A Case of Traumatic Aortic Transection Presenting With Hemorrhagic Shock.

BACKGROUND: Aortic transection, or aortic rupture, is a rare diagnosis in trauma patients and carries a high mortality. CASE REPORT: We present the case of a 61-year-old man presenting to a Level I trauma center after being struck by a motor vehicle, found to have an aortic transection. He was initially hypotensive and resuscitated with blood products due to concern for hemorrhagic shock. Aortic injury was suspected after chest x-ray study demonstrated a widened mediastinum. Traumatic thoracic aortic transection with pseudoaneurysm was diagnosed on computed tomography of the aorta, and the pa

The treatment of traumatic shock: recent advances and unresolved questions.

Uncontrolled bleeding remains a leading cause of potentially preventable death after trauma. Timely, adequate resuscitation in traumatic shock is an essential, lifesaving aspect of polytrauma care. Whilst basic principles in the treatment of traumatic shock remain the same-achieving hemorrhage control and replacing lost volume, the way this is achieved has changed significantly in the last five years. The abandonment of blood pressure driven uncontrolled fluid resuscitation, the introduction of the concept of hemostatic resuscitation, and the increasing use of massive transfusion protocols hav

[Application of Rheosorbilactum in the therapy of traumatic shock: comparative study].

The authors have studied influence of infusion of 800 ml RHEOSORBILACTUM, 500 ml VENOFUNDIN and 800 ml of 0,9% isotonic solution of sodium chloride with speed of 50-55 ml/mines on hemostasis, the maintenance of a liquid in the thorax, hemodynamic parameters in 60 patients with traumatic shock. It was revealed that the use of investigated volume of RHEOSORBILACTUM increases reliably (P<0,05) oxygen delivery for the account of the expressed haemodynamic effect which duration is less than during the use of VENOFUNDIN. It was revealed also that RHEOSORBILACTUM increases reliably (P<0,05) quantity

Androstenetriol improves survival in a rodent model of traumatic shock.

UNLABELLED: Trauma results in activation of the hypothalamic-pituitary-adrenal axis to mediate a cascade of neurohormonal changes as a defensive mechanism. Its prolongation, however, leads to a hypermetabolic, hypoperfused, and immunosuppressed state, setting the stage for subsequent sepsis and organ failure. Androstenetriol (5-androstene-3beta, 7beta, 17betatriol - AET), a metabolite of dehydroepiandrosterone, up-regulates the host immune response markedly, prevents immune suppression and controls inflammation, leading to improved survival after lethal infections by several diverse pathogens

[Manifestations of oxidative stress and its correction in traumatic shock].

The case study purpose was to investigate the oxidation-antioxidation process in the affected organism after traumatic shock and under the impact of Plapherone LB. A randomized experiment involved 94 patients who were shared between 2 groups: group 1--routine intensive therapy, group 2--it received additionally Plapherone LB (sublingually, 2 mg protein/day). Ten persons of the control group were investigated separately. It was found by the method of electron paramagnet resonance that the antixidant system became weaker (in patients with traumatic chock), the lipoid oxidants got more intensifie

[Mechanisms of the favorable effect of spinal anesthesia in traumatic shock].

Changes in the level of total peptidoleukotriens-pLT (LTC4 + LTD4 + LTE4) and inositol triphosphates (IP3) were analyzed in the thalamus of rats subjected to "tourniquet" trauma of both hind extremities (LD50) depending on the power of afferent nociceptive nerve impulses. Prevention of spreading, i.e., reduction of painful impulses from the damaged regions was achieved by the spinal anesthesia with bupivacain-chloride immediately after removal of the rubber bandage. Together with pLT and IP3 levels determined by radio immunoassay, physiologic parameters (mean arterial pressure, blood-brain bar

[The mechanism of action of opiate antagonists in experimental traumatic shock].

Acute experiments on 86 adult rabbits have shown that in the early period of Cannon's traumatic shock nalophine and naloxone relieve torpidity, restore motor activity and responses to standard stimuli. This prolongs the animal life (nalorphine) and increases the survivability (naloxone). However in some rabbits a premature relief of torpidity (0.5 mg/kg of nalorphine) precipitates the animal death. It is concluded that in the late period the drugs exert no effect on the shock outcome and that inactivation of opioid suppression system can either trigger the mechanism of shock withdrawal or prom

[Experience with using narcanti (naloxone) in patients with traumatic shock].

Twelve patients with traumatic shock caused by a mechanical trauma were treated with narcanti within 6 h after trauma with a single dose 0.4 mg, and then intravenously in drops (0.4 mg/100 ml of the sodium chloride solution) with the general dose 2.8 +/- 0.8 mg. It gave favourable hemodynamic and respiratory effects and contributed to stabilization of the indices of metabolism homeostasis.

[Therapeutic effect of nalorphine and naloxone in experimental traumatic shock].

In their acute experiments on 86 rabbits the authors have established that the intravenous administration of nalorphine and naloxone produces a marked therapeutic effect only during the early period of experimental traumatic shock. In most of the animals there was a reliable extension of the lifetime (nalorphine) and an increase in the survival rate (naloxone). In a lesser part of the animals the administration of nalorphine had no considerable influence on the outcomes. During the late period the therapeutic effect of these drugs has not yet been manifested. A conclusion has been made that th

Significance of production of peptide leukotrienes in murine traumatic shock.

We studied the formation of a leukotriene metabolite in plasma and bile during traumatic shock. Anesthetized rats subjected to Noble-Collip drum trauma developed a lethal shock state characterized by a survival time of 1.9 +/- 0.3 h, a 4.5-fold increase in plasma cathepsin D activity, and a reduction in mean arterial blood pressure to 45 +/- 2 mmHg compared with 108 +/- 5 mmHg in sham-shock controls. Plasma and bile samples were analyzed by reverse-phase high-pressure liquid chromatography (HPLC) for peptide leukotrienes (e.g., LTC4, LTD4, and LTE4), and their retention times were confirmed by

Beneficial effects of a neutral protease inhibitor in traumatic shock.

Eglin C is a polypeptide inhibitor of the neutral proteases elastase and cathepsin G. We have investigated its action in traumatic shock in rats. Eglin C (2 mg/kg) given following trauma prolonged survival time from 2.3 +/- 0.5 h to 3.6 +/- 0.4 h (p less than 0.05) in traumatized rats. Although eglin C treatment had no significant effect on the increase in plasma cathepsin D activity, eglin C administration significantly blunted plasma myocardial depressant factor (MDF) accumulation, 54 +/- 3 vs 79 +/- 8 U/ml (p less than 0.02). Our findings indicate a potential role for neutral proteases in t

Beneficial effect of a thromboxane synthetase inhibitor in traumatic shock.

Traumatic shock was induced in anesthetized rats using the Noble-Collip method. This resulted in an abrupt decline in mean arterial blood pressure (MABP) and heart rate. Plasma cathepsin D activity increased sixfold, plasma thromboxane B2 (TxB2) concentration increased 2.5-fold, plasma myocardial depressant factor (MDF) activity increased 3.5 fold, and the mean survival time was 1.4 +/- 0.2 hours. Administration of the selective thromboxane synthetase inhibitor 5-(3-pyridinylmethyl) benzofuran-2-carboxylate (U-63,557A) (4 mg/kg) resulted in a significant improvement in survival time, 3.3 +/- 0

[The influence of traumatic shock on alcohol elimination (author's transl)].

A group of 31 persons being under the influence of alcohol were examined. In traffic accidents they suffered craniocerebral (N = 16) or other body injuries (N = 15). The first group showed an average alcohol level decrease in blood of 0.24% per h (range 0.06--0.39), the second group an average of 0.22% (range 0.07--0.32). According to the previous investigations of Forster (1958) and Grüner (1958) it was assumed that the speed of alcohol elimination (beta 60) in the traumatic shock is increased.

Importance of thermoregulation in traumatic and haemorrhagic shock in the anaesthetized rabbit.

The effects of external thermal support was studied in 66 anaesthesized adult rabbits divided into six groups: control, traumatic shock and haemorrhagic shock; each of these groups had a subgroup with an external thermal support and a subgroup without support. During the four hour experimental period, anaesthesia alone resulted in a reduced core temperature and metabolic acidosis. The external thermal support attenuated these changes. In the shocked animals, the external thermal support reduced the mortality, assisted the body's thermoregulation and reduced the adverse metabolic effects of sho
