An elderly case of idiopathic retroperitoneal fibrosis (IRPF) with bilateral ureteral obstruction was reported. A 74-year-old man was admitted with complaints of general fatigue and loss of appetite. An elastic hard mass with a smooth surface was palpated in the left upper quadrant of the abdomen. Abnormal ESR and CRP were noticed. Retroperitoneal fibrosis rarely affects children. Its clinical manifestations are protean, but if recognized early and properly treated, the prognosis is generally good. This is the report of a 14-year-old girl with Turner's syndrome and retroperitoneal fibrosis.

One of the less common associations of retroperitoneal fibrosis is malignancy. Although there have been many reports on tumors associated with retroperitoneal fibrosis urothelial tumors are rare. We report a case of retroperitoneal fibrosis and urothelial malignancy of the renal pelvis.

Based on retrospective analysis of 430 cases of retroperitoneal fibrosis, a classification into various etiological groups and an evaluation of the best diagnostic approach and treatment was possible. In patients presenting moderate upper urinary dilation corticosteroids alone proved to be of value in 93% of cases. Azathioprin given in combination with corticosteroids seems to be effective. We report the findings on computed tomography (CT) in idiopathic and secondary retroperitoneal fibrosis in 7 patients. In all cases CT showed the fibrotic mass, accurately defining its extent and the involvement of adjacent anatomic structures. The densitometric and anatomopathologic aspects were correlated; contrast uptake was less in areas thought to be dense in collagen than it was in areas with more vascular regions of inflammatory change. CT assessed the extent and nature of the fibrotic plaque.

We evaluated 4 patients with surgically proved retroperitoneal fibrosis by computerized tomography and ultrasound. Retroperitoneal fibrosis was diagnosed correctly in all 4 patients by computerized tomography but was detected by ultrasound in only 1. Retroperitoneal fibrosis generally occurs above the pelvic brim and is rarely encountered by the gynecologist. In the case presented, the fibrosis first became manifest in the pelvis and simulated metastatic carcinoma of the cervix. Treatment with cyclophosphamide and prednisone produced an apparent remission.
