Novice nurses' sleep disturbance trajectories within the first 2 years of work and actual turnover: A prospective longitudinal study.

BACKGROUND: Novice nurses experience radical changes in their sleep-wake cycle and sleep difficulties after exposure to a rotating shift work schedule. The link between workplace hardship and employee turnover may be dependent on factors at the individual level, e.g., individual health. Nurses with sleep disturbance are more vulnerable to stressful stimuli and lack resilience at work compared with those without sleep disturbance. OBJECTIVES: To classify novice nurses according to sleep disturbance trajectories during the first 2 years of work, to compare turnover rates according to trajectory…

Comparative Analysis of Methods of Evaluating Human Fatigue.

The present study used four different methods to estimate fatigue. Forty-seven volunteers (45 men and 2 women), 41.3 ± 7.5 years old, truck operators for 11.5 ± 6.0 years, were included. All participants accepted the invitation to be included in the study. Actigraphy and core temperature were evaluated. The 5-minute psychomotor vigilance test, the Karolinksa Sleepiness Scale (KSS), and the postural assessment using the Light Sonometer™ (Belo Horizonte, Minas Gerais, Brazil) were performed. Fatigue prediction was performed using the Fatigue Avoidance Scheduling Tool (FAST) program. In response

Beyond radiation dosimetry: metabolic burden and a possible healthy worker survivor pattern in the medical imaging workforce.

OBJECTIVE: Current occupational health protocols in radiology remain strongly focused on ionizing radiation while underemphasizing metabolic health concerns potentially associated with sedentary and shift-based occupational environments. This study examined the prevalence of Non-Alcoholic Fatty Liver Disease (NAFLD) in medical imaging professionals and evaluated whether the observed tenure-related pattern was more consistent with early-career occupational burden and possible survivor-selection processes than with a simple linear cumulative-risk model. METHODS: A cross-sectional analysis was c

Circadian desynchrony in early life leads to enduring autistic-like behavioral changes in adulthood.

Circadian rhythm regulates a variety of biological processes in almost all living organisms. Modern lifestyles, e.g. transmeridian travel, night shift, light at night, etc., frequently disrupt people's regular sleep-wake cycles and create a misalignment (circadian desynchrony) between the natural environment and the endogenous body clock, and between different circadian oscillators within the body. The long-term consequences of circadian desynchrony on neurodevelopment and adult behavior remain elusive. Increasing clinical evidence supports a correlation between the disruption of the circadian

The Correlation Between Stress of Conscience and Burnout Among Health Care Personnel at an Acute Care Hospital in Southern Sweden: A Cross-Sectional Study.

INTRODUCTION: Turnover rates among health-care personnel are rising, which could jeopardise patient safety and the quality of care. One contributing factor to the high turnover is the prevalence of mental distress. Stress of conscience among healthcare personnel has been shown to affect them negatively, and feeling that they cannot provide the care their patients need increases their stress levels. Therefore, an increased understanding of stress of conscience, its relation to burnout and its consequences for healthcare can improve hospital care for both patients and staff. AIM: This study aim

The pharmacology of impulsive behaviour in rats II: the effects of amphetamine, haloperidol, imipramine, chlordiazepoxide and other drugs on fixed consecutive number schedules (FCN 8 and FCN 32).

The effects of drugs on one aspect of impulsive behaviour were evaluated using a schedule in which rats were trained to complete a fixed consecutive number of responses on one of two levers before pressing the second to obtain a reinforcer (FCN). Terminating the chain before completing the FCN resulted in the omission of the food, and can be considered an impulsive decision. Two groups of food-deprived rats were trained to press either 8 or 32 times on the left lever (FCN lever) of a two lever operant chamber before pressing the right lever (Reinforcement lever) to deliver a food pellet. Carotid intimal medial thickness in rotating night shift is related to IL1β/IL6 axis.

BACKGROUND AND AIMS: Sleep disturbances may promote glucose abnormalities and inflammatory burden among shift workers. Therefore, precocious subclinical atherosclerotic process might develop in healthy shift workers even without known metabolic and cardiovascular risk factors. METHODS AND RESULTS: We measured anthropometric parameters, glucose, lipids, inflammation and common carotid Intimal Medial Thickness (cIMT) in rotating-night shift workers (r-NSW, n = 88, age = 40.3 ± 7.8 y) in comparison with former-night shift workers (f-NSW, n = 35, age = 44.2 ± 6.4 y) and with day-only workers (DW,

Encapsulated Leptin-Producing Cells Facilitate Entrainment of Circadian Rhythms in Rodents and Nonhuman Primates.

Both the timing and composition of food intake have gained recognition for their effects on circadian (daily) rhythms in mammals, however the mechanisms by which these effects are exerted are incompletely understood. The present studies focus on synchronization of circadian rhythms to the environmental light-dark cycle, particularly re-entrainment following a phase shift such as occurs with travel across time zones or shift work. To examine the role and therapeutic potential of the peptide hormone leptin in these effects, we create and characterize injectable leptin factories consisting of enc

Amplitude reduction and phase shifts of melatonin, cortisol and other circadian rhythms after a gradual advance of sleep and light exposure in humans.

BACKGROUND: The phase and amplitude of rhythms in physiology and behavior are generated by circadian oscillators and entrained to the 24-h day by exposure to the light-dark cycle and feedback from the sleep-wake cycle. The extent to which the phase and amplitude of multiple rhythms are similarly affected during altered timing of light exposure and the sleep-wake cycle has not been fully characterized. METHODOLOGY/PRINCIPAL FINDINGS: We assessed the phase and amplitude of the rhythms of melatonin, core body temperature, cortisol, alertness, performance and sleep after a perturbation of entrain

Time of day, time of sleep, and time on task effects on sleepiness and cognitive performance of bus drivers.

PURPOSE: Optimal cognitive performance might prevent vehicle accidents. Identifying time-related circadian and homeostatic parameters having an impact on cognitive performance of drivers may be crucial to optimize drivers' performance. METHODS: In this prospective study conducted on bus drivers, two drivers alternated driving during a 24-h round trip and were accompanied by an interviewer. Each driver was tested using Karolinska Sleepiness Scale (KSS) and the reversed digit span Wechsler Working Memory test before the start of his shift and then every 6 h during a "work/driving" day. Is healthy sleep possible for professional firefighters? A comparison of "On-Duty" and "Off-Duty" sleep.

OBJECTIVES: Healthy sleep is vital for firefighter safety, health, wellness, and for public well-being. However, professional firefighters experience disturbed sleep at disproportionately high rates. The current study investigated firefighter sleep in terms of (1) differences in on-duty and off-duty sleep, (2) risk factors for poor sleep, and (3) whether risk factors differed for on-duty and off-duty sleep. METHODS: Professional firefighters from a large, urban fire department in central Virginia (N = 268) reported their sleep using the Pittsburgh Sleep Quality Index both on- and off-duty. Pa

Vaccine-preventable diseases in health care.

It cannot be overemphasized that good preventive measures such as universal precautions, good hand-washing techniques, and appropriate personal protection is the mainstay of reducing the risk of nosocomial infections to other patients and HCWs. Vaccines that are safe and effective in reducing risks of nosocomial infections must be promoted through educational campaigns and made easily available to all employees, including those on night and weekend shifts. Extended hours and onsite vaccination programs help to increase acceptance of vaccines at certain times, such as during the annual influenz

Long Working Hours and Risk of Nonalcoholic Fatty Liver Disease: Korea National Health and Nutrition Examination Survey VII.

BACKGROUND: The global incidence of NAFLD is rising sharply due to various risk factors. As previous studies reported adverse health impact of long working hours on metabolic diseases, such as diabetes mellitus and obesity, it is plausible that NAFLD is also associated with working excessive hours. However, data regarding this issue is limited. METHODS: In this cross-sectional study based on Korea National Health and Nutrition Examination Survey VII, 5,661 working adults without previous liver disease or heavy alcohol drinking habits were included. The subjects were categorized into three gro

A Personalized Recovery Intervention to Support Sleep Health in Emergency Healthcare Professionals.

Optimizing sleep health in healthcare professionals is a challenge, as misalignment between endogenous circadian rhythms and exogenous work schedules undermines recovery, well-being, and performance. This study evaluated a tailored recovery intervention based on cognitive-behavioural approaches for insomnia designed to support recovery to rotating night shifts. In a randomized controlled trial (REST), 72 emergency healthcare professionals were randomized to intervention or control. The intervention combined personalized sleep-wake feedback with stress management techniques. Sleep-wake rhythms
