Associations of retinal microvascular signs and intracranial large artery disease.

BACKGROUND AND PURPOSE: Intracranial large artery disease (ICLAD) is a major cause of ischemic stroke. Retinal microvascular changes are associated with stroke, including small vessel cerebral disease and extracranial carotid disease. We examined the relationship between ICLAD and retinal microvascular changes. METHODS: This is a prospective cohort of 802 acute ischemic stroke patients. Retinal changes were assessed from photographs by graders masked to clinical data. ICLAD was evaluated using prespecified criteria. RESULTS: ICLAD was not associated with ipsilateral retinal arteriolar/venula

Central retinal artery occlusion in Wegener's granulomatosis: a diagnostic dilemma.

PURPOSE: To report a case of central retinal artery occlusion (CRAO) in a patient with biopsy-verified Wegener's granulomatosis (WG) with positive C-ANCA. METHODS: A 55-year-old woman presented with a 3-day history of acute painless bilateral loss of vision; she also complained of fever and weight loss. Examination showed a CRAO in the left eye and angiographically documented choroidal ischemia in both eyes. RESULTS: The possibility of systemic vasculitis was not kept in mind until further studies were carried out; methylprednisolone pulse therapy was then started. Renal biopsy disclosed foc…

[Central retinal artery occlusion following coronary angiography: a case report].

We report the case of a 74-year-old man referred in ophthalmology for acute central visual loss in his right eye 2 hours after a coronary angiography. Visual acuity was limited to light perception RE and 20/20 LE. Fundus examination revealed a central retinal artery occlusion with retinal edema and a cherry-red spot in the right eye. Fluorescein angiography confirmed that the central retinal artery was not filling. Despite selective ophthalmic artery fibrinolysis, visual acuity remained very low. Embolic occlusion is the most probable etiology in this complication. Central retinal artery occlu…

Unilateral electronegative ERG in a presumed central retinal artery occlusion.

A unilateral electronegative electroretinogram (ERG) was seen in a 94-year-old man with presumed central retinal artery occlusion. Goldmann perimetry revealed central scotoma in the right eye and no abnormalities in the left eye. Full-field ERG in the right eye described a reduction of the b-wave with a relative preservation of the a-wave which is characteristic of electronegative ERG. Hence, our case illustrates that ERG testing is essential for the work-up of individuals with suspected retinal vascular disorders.

Branch retinal artery embolization due to calcific aortic valve stenosis.

PURPOSE: Branch retinal artery occlusion caused by calcific embolization secondary to calcific aortic valvulopathy. METHODS: A 45-year-old woman came to our attention complaining a sudden painless loss of her peripheral superior visual field. Best visual acuity was 20/20. Fundus examination revealed the presence of a retinal arterial embolic occlusion of the inferior branch. Fundus retinography, visual field, and fluorescein angiography were performed and medical therapy was started. The echocardiography examination revealed a tricuspid and calcified aortic valve with moderate stenosis and re…

Functional in vivo assessment of retinal artery microirregularities in glaucoma.

PURPOSE: We investigated whether retinal branch arteries in healthy subjects, and non-treated and treated primary open-angle glaucoma (POAG) patients show irregular local patterns during dynamic reaction to acute increases of different magnitudes in intraocular pressure (IOP). METHODS: Nine POAG patients and nine age-matched normal volunteers were examined with the retinal vessel analyser (RVA) using a suprasystolic IOP increase (Study 1). Fourteen POAG patients and 13 age-matched controls were examined using a moderate IOP increase for 100 seconds (Study 2). Longitudinal arterial profiles we

Protein Z levels and central retinal vein or artery occlusion.

OBJECTIVES: Central retinal vein occlusion (CRVO) and central retinal artery occlusion (CRAO) are common disorders associated with risk factors for atherosclerosis. Protein Z is a cofactor for the inactivation of activated factor X (Xa) by the protein Z dependent protease inhibitor. Protein Z deficiency was recently linked to increased risk of arterial thrombosis. We investigated whether CRVO and CRAO are associated with low protein Z levels. PATIENTS AND METHODS: Patients with CRVO, CRAO or recurrent branch retinal vein occlusion were recruited to the study. Protein Z level, lupus anticoagul…

Central retinal artery occlusion in a patient with pseudoxanthoma elasticum.

We examined a patient with central retinal artery occlusion (CRAO) who was diagnosed as having pseudoxanthoma elasticum and exhibited angioid streaks. Echocardiography revealed stenosis and plaques of the right carotid artery. Magnetic resonance imaging (MRI) showed multiple cerebral infarctions, which were considered to be the result of vascular-endothelial abnormalities associated with pseudoxanthoma elasticum. Systemic examination of any plaque which may cause CRAO is recommended.

Endothelin-1-mediated retinal artery vasospasm and the rabbit electroretinogram.

Electroretinogram (ERG) changes invariably accompany the selective interruption of the retinal circulation that occurs in human central retinal artery occlusion. Since arteriolar ligation or ocular hypertension in the rabbit eye is occasionally used to model human central retinal artery occlusion, we conducted the present study to determine whether selective interruption of the retinal circulation of the rabbit eye alters retinal function as measured by the ERG. The vasoconstrictor, endothelin-1, was injected into the vitreous of rabbits' eyes to induce complete vasospasm and selective interru…

Spontaneous central retinal artery occlusion in hemoglobin sickle cell disease.

PURPOSE: To describe a case of spontaneous central retinal artery occlusion in a young man with hemoglobin sickle cell disease. METHOD: Case report. RESULTS: A 31-year-old African-American man with a history of hemoglobin sickle cell disease developed sudden painless loss of vision in the right eye. Medical history was remarkable for the recent history of a mild painful crisis, but no other systemic illness or contributing factors. Central retinal artery occlusion was diagnosed with retinal whitening, cherry red spot, and delayed arteriovenous transit on fluorescein angiography. Over the ens…

Branch retinal artery occlusion in systemic diseases: a case report.

Although branch retinal artery occlusion (BRAO) is a relatively benign disease in terms of permanent visual impairment, the associated systemic diseases confer significant morbidity and mortality. The following systemic disorders can be found among patients presenting with a retinal artery occlusion: hypertension (59%), significant atherosclerotic cardiovascular disease (21%), diabetes mellitus (15% to 21%), left-sided valvular heart disease (5%), and cerebrovascular accidents (5%). These underlying systemic diseases are often responsible for a significant reduction in life expectancy and are …

[The relation of retinal vascular occlusion and carotid artery stenosis].

We evaluated carotid ulceration and stenosis in 16 patients with central retinal artery occlusion (CRAO), 13 patients with branch retinal artery occlusion (BRAO), and 27 patients with central retinal vein occlusion (CRVO). One of three kinds of angiography was selected according to the patient's condition. These included conventional cerebral angiography, intravenous digital subtraction angiography (IVDSA), and magnetic resonance angiography (MRA). Carotid artery stenosis (50% or greater) was noted in 37.5% (6/16) of the patients with CRAO and in 23.1% (3/13) of the patients with BRAO. Only on…

Central retinal artery occlusion after back surgery: a case report.

Central retinal artery occlusion can be caused by excessive extrinsic pressure on the eyeball during surgery. It is a well-known and severe complication after elective back surgery. The authors report a case of central retinal artery occlusion in an adult after posterior lumbar fusion in which a rectangular headrest was used.
