A robust deep learning classifier for screening multiple retinal diseases on optical coherence tomography.

Retinal diseases are among the leading causes of visual impairment worldwide, where timely diagnosis and management are critical to prevent irreversible vision loss and blindness, especially in regions with limited access to ophthalmologists. While artificial intelligence (AI) has shown remarkable potential for screening ocular diseases, many existing models are heavily dependent on the datasets used for their development and often experience significant performance drops when tested on external datasets. These limitations in robustness and generalizability reduce their practical applicability…

When less is more: the role of non-vitrectomized vitreous surgery in retinal diseases.

Anatomically, the vitreous is not merely a transparent medium; it is a complex gel structure crucial for maintaining intraocular architecture and biochemical homeostasis. By minimizing disturbance to the vitreous, non-vitrectomized vitreous Surgery (NVS) which involves surgery with no or minimal removal of the vitreous gel, attempts to achieve the indication of the surgery while preserving the vitreous’ physiological functions, such as buffering intraocular oxygen tension and limiting the influx of inflammatory cells. This selective approach is especially relevant in younger patients with clea

Development and standardisation of 'time-in-range' measurement for clinical endpoints in retinal diseases.

Clear and updated endpoints are required to measure outcomes of a disease course and/or a therapeutic intervention. The aim of this review i … results showed that both functional and morphological endpoints have been used in the evaluation of retinal diseases. At present, the most widely accepted and clinically meaningful marker of ocular disease is 'mean change' in best corrected visual acuity (BCVA). While comparisons to baseline at various timepoints are commonly recommended to evaluate statistical an …

Preventive and Protective Effects of Nicotinamide Adenine Dinucleotide Boosters in Aging and Retinal Diseases.

Nicotinamide adenine dinucleotide (NAD+) boosting can sustain energy metabolism and neurovascular stability in the retinal tissue. Depletion of NAD+ is linked to the development of pathological retinal conditions, such as age-related macular degeneration (AMD) and diabetic retinopathy (DR). Mitochondrial dysfunction, oxidative stress, and inflammation occur in these diseases. This review summarizes substantial evidence of therapeutic NAD+ boosters, including nicotinamide, nicotinamide mononucleotide, or nicotinamide riboside. They help improve mitochondrial function and lessen neurovascular in

Using retinal diagnostics as a biomarker for neurodegenerative diseases: protocol for a systematic review.

INTRODUCTION: Retinal neurodegeneration has recently been shown to occur in tandem with neurodegenerative disease. In the expectation that disease-modifying treatments for Alzheimer's disease (AD) and Parkinson's disease (PD) will soon become available, it will be important to have clinically useful biomarkers for neurodegenerative disease subtyping to guide early diagnosis, inform on prognosis and stratify subgroups for treatment. Understanding differences in detectable retina changes in individuals with different neurodegenerative disease subtypes is therefore fundamental. The emerging field

Optogenetic tools and their applications for therapeutic intervention in end-stage inherited retinal diseases.

Retinal degenerative diseases pose significant therapeutic challenges; however, optogenetic vision restoration offers a promising strategy that is independent of mutations and disease progression. Optogenetic tools consisting of photosensitive membrane proteins can be ectopically expressed in retinal cells, effectively converting them into artificial photoreceptors. This review outlines the development and application of optogenetic tools to restore light sensitivity in degenerated retinas. A thorough understanding of retinal cell morphology, pathological remodeling, and functional loss is cri

Genetic characterization of 1210 Japanese pedigrees with inherited retinal diseases by whole-exome sequencing.

Inherited retinal diseases (IRDs) comprise a phenotypically and genetically heterogeneous group of ocular disorders that cause visual loss via progressive retinal degeneration. Here, we report the genetic characterization of 1210 IRD pedigrees enrolled through the Japan Eye Genetic Consortium and analyzed by whole exome sequencing. The most common phenotype was retinitis pigmentosa (RP, 43%), followed by macular dystrophy/cone- or cone-rod dystrophy (MD/CORD, 13%). In total, 67 causal genes were identified in 37% (448/1210) of the pedigrees. The first and second most frequently mutated genes w…

Role of Retinal Amyloid-β in Neurodegenerative Diseases: Overlapping Mechanisms and Emerging Clinical Applications.

Amyloid-β (Aβ) accumulations have been identified in the retina for neurodegeneration-associated disorders like Alzheimer's disease (AD), glaucoma, and age-related macular degeneration (AMD). Elevated retinal Aβ levels were associated with progressive retinal neurodegeneration, elevated cerebral Aβ accumulation, and increased disease severity with a decline in cognition and vision. Retinal Aβ accumulation and its pathological effects were demonstrated to occur prior to irreversible neurodegeneration, which highlights its potential in early disease detection and intervention. Using the retina a

Hydrogen Sulfide: Novel Endogenous and Exogenous Modulator of Oxidative Stress in Retinal Degeneration Diseases.

Oxidative stress (OS) damage can cause significant injury to cells, which is related to the occurrence and development of many diseases. This pathological process is considered to be the first step to trigger the death of outer retinal neurons, which is related to the pathology of retinal degenerative diseases. Hydrogen sulfide (H2S) has recently received widespread attention as a physiological signal molecule and gas neuromodulator and plays an important role in regulating OS in eyes. In this article, we reviewed the OS responses and regulatory mechanisms of H2S and its donors as endogenous a

Ubiquitous Chromatin Modifiers in Congenital Retinal Diseases: Implications for Disease Modeling and Regenerative Medicine.

Retinal congenital malformations known as microphthalmia, anophthalmia, and coloboma (MAC) are associated with alterations in genes encoding epigenetic proteins that modify chromatin. We review newly discovered functions of such chromatin modifiers in retinal development and discuss the role of epigenetics in MAC in humans and animal models. Further, we highlight how advances in epigenomic technologies provide foundational and regenerative medicine-related insights into blinding disorders. Combining knowledge of epigenetics and pluripotent stem cells (PSCs) is a promising avenue because epigen

The role of reactive oxygen species in the pathogenesis and treatment of retinal diseases.

Reactive oxygen species (ROS) normally play an important physiological role in health regulating cellular processes and signal transduction. The amount of ROS is usually kept in fine balance with the generation of ROS largely being offset by the body's antioxidants. A tipping of this balance has increasingly been recognised as a contributor to human disease. The retina, as a result of its cellular anatomy and physical location, is a potent generator of ROS that has been linked to several major retinal diseases. This review will provide a summary of the role of oxidative stress in the pathogene…
