Traumatic first rib fracture: is angiography necessary? A review of 730 cases.

The two most common sites of first rib fracture were at the subclavian sulcus and in the neck of the first rib, posteriorly. Five distinct mechanisms for rib fracture were identified and included: (i) posteriorly directed trauma to the upper thorax or shoulder girdle; (ii) a direct blow to the sternum and anterior chest wall; (iii) a blow fracturing the clavicle; (iv) a strong sudden contraction of the scalenus anticus muscle; and (v) radiographic findings of a first rib fracture without history of trauma. Isolated first rib fracture regardless of mechanism of injury, results in a low incidenc

Fractures of first and second ribs: predictive value for arterial and bronchial injury.

Fractures of the first and second ribs had a very low association with ruptured bronchus (2%) and ruptured aorta or brachiocephalic vessel (8%) in a series of 50 patients. Radiographic abnormalities indicative of ruptured bronchus include massive pneumothorax, pneumomediastinum, subcutaneous emphysema, and lobar or whole lung collapse which may fall to the dependent part of the thorax on erect views. Abnormalities pointing to a laceration of the aorta or one of the brachiocephalic vessels include widening of the superior mediastinum, shift of the trachea or nasogastric tube to the right, enlar

Adult Presentation of a Large Congenital Pulmonary Airway Malformation Discovered After Traumatic Rib Fractures.

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung lesion that is usually identified prenatally or in early childhood, and adult presentations are uncommon. A 44-year-old woman presented after a fall with right-sided chest pain and dyspnea. CT chest revealed nondisplaced fractures of the right seventh through ninth ribs and incidentally identified a large multilocular cystic lesion in the right lower lobe consistent with CPAM, unchanged from imaging performed 15 years earlier. Pulmonary function testing demonstrated mild small-airway dysfunction, air trapping, and red

Association of four or more rib fractures with complications in isolated blunt chest trauma.

INTRODUCTION: Rib fractures are common in blunt chest trauma and are associated with significant morbidity. Although increasing rib fracture count has been linked to worse outcomes, a clear and clinically applicable threshold for complication risk remains uncertain. This study aimed to evaluate the predictive value of rib fracture burden in patients with isolated blunt chest trauma and to identify a clinically meaningful cut-off. METHODS: This retrospective observational study included adult patients with isolated blunt chest trauma presenting between January 2021 and December 2025 at a terti…

Video-assisted thoracoscopic surgery surgical stabilization of rib fractures: surgical technique and case report.

Rib fractures are a common consequence of chest trauma. In some instances, the presence of rib fractures may cause chest instability, leading to respiratory failure, dependance from ventilatory support and increased risk of pulmonary infections. Surgical stabilization of rib fractures (SSRF) is a well-established treatment to restore the stability of the rib cage and prevent respiratory complications. While this procedure is commonly performed through an open approach that allows a wide exposure of the ribs surface, in recent years a minimally invasive procedure consisting in thoracoscopic-ass…

Current and Emerging Techniques for Surgical Stabilisation of Rib Fractures: A Systematic Scoping Review.

BACKGROUND: Surgical stabilisation of rib fractures in select patients is shown to significantly improve outcomes. However, there is considerable variation in surgical techniques necessitating a comprehensive analysis of current practices. METHODS: The MEDLINE, EMBASE, Cochrane Library Compendex, Proquest, Scopus, and IEEE databases were systematically searched for studies across four time periods P1(2000-2006), P2(2007-2011), P3(2012-2016), and P4(2017-2021). SSRF Techniques of fixation were assessed and classified by design principles. Variation in approach, indications, and postoperative c…

Comparing How Three Frailty Scales Predict Negative Outcomes in Trauma Patients With Rib Fractures.

INTRODUCTION: Frailty is a risk factor for adverse outcomes after injury. Herein, we compared three frailty scales: the Canadian Study of Health and Aging clinical frailty scale, the rib fracture frailty index (RFFI) and the modified frailty index-5, to assess which scale is most applicable in predicting risk for negative outcomes in older patients with rib fractures. METHODS: Patients ≥65 admitted for rib fractures were retrospectively scored for frailty using the RFFI, Canadian Study of Health and Aging clinical frailty scale, and modified frailty index-5. Outcomes examined were in-hospital…

Aortic Laceration From Posterior Rib Fractures After a Ground-Level Fall: A Case Report.

BACKGROUND: Rib fractures represent a common injury after blunt chest wall trauma with known complications including pneumothorax, hemothorax, pulmonary contusion, and pneumonia. This case report describes an emergency department patient with acute decompensation from aortic laceration as a rare complication of rib fractures. There are rare documented cases of this complication occurring in admitted patients with rib fractures, but this is one of the only cases that describes this complication occurring in a patient presenting to the emergency department. CASE REPORT: This case describes a pa…

Implementation of a Modified Pain, Inspiration, Cough Protocol in Patients With Traumatic Rib Fractures.

INTRODUCTION: Patients with blunt chest wall injuries and rib fractures are known to have high rates of atelectasis, pneumonia, pulmonary contusion, and can develop acute respiratory distress syndrome. This can lead to ventilator requirement and dependence, deconditioning secondary to uncontrolled pain, and increased hospital length of stay (LOS). Many studies in the literature have developed triage algorithms in patients with rib fractures to guide disposition and management, and several institutions have gone on to describe their institution-specific management protocols to decrease complica…

Investigating the link between frailty and outcomes in geriatric patients with isolated rib fractures.

BACKGROUND: Studies have shown an increased risk of morbidity in elderly patients suffering rib fractures from blunt trauma. The association between frailty and rib fractures on adverse outcomes is still ill-defined. In the current investigation, we sought to delineate the association between frailty, measured using the Orthopedic Frailty Score (OFS), and outcomes in geriatric patients with isolated rib fractures. METHODS: All geriatric (aged 65 years or older) patients registered in the 2013-2019 Trauma Quality Improvement database with a conservatively managed isolated rib fracture were con…

Ultrasound-guided thoracic nerve blocks for emergency department patients with rib fractures: A review.

BACKGROUND: Rib fractures are frequently diagnosed and treated in the emergency department (ED). Thoracic trauma has serious morbidity and mortality, particularly in older adults, with complications including pulmonary contusions, hemorrhage, pneumonia, or death. Bedside ED-performed ultrasound-guided anesthesia is gaining in popularity, and early and adequate pain control has shown improved patient outcomes with rare complications. OBJECTIVE OF THE REVIEW: We describe thoracic nerve block options that can be used clinically for rib fractures or other thoracic pain or trauma. We identify the …

Cough-Induced Multiple, Bilateral, Asymmetrical Rib Fractures in a Scoliosis Patient: A Case Report.

Cough-induced rib fractures represent infrequent complications of strenuous and prolonged coughing, mostly provoked by respiratory tract infections, with localized chest pain being the most indicative component of the clinical picture. This paper reports a case of a 27-year-old female patient who presented with four cough-induced rib fractures following the contraction of an upper respiratory tract infection. The unique character of this case is provided by the young age of the patient, the presence of multiple and bilaterally located rib fractures, and the absence of predisposing factors rela…

Scalable deep learning algorithm to compute percent pulmonary contusion among patients with rib fractures.

BACKGROUND: Pulmonary contusion exists along a spectrum of severity, yet is commonly binarily classified as present or absent. We aimed to develop a deep learning algorithm to automate percent pulmonary contusion computation and exemplify how transfer learning could facilitate large-scale validation. We hypothesized that our deep learning algorithm could automate percent pulmonary contusion computation and that greater percent contusion would be associated with higher odds of adverse inpatient outcomes among patients with rib fractures. METHODS: We evaluated admission-day chest computed tomog…

Comparison of Infectious Complications after Surgical Fixation versus Epidural Analgesia for Acute Rib Fractures.

Background: Surgical stabilization of rib fractures (SSRF) is associated with decreased mortality and respiratory complications. Patients who are not offered SSRF are often treated with epidural analgesia (EA) to reduce pain and improve pulmonary mechanics. We sought to compare infectious complications in patients undergoing either SSRF or EA. We hypothesized that infectious complications are equivalent between the two treatment groups. Patients and Methods: We performed a retrospective cohort study of adult trauma patients with acute rib fractures within the Trauma Quality Improvement Program…

Comparison of surgical stabilization of rib fractures vs epidural analgesia on in-hospital outcomes.

INTRODUCTION: Surgical stabilization of rib fractures (SSRF) improves functional outcomes compared to controls, partly due to reduction in pain. We investigated the impact of early SSRF on pulmonary complications, mortality, and length of stay compared to non-operative analgesia with epidural analgesia (EA). METHODS: Retrospective cohort study of the Trauma Quality Improvement Program (TQIP) 2017 dataset for adults with rib fractures, excluding those with traumatic brain injury or death within twenty-four hours. Early SSRF and EA occurred within 72 h, and we excluded those who received both o…

The Chinese consensus for surgical treatment of traumatic rib fractures 2021 (C-STTRF 2021).

Rib fracture is the most common injury in chest trauma. Most of patients with rib fractures were treated conservatively, but up to 50% of patients, especially those with combined injury such as flail chest, presented chronic pain or chest wall deformities, and more than 30% had long-term disabilities, unable to retain a full-time job. In the past two decades, surgery for rib fractures has achieving good outcomes. However, in clinic, there are still some problems including inconsistency in surgical indications and quality control in medical services. Before the year of 2018, there were 3 guidel…
