[The surgical treatment of retroperitoneal phlegmons].

Based on the experience with treatment of 112 patients with retroperitoneal phlegmons of different etiology the authors distinguish and describe the diffuse and complicated forms of the disease. In treatment of phlegmons special importance is attached to the optimal surgical access. The article describes main principles of performing sanitation of the suppurative foci of the retroperitoneal space, variants of the aspiration-irrigation methods of treatment of the suppurative process. The authors believe that their original differentiated surgical method using different variants of the aspiratio…

Atypical retroperitoneal extension of iliopsoas bursitis.

We report two rare cases of iliopsoas bursitis extending into the retroperitoneal space. The first lesion contained much gas, mimicking a retroperitoneal abscess, and the second was responsible for atypical inguinal pain. The diagnosis was made by contrast-enhanced CT in both cases and arthrography in the first case. Iliopsoas bursitis in these two patients, it is hypothesized, extended into the retroperitoneum, at least in part, by way of intraneural or perineural structures.

Anatomy of the retroperitoneum: observations of the distribution of pathologic fluid collections.

PURPOSE: To correlate anatomic dissections with clinical observations regarding anatomic distribution of retroperitoneal fluid, and to document the existence of planes that lie between classically described retroperitoneal spaces. MATERIALS AND METHODS: Latex was injected in varying amounts into the pancreatic tail in three fresh cadavers to simulate peripancreatic fluid collections. Spiral computed tomography (CT) was performed of the abdomen and pelvis after each latex injection. Two cadavers were subsequently frozen and sectioned in axial planes; limited dissections were performed on these…

Difficulties of diagnosis in retroperitoneal tumors.

Retroperitoneum is a large space where the primary and metastatic tumors grow silently before clinical signs appear. Retroperitoneal sarcomas represent 10% to 15% of all soft tissue sarcomas. This group, retroperitoneal soft tissue sarcomas is associated with a very poor long-term survival rate, difficulty in diagnosis and complete surgical removal with a rim of normal tissue around the tumors. Liposarcoma is the most frequent retroperitoneal sarcoma. MFH is the second most common retroperitoneal sarcoma. The diagnosis in retroperitoneal sarcomas is clinical, radiological and histological. Total resection of a giant retroperitoneal and mediastinal ganglioneuroma-case report and systematic review of the literature.

BACKGROUND: Ganglioneuromas (GNs) are extremely rare, slowly growing, benign tumors that can arise from Schwann cells, ganglion cells, and neuronal or fibrous tissues. Due to their origin from the sympathetic neural crest, they show neuroendocrine potential; however, most are reported to be hormonally inactive. Nevertheless, complete surgical removal is recommended for symptom control or for the prevention of potential malignant degeneration. CASE REPORT: A 30-year-old female was referred to our oncologic center due to a giant retroperitoneal and mediastinal mass detected in computed tomograp

Retroperitoneal liposarcoma excreting insulin-like growth factor 2 that induced severe hypoglycemia.

Insulin-like growth factor 2 is overexpressed in various cancers, and is associated with a poor prognosis. Also, it is known that insulin-like growth factor 2 is an etiology of non-islet cell tumor hypoglycemia. In this report, we describe a case of unexpected hypoglycemia caused by a dedifferentiated liposarcoma producing insulin-like growth factor 2. A large mass in the retroperitoneum was detected in a 61-year-old man who complained of appetite loss. Despite having no history of diabetes mellitus, hypoglycemia suddenly occurred after admission, but oral glucose therapy was ineffective. Afte

Coxalgia? Call the gynecologist.

Cellular angiofibroma is a rare mesenchymal tumor that usually affects the vulvar region. It is commonly asymptomatic and treated with radical excision. We describe the first case of cellular angiofibroma located in the retroperitoneum presenting with coxalgia and laparoscopically treated. At the 6-month follow-up, the patient was coxalgia and disease free. Coxalgia is an unusual symptom for gynecologists; nevertheless, it is possible that several neoplasms may originate in the female pelvis, so any symptom requires careful evaluation. The laparoscopic approach may be effective in removing cel

Radiologically guided percutaneous fine-needle aspiration biopsy of pelvic and retroperitoneal masses: a retrospective study of 68 cases.

Fine-needle aspiration (FNA) of the pelvis and retroperitoneum (excluding the pancreas, kidney, and adrenal masses) has not achieved its full potential as a diagnostic modality. We reviewed 68 percutaneous, radiologically guided FNAs from these locations to assess the clinical utility and complication rate of this procedure. Satisfactory material was obtained in 66 cases (97.1%), of which 37 were deemed positive (55%), 3 suspicious (4%), 4 atypical (6%), and 22 negative (32%) for malignancy; two cases (3%) were unsatisfactory. Compared to biopsy (36 patients) and clinical information, the sens

[A case of retroperitoneal pseudotumor (xanthogranuloma)].

A 57-year-old male patient was admitted to our hospital for left flank pain and a slight increase in serum creatinine. He had left hydronephrosis and extrinsic stenosis of left lower ureter shown by intravenous and retrograde pyelography. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated a mass along the bilateral lower ureters, the left side of which was larger (5 x 3 x 4 cm). After surgical exploration and biopsy of the tumor the patient was diagnosed as having xanthogranulomatous inflammatory pseudotumor. No malignant findings were observed. Tumor size spontaneously

Medical therapy in idiopathic retroperitoneal fibrosis.

Except for its linkage with the ergot derivatives retroperitoneal fibrosis is a disease of unknown etiology. Two cases in which the traditional surgical approach was used but with the subsequent use of steroids are presented. The third patient was treated with steroids alone. The use of steroids alone is suggested for a limited number of poor risk patients when certain criteria are met.

Left Compartmentectomy for Adult-Type Granulosa Tumor of the Retroperitoneum: A Case Report and Review of the Literature.

Granulosa tumors are rare tumors arising from the cells of the sexual cord and stroma of the ovary. They account for 5% of ovarian cancers and 70% of stromal cancers of the sex cords. Retroperitoneal tumors (RPTs) are also rare and develop in the retroperitoneal and subperitoneal space. They are usually diagnosed late and are dominated by sarcomas and around 10% of teratomas. Based on principles derived from limb sarcoma surgery, compartmentectomy is currently the approach recommended by experts for the surgical removal of soft tissue sarcomas of the retroperitoneum. A primary retroperitoneal

Overview of primary adult retroperitoneal tumours.

In front of a primary retroperitoneal tumour, it is necessary to have in mind all possible diagnoses in order to specify the diagnostic stra … noma metastasis, and adenopathy are excluded from this definition, but they can also develop in the retroperitoneal space and lead to misdiagnoses. In the absence of positive tumour markers or an evocative biology, percutaneous biopsy is necessary. Pathological diagnosis is necessary to decide whether surgery must be done, its timing among the other treatments, and its …

[Idiopathic retroperitoneal fibrosis (Ormond's disease)].

INTRODUCTION: Idiopathic retroperitoneal fibrosis (RPF) represents a rare inflammatory disease, which leads to extensive fibrosis of the retroperitoneal space. In the course of the progressive fibrosis, fibrous tissue compresses the retroperitoneal structures with the development of consecutive ureteral obstruction. Because of the unknown aetiology, no consensus between conservative and surgical treatment exists. CASE REPORT: A 60-year-old patient was admitted to hospital with left-sided flank pain, hydronephrosis, and retroperitoneal tumour. A CT scan-guided biopsy revealed RPF. The hydronep…

Pelvic confined idiopathic retroperitoneal fibrosis mimicking a large tumor.

Idiopathic retroperitoneal fibrosis (IRF) entirely confined to the pelvic cavity is an extremely rare clinical entity. Herein, is described the case of a 36- year old male who presented with clinical and imaging manifestations of a large pelvic tumor. Exploratory laparotomy revealed a large mass in the right pelvis originating from the retroperitoneal space, displacing the right iliac vessels, the right ureter and the urinary bladder completely to the left. A laborious resection of the mass measuring 14 x 10cm was performed. Histopathological examination and detailed immunohistochemistry analy…

Retroperitoneal liposarcoma: MR characteristics and pathological correlative analysis.

BACKGROUND: Liposarcoma is the most common primary retroperitoneal malignant neoplasm. However, preoperative diagnosis is a common problem due to lack of characteristic clinical presentations. It has been assumed that MRI, based on its high soft-tissue resolution, could discover and discern different subtypes of this tumor. Moreover, there has been little in the literature to compare the MRI features with pathological appearances of the retroperitoneal liposarcomas. METHODS: We retrospectively analyzed 19 cases of retroperitoneal liposarcoma (11 males and 8 females, aged 41-79 years) proved s
