Surgical treatment of complex proximal humeral fractures in elderly patients: a matched-pair analysis of angular-stable plating vs. reverse shoulder arthroplasty.

BACKGROUND: To date, there is a lack of consensus regarding the type of surgical treatment for complex proximal humeral fractures (PHFs) in elderly patients, especially between joint preservation and joint replacement techniques. MATERIALS AND METHODS: We matched 60 patients (aged 73.2 ± 6.4 years) with complex PHFs in terms of sex, age, and Charlson Comorbidity Index, who underwent locking-plate open reduction-internal fixation (ORIF) or reverse shoulder arthroplasty (RSA) (2011-2017). Postoperative range of motion and functional outcome were assessed using the American Shoulder and Elbow Su

High tuberosity healing rate associated with better functional outcome following primary reverse shoulder arthroplasty for proximal humeral fractures with a 135° prosthesis.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a common treatment for proximal humeral fractures. (PHF) in the elderly. This study evaluates the functional outcome and the influence of. tuberosity healing (TH) following RSA with 135° humeral inclination and a neutral glenosphere without lateralization for PHFs. METHODS: In this retrospective case series, all patients with an acute PHF treated with primary RSA with 135° humeral inclination and a standard glenosphere without lateralization during a four-year period were followed up. Constant score (CS), patient satisfaction (subjective shou

Painful and weak shoulder following a generalized seizure: a missed odontoid fracture causing central cord compression-a case report.

BACKGROUND: Spinal fractures following seizures are reported but can be easily missed, as symptoms may be misinterpreted. Most seizure-induced spinal fractures occur in the thoracic and lumbar spine. Of all cervical spinal fractures, only 10% are odontoid fractures, making odontoid fractures following generalized convulsive seizures even rarer. Most odontoid fractures are unrecognized at the time of injury, as almost none are associated with neurological deficit at presentation. However, with time, it may result in incomplete or complete cervical cord compression owing to spinal instability, r

Clinical impact of scapular stress fracture after reverse total shoulder arthroplasty: a single-center propensity score-matchedanalysis.

BACKGROUND: Scapular stress fracture (SSF) is a recognized complication after reverse total shoulder arthroplasty (rTSA), yet its clinical impact remains controversial. This study aimed to determine the incidence of SSF after rTSA; to compare outcomes with a 1:3 propensity score‑matched control group; and to evaluate the influence of fracture location, treatment modality, and radiographic union on clinical outcomes. METHODS: A total of 551 consecutive primary rTSAs performed between January 2010 and December 2024 at a single tertiary referral center were retrospectively reviewed. Thirty patie

Constant-Murley score categorization to assess outcomes after reverse shoulder arthroplasty following complex proximal humeral fracture.

BACKGROUND: The Constant-Murley (CM) score is considered the gold standard of shoulder scoring systems. However, it does not include scoring categorization, and in some acute traumatic processes, a paired comparative analysis is not feasible. The purpose of this study was to determine cutoff values for the CM score that indicate a specific category on the outcome score following reverse total shoulder arthroplasty (rTSA) for fracture. METHODS: Consecutive patients with complex proximal humeral fracture who underwent rTSA from 2010 to 2022, were included in the study. Two years of postoperativ

Chronic Bilateral Anterior Shoulder Fracture-Dislocation Following Febrile Seizure After COVID-19 Vaccine: A Case Report.

Although the shoulder is one of the most commonly dislocated joints in the body, bilateral gleno-humeral joint dislocation is considered rare. Due to its complexity and paucity of cases reported in the literature, it represents both a diagnostic and therapeutic challenge. We report a rare case of an adolescent boy who suffered chronic bilateral anterior shoulder dislocations with proximal humerus fracture and Hill-Sachs lesion after febrile seizure following COVID-19 vaccination. An 18-year-old male presented with bilateral proximal humerus fracture with anterior shoulder dislocation following

Outcomes associated with secondary implant removal and arthrolysis in patients with a painful stiff shoulder after proximal humeral fracture fixation.

AIMS: Open reduction and plate fixation (ORPF) for displaced proximal humerus fractures can achieve reliably good long-term outcomes. However, a minority of patients have persistent pain and stiffness after surgery and may benefit from open arthrolysis, subacromial decompression, and removal of metalwork (ADROM). The long-term results of ADROM remain unknown; we aimed to assess outcomes of patients undergoing this procedure for stiffness following ORPF, and assess predictors of poor outcome. METHODS: Between 1998 and 2018, 424 consecutive patients were treated with primary ORPF for proximal h

Simultaneous bilateral reverse shoulder arthroplasty for bilateral four-part fracture of the proximal humerus in an elderly patient: A case report.

BACKGROUND: Proximal humeral fracture is the third most common fracture in elderly people after fractures of proximal femur and distal radius. They typically occur after low-energy trauma in women affected by osteoporosis, bilateral involvement is rare and usually with a simple pattern of fracture. Bilateral four-part proximal humerus fractures are even less frequent, with only a few reports published previously, with all of them caused by a seizure or electrocution in patients < 65 years old. CASE PRESENTATION: We present a 77-year-old right-handed female that sustained a bilateral simultane

Smoking status and the Disabilities of the Arm Shoulder and Hand score are early predictors of symptomatic nonunion of displaced midshaft fractures of the clavicle.

AIMS: This study identifies early risk factors for symptomatic nonunion of displaced midshaft fractures of the clavicle that aid identification of an at risk group who may benefit from surgery. METHODS: We performed a retrospective study of 88 patients aged between 16 and 60 years that were managed non-operatively. RESULTS: The rate of symptomatic nonunion requiring surgery was 14% (n = 13). Smoking (odds ratio (OR) 40.76, 95% confidence intervals (CI) 1.38 to 120.30) and the six week Disabilities of the Arm Shoulder and Hand (DASH) score (OR 1.11, 95% CI 1.01 to 1.22, for each point increas
