transformation of renal vein.

We studied renal histopathologic findings of 20 autopsy cases with multiple myeloma with particular attention to structural tubulovenous relationship. Urinary deposition in renal vein or lymphatics, which has been previously noted in pyelonephritis, obstructive uropathy, or vesicoureteral reflux was found in 14 cases. Four of 14 cases had unique tubulovenous communications designated as tubular prolase and tubular transformation of vein. Such communications were most frequently seen at the architectural weak point where the thin-walled interlobar vein and their tributaries adjoin the renal pyr

The effect of peritoneovenous shunting on catecholamine metabolism in patients with hepatic ascites.

The elevated catecholamine levels in cirrhotic patients with ascites have been proposed to be due to sympathetic overactivity secondary either to reduced intravascular volume or to an underlying cardiovascular abnormality such as reduced pressor responsiveness. Furthermore, these elevated catecholamine levels have been proposed to be involved in the pathogenesis of salt and water retention. Therefore, the effect of peritoneovenous shunting on the circulating levels and renal excretion of catecholamines were studied in six patients with massive refractive ascites preoperative within the first 8

Pathophysiology, diagnosis and clinical management of hepatorenal syndrome: from classic to new drugs.

Advanced cirrhosis is frequently associated with renal dysfunction. Hepatorenal syndrome (HRS) is characterized by the occurrence of kidney injury in cirrhotic patients in the absence of other identifiable causes. HRS is classified in 2 different types. Type 1 is characterized by acute renal failure and rapid functional deterioration of other organs, usually related to a precipitating event. Type 2 is characterized by slowly progressive renal failure and refractory ascites. Advanced liver disease induces the progression of hemodynamic alterations such as arterial vasodilation of splanchnic cir

Liver and systemic hemodynamics in cirrhotic children.

Portal hypertension and cirrhosis are associated with severe hemodynamic changes in hepatic and systemic circulation in the adult population. During cirrhosis progression, circulation becomes hyperdynamic, with cardiac, pulmonary and renal consequences. Cirrhotic adults also present with cirrhotic cardiomyopathy, with systolic and diastolic dysfunction and electrophysiological abnormalities. This article provides an update on normal liver hemodynamics, a brief reminder of the liver and systemic hemodynamics in cirrhotic adults, and a description of liver and systemic hemodynamics in cirrhotic

[Efficacy of using Naftusya type mineral water in pregnant women with cardiovascular diseases and concomitant pyelonephritis].

The present study focuses on blood circulation in pregnant women presenting with impaired renal function. In chronic pyelonephritis (CP), at the expense of increase in volume indicies (stroke index, systolic index) peripheral resistance gets moderately increased, with the hyperkinetic type circulation being mainly formed. The medicamentous method of treatment is but unacceptable more often as not because of its toxic action on the fetus, on account of which fact the complex of sanatorium-and-health resort treatment of pregnant women has been supplemented with the lightly mineralized hydrocarbo

The effects of steroid hormones on vascular tissue.

Results of clinical and experimental studies suggest that extrarenal actions of corticosteroid hormones must be involved in the overall effects of these steroids on the circulation. A dissociation between levels of mineralocorticoids and sodium balance or renal effects of steroid hormones is often observed in patients with primary or secondary forms of hypertension. Receptors for various steroids have been found in the peripheral circulation. Stimulation of these receptors probably leads to changes in the electrolyte content of the cell. In turn, these changes have a direct or indirect effect

Hypertension in the systemic blood of animals with experimental renal hypertension.

1. A method has been developed which makes possible the demonstration of a pressor substance in the circulating systemic blood of dogs with experimental renal hypertension. 2. After the intravenous injection of renin into normal dogs, it was possible to detect a pressor substance formed in the systemic blood. After the intravenous injection of 1 unit of renin, as much as 1 unit of the pressor substance was detected in the plasma from 200 cc. of systemic blood. 3. Large amounts of systemic blood pooled from several normal dogs did not contain detectable amounts of pressor substance. 4. In exper

[Pneumonia in patients with terminal renal failure on programmed hemodialysis].

Of 183 patients with terminal renal failure (TRF) on programmed hemodialysis 12(6.5%) were diagnosed to have acute pneumonia. As a rule, the disease develops in hyperhydration and hypertension of the lesser circulation, more rarely concomitant heart disease is diagnosed which runs with general circulation insufficiency. This indicates that pneumonia in dialysis patients is secondary. Pulmonary congestion in pneumonia debut may mask infiltrative alterations. Therefore, in obscure cases it is desirable to combine x-ray examination with polyposition tomoscintigraphy of the lungs. The agent was ve

Schistosomal glomerular disease (a review).

In this review paper schistosomal glomerulopathy is defined as an immune-complex disease. The disease appears in 12-15 per cent of the individuals with hepatosplenic schistosomiasis. Portal hypertension with collateral circulation helps the by pass of the hepatic clearance process and the parasite antigens can bind to antibodies in the circulation and be trapped in the renal glomerulus. Chronic membranous-proliferative glomerulonephritis is the most common lesion present and the nephrotic syndrome is the usual form of clinical presentation. The disease can be experimentally produced, and schis

Case Report: Recurrent Deposition in Renal Allografts: A Rare Case of Fibronectin Glomerulopathy Overlooked in Native Kidneys.

Fibronectin glomerulopathy (FNG) is a rare inherited kidney disease characterized by extensive deposition of fibronectin in the glomeruli, especially in the mesangial and subendothelial regions. The disease progresses slowly and eventually leads to kidney failure in 15-20 years. Here, we report an interesting case. The patient presented with proteinuria and was diagnosed with immune complex-mediated glomerulonephritis, and lupus nephritis was suspected. This patient progressed to end-stage renal disease after 18 years and received an allogeneic kidney transplant. However, proteinuria recurred

The Proteomic Profile of Adults With a Fontan Circulation.

BACKGROUND: The Fontan circulation is associated with unique physiology resulting from surgical and hemodynamic alterations. Limited data exist on the circulating proteome in adults with a Fontan circulation. OBJECTIVES: We aimed to understand how the Fontan circulation impacts the circulating plasma proteome, potentially identifying molecular targets to prevent or treat Fontan-associated complications. METHODS: We enrolled 162 adults with a Fontan circulation and 36 age/sex-matched control participants without known cardiovascular disease: 192 participants (161 = Fontan, 31 = Control) had 9

Histones and Neutrophil Extracellular Traps Enhance Tubular Necrosis and Remote Organ Injury in Ischemic AKI.

Severe AKI is often associated with multiorgan dysfunction, but the mechanisms of this remote tissue injury are unknown. We hypothesized that renal necroinflammation releases cytotoxic molecules that may cause remote organ damage. In hypoxia-induced tubular epithelial cell necrosis in vitro, histone secretion from ischemic tubular cells primed neutrophils to form neutrophil extracellular traps. These traps induced tubular epithelial cell death and stimulated neutrophil extracellular trap formation in fresh neutrophils. In vivo, ischemia-reperfusion injury in the mouse kidney induced tubular ne

A circulating permeability factor in focal segmental glomerulosclerosis: the hunt continues.

Primary focal segmental glomerulosclerosis (FSGS) is one of the major causes of steroid-resistant nephrotic syndrome, and renal prognosis in patients with steroid-resistant FSGS is poor. It has been long speculated that a circulating permeability factor should be implicated in the pathogenesis of the disease because a substantial portion of the patients with primary FSGS experience recurrence shortly after transplantation. Although molecules such as cardiotrophin-like cytokine 1 (CLC-1) and anti-CD40 antibody have been proposed to be potential circulating permeability factors, a definitive fac

The assay of 1alpha,25-dihydroxyvitamin D3: physiologic and pathologic modulation of circulating hormone levels.

A sensitive radioreceptor assay for 1alpha,25-dihydroxyvitamin D3 (1alpha,25-(OH)2D3) is utilized to quantitate the circulating concentration of this sterol in experimental animals and humans. When weanling rats are grown for 2 weeks on low calcium or low phosphate diets, limited availability of either ion elicits a five-fold increase in the plasma level of 1alpha,25-(OH)2D3. The enhancement of 1alpha,25-(OH)2D3 in calcium deficiency is dependent upon the presence of the parathyroid and/or thyroid glands, which is consistent with parathyroid hormone (PTH) mediation of this effect. In contrast,

Use of the gastroduodenal artery in right renal artery revascularization.

Of 15 patients having revascularization of the right renal artery with the use of the hepatic circulation from May 1984 through March 1987 at the Massachusetts General Hospital, eight patients had this accomplished with end-to-end anastomosis of the gastroduodenal artery and right renal artery. Operative indications were acute azotemic renal failure (three patients), poorly controlled renovascular hypertension (four patients), and staged repair of bilateral renal artery disease (one patient). All revascularizations were successful in restoring renal function or rendering hypertension manageabl

Antihypertensive effect of nitrendipine in the hypertensive patient with renal impairment.

Calcium antagonists exert several characteristic effects on the kidney that potentiate their antihypertensive effect. The objective of the present study was to investigate the effectiveness of nitrendipine in the presence of different degrees of renal impairment. Two groups of hypertensive patients were included in the study. Group 1:10 patients with arterial hypertension secondary to chronic renal parenchymatous disease and adequately controlled with a diuretic and/or a beta-blocker who were switched to nitrendipine. These patients were then followed monthly for 1 year. Group 2:24 patients di

Recurrent Goodpasture's disease due to a monoclonal IgA-kappa circulating antibody.

We describe the case of a 54-year-old man who first presented with a clinical syndrome manifested by recurrent pulmonary hemorrhage, hematuria, and mild renal insufficiency. Direct immunofluorescence of renal biopsy sections showed linear deposition of IgA-kappa in the glomerular (GBM) and tubular basement membranes. Serum protein immunoelectrophoresis was positive for a monoclonal immunoglobulin A (IgA)-kappa protein. Serum analysis showed circulating IgA anti-GBM antibodies. Treatment with high-dose steroids, cyclophosphamide, and plasma exchange resulted in resolution of the clinical pictur
