Mediastinitis and Bronchial Perforations Due to Mucormycosis.

Diabetes mellitus is the most common predisposing condition for mucormycosis, which is emerging as an important invasive fungal infection worldwide. Isolated mediastinitis is a very rare presentation of mucormycosis. A 57-year-old woman with uncontrolled type 2 diabetes mellitus and ketoacidosis presented with septic shock and was subsequently found to have mucor mediastinitis with multiple bronchial perforations. The organism was identified as Rhizopus oryzae with the help of DNA sequencing.

Vesicular eruption on the arm of an infant.

Cutaneous mucormycosis is an uncommon disease and occurs mainly in immunocompromised patients. We report an immunocompetent infant who developed primary cutaneous mucormycosis at an intravenous line site secured with an arm board and elastic bandage. The isolate was identified as the zygomycete, Rhizopus sp. High mortality rates are reported with invasive mucormycosis; however, early identification of the causative agent and antifungal therapy led to complete cure of the lesions in the reported case.

Successful treatment of rhinocerebral zygomycosis with a combination of caspofungin and liposomal amphotericin B.

Genera of the order Mucorales (Rhizopus, Mucor, Rhizomucor, Absidia, Apophysomyces, Cunninghamella, and Saksenaea) cause an angioinvasive infection called zygomycosis. Mortality rates can approach 100% depending on the patient's underlying disease and form of zygomycosis. We report here on the unusual case of a patient with acute myelogenous leukemia and zygomycosis unresponsive to monotherapy with liposomal amphotericin B, who responded favorably following the addition of the echinocandin caspofungin acetate.

Primary cutaneous mucormycosis in a trauma patient.

We report a rare case of primary cutaneous mucormycosis caused by Rhizopus oryzae that occurred in an immunocompetent trauma patient. The patient had encrusted erythematous plaques with pustules on the left shin, which had been abraded in a traffic accident. Histologic examination revealed widespread granulomatous inflammation and characteristic broad, non-septate hyphae with right-angle branching in the dermis. The infection was cured with intravenous amphotericin B therapy.

Identification of potential anti-mucor agents by targeting endothelial cell receptor glucose-regulated protein-78 using in silico approach.

Mucormycosis is a fungal infection of the sinuses, brain and lungs that is the cause of approximately 50% mortality rate despite the available first-line therapy. Glucose-Regulated Protein 78 (GRP78) is already reported to be a novel host receptor that mediates invasion and damage of human endothelial cells by Rhizopus oryzae and Rhizopus delemar, the most common etiologic species of Mucorales. The expression of GRP78 is also regulated by the levels of iron and glucose in the blood. There are several antifungal drugs in the market but they pose a serious side effect to the vital organs of the …

In vitro interaction of caspofungin and immunosuppressives against agents of mucormycosis.

OBJECTIVES: Mucormycosis is a rare but refractory mycosis with high mortality. Few therapeutic options are available and novel strategies are needed. Calcineurin inhibitors are known to have antifungal activity, including synergy with various antifungals. We examined the interaction of caspofungin with calcineurin inhibitors and sirolimus against Glomeromycetes. METHODS: Twenty-six strains of Glomeromycetes representing seven species (Rhizopus arrhizus, Rhizopus microsporus, Mucor sp., Rhizomucor pusillus, Cunninghamella berthollettiae, Mycocladus corymbifera and Apophysomyces elegans) were s…

A case of invasive pulmonary mucormycosis resulting from short courses of corticosteroids in a well-controlled diabetic patient.

A patient with well-controlled type 2 diabetes mellitus developed a severe pulmonary infection secondary to Rhizopus spp. after receiving short courses of corticosteroids for a respiratory tract infection. He recovered after an aggressive surgical intervention and treatment with isavuconazole. Patients on chronic corticosteroid therapy have a higher risk for pulmonary mucormycosis, but there are much fewer reports of mucormycosis occurring in patients after only short courses of steroid therapy.

A 39-Year-Old Male Congenital Tricuspid Atresia Patient Who Presented with a New Axillary Lesion after an Orthotopic Heart Transplant.

Mucormycosis is a rare and aggressive fungal infection, most often caused by species of the Mucor, Rhizomucor, Rhizopus, Absidia, and Cunninghamella genera. The condition most commonly affects patients with uncontrolled diabetes, HIV/AIDS, malignancy, and those receiving long-term immunosuppressive therapy. We report the case of a 39-year-old male with biopsy-proven cutaneous mucormycosis of the left axilla 4 months after an orthotopic heart transplant for congenital tricuspid atresia.

Zygomycosis in ostriches.

An unusual disease characterized by anorexia, progressive stunting and stomach impaction was observed in young ostriches after treatment with antibiotics. The massive accumulation of a clear white gelatinous mucus adherent to the inner surface of the ventriculus plus a severe necrosis of the glandular layer were the only consistent findings. Histological examination revealed typical forms of a zygomycete invading the affected tissue. The zygomycete Rhizopus oryzae was isolated from the ventriculus of all the affected ostriches.

Cutaneous mucormycosis in systemic lupus erythematosus: a case report.

Mucormycosis is a life-threatening angioinvasive fungal infection caused by the Mucorales acidophilic fungi. In the article we report a case of cutaneous mucosporidiosis caused by infection with Rhizopus arrhizus in a patient with systemic lupus erythematosus (SLE). We analyze the causes of infection and death and concluded that cutaneous mucormycosis is a severe and refractory infection that has higher mortality in immunocompromised individuals or those with immune dysfunction. Early diagnosis and proactive treatment are paramount for a favorable prognosis.

Self-resolving superficial primary cutaneous mucormycosis in a 7-week-old infant.

A 7-week-old girl, born at 30 weeks' gestational age, presented to clinic for evaluation of a crop of vesicular lesions that were noted after removal of a bandage that had been in place for 4 days. A punch biopsy of the lesion revealed fungal elements that were later identified as Rhizopus spp. The lesion began to self-resolve, and no further treatment was needed, with full resolution of the lesion by 1 month after presentation. Clinicians should be aware of the variable presentations of mucormycosis and consider fungal infection in the differential diagnosis when evaluating vulnerable patient…

Cutaneous zygomycosis in a patient with lymphoma.

A large variety of underlying conditions and diseases have been associated with zygomycosis. The most common ones include leukemia or lymphoma, burns, renal failure, septicemia, malnutrition, diabetes, cancer, chemotherapy, organ transplantation, and corticosteroid administration. While being ubiquitous in the environment and relatively harmless to healthy individuals, Zygomycetes can be fatal in the immunocompromised patient, especially if untreated or if treatment is delayed.
